fa مقایسه اثر بخشی داروی خوراکی کلونیدین و ترانگزامیک اسید بر متغیرهای همودینامیک و میزان خونریزی حین عمل رینوپلاستی تحت بیهوشی عمومی: کارآزمایی بالینی Comparing the Effect of oral Clonidine and Tranexamic Acid on Bleeding During Rhinoplasty Under General Anesthesia: A Clinical Trial جراحي دهان وفک وصورت oral and maxillofacial surgery, OMFS پژوهشي-کامل Research(Original) سابقه و هدف: خونریـزی حیـن رینوپلاستی عارضـه‌ای اسـت کـه موجـب کاهـش دیـد جـراح گردیـده و خـود موجـب عوارضـی دیگـر خواهـد شـد. تاکنـون روش‌هـای مختلفـی بـرای کاهـش خونریـزی حیـن جراحی‌هـای بینـی پیشـنهاد شـده‌اند. این مطالعه با هـدف مقایسـه اثـرکلونیدین و ترانگزامیک اسید بـر میـزان خونریـزی بیمـاران کاندیـد عمل رینو پلاستی، انجام پذیرفت. مواد و روش‌ها: در این مطالعه کارآزمایی بالینی دو سورکور تصادفی، 120 نفر نفـر بیمـار کاندیـد جراحـی انتخابـی رینوپلاستی وارد مطالعـه شــدند. در گروه اول کلونیدین 3 میکروگرم به ازای هر کیلوگرم به صورت خوراکی 90 دقیقه قبل از انجام عمل جراحی تجویز شد و در گروه دوم کپسول ترانگزامیک اسید 250 میکروگرم براساس وزن بدن دو ساعت قبل از عمل جراحی به صورت خوراکی برای بیماران تجویز شد. در گروه کنترل مداخله‌ای صورت نگرفت. میزان خونریزی، پارامترهای همودینامیک و رضایت دید جراح مورد ارزیابی قرار گرفت. یافته‌ها: میانگین سن بیماران (5/35±28/67) سال و 96 نفر (80/0 درصد) زن بودند. از نظر تاثیر دارو و رضایت دید جراح بین گروه کلونیدین و ترانگزامیک اسید اختلاف معنی‌داری وجود دارد (0/055P=) به طوری که میانگین حجم خونریزی در گروه کلونیدین 0/47±112/0و در گروه ترانگزامیک اسید 0/36±132/0 می‌باشد. هم‌چنین گروه کلونیدین (0/008P=) و ترانگزامیک اسید (0/001P<) از نظر حجم خونریزی با گروه کنترل 28/1±195/0 اختلاف معنی‌داری داشتند. سه متغیر نوع دارو، وزن و مدت بیهوشی در مجموع می‌توانند 51 درصد از تغییرات (حجم خون) را تبیین و پیش بینی کنند. استنتاج: نتیجه مطالعه حاضر نشان داد که داروی کلونیدین می‌تواند بر کاهش خونریزی نسبت به ترانگزامیک اسید اثر گذارتر باشد و با ایجاد یک محیط نسبتا خشک و عاری از خونریزی سبب افزایش کیفیت دید محل عمل و کاهش مدت زمان جراحی رینوپلاستی گردد. شماره ثبت کارآزمایی بالینی :n60121229011923 IRCT Background and purpose: Bleeding during rhinoplasty is a complication that causes the surgeon to lose vision and will cause other complications. So far, various methods have been proposed to reduce bleeding during nose surgery. The present study aims to compare the effect of oral clonidine and tranexamic acid on intraoperative bleeding in patients undergoing rhinoplasty. Materials and methods: In this randomized, double-blind clinical trial, 120 patients who were candidates for elective rhinoplasty surgery were included. In the first group, three µg/kg of clonidine was administered orally 90 minutes before surgery. In the second group, 250 µg/kg bw tranexamic acid capsules were administered orally two hours before surgery. There was no intervention in the control group. Bleeding levels, hemodynamic parameters, and surgeons' visual satisfaction were evaluated. Results: The mean age of patients (28.67±5.35) years and 96 (80.0%) were women. Regarding the effect of drug and surgeon’s visual satisfaction, there is a significant difference between the clonidine and tranexamic acid groups (P=0.055); the mean bleeding volume in the clonidine group was 112.0±0.47 and in the tranexamic acid group was 132.0±0.36. In addition, the clonidine group (P=0.008) and tranexamic acid (P<0.001) showed a significant difference in terms of bleeding volume compared with the control group (195.0±28.1). The three variables of drug type, weight, and duration of anesthesia can explain and predict 51% of the changes (blood volume).
