fa ویژگی‌های اپیدمیولوژیک و مرگ و میر بیماران بستری شده مبتلا به SARS-CoV-2 در مازندران، ایران: نقش بیماری‌های زمینه‌ای Epidemiological Characteristics and Mortality Among Hospitalized Patients with SARS-CoV-2 Infection in Mazandaran Province, Iran: The Role of Underlying Diseases اپیدمیولوژی Epidemiology پژوهشي-کامل Research(Original) سابقه و هدف: بیماری کووید-۱۹ به‌عنوان یک بحران بهداشت عمومی جهانی، مرگ‌ومیر قابل‌توجهی به‌ویژه در بیماران بستری ایجاد کرده است. این مطالعه با هدف بررسی ویژگی‌های اپیدمیولوژیک، عوامل مرتبط با مرگ‌ومیر و نقش بیماری‌های زمینه‌ای در بیماران مبتلا به کووید-۱۹ بستری‌شده در بیمارستان‌های استان مازندران در سال ۱۳۹۹ انجام شد. مواد و روش ‌ها: در این مطالعه مقطعی، داده‌های 45/651 بیمار از سامانه مرکز پایش مراقبت‌های درمانی دانشگاه علوم پزشکی مازندران استخراج گردید. معیار ورود، تشخیص قطعی بر اساس آزمایش واکنش زنجیره‌ای پلیمراز (PCR) مثبت یا علائم بالینی و یافته‌های تصویربرداری سازگار بود. تحلیل آماری با استفاده از آزمون‌های کای‌دو، تی‌ مستقل و رگرسیون کاکس تک‌متغیره و چندمتغیره در نرم‌افزار SPSS نسخه ۲۵ انجام شد. یافته‌ها: میانگین سنی بیماران ۵۸ سال و بیشترین فراوانی مربوط به گروه سنی ۶۰–۶۹ سال (۲۱ درصد) بود. نرخ مرگ‌ومیر کلی حدود 8/8 درصد و در مردان (9/5 درصد) به‌طور معنی‌داری بالاتر از زنان (8/2 درصد) گزارش شد. بالاترین نرخ مرگ‌ومیر در افراد بالای ۸۰ سال (19/7 درصد)، بیماران مبتلا به نارسایی کلیوی (20/5 درصد)، بیماران مبتلا به سرطان و افراد بستری در بخش مراقبت‌های ویژه (32/5 درصد) مشاهده گردید. سطح اکسیژن خون کمتر از ۹۳ درصد با مرگ‌ومیر 19/5 درصد و لوله‌گذاری داخل تراشه (اینتوباسیون) با نرخ 51/3درصد همراه بود. رگرسیون کاکس چندمتغیره نشان داد که سن بالای ۵۰ سال، جنس مذکر، بیماری‌های زمینه‌ای (به‌ویژه نارسایی کلیوی، سرطان، بیماری قلبی، دیابت و بیماری مزمن ریوی)، سوءمصرف مواد و هیپوکسمی عوامل خطر مستقل مرگ‌ومیر هستند. استنتاج: سن بالا، جنس مذکر، بیماری‌های زمینه‌ای و اختلال عملکرد تنفسی مهم‌ترین پیش‌بینی‌کننده‌های مرگ در بیماران بستری مبتلا به کووید-۱۹ هستند. این نتایج به شناسایی زودهنگام گروه‌های پرخطر، بهینه‌سازی مدیریت بالینی و تخصیص منابع درمانی در همه‌گیری‌های مشابه کمک می‌کند.   Background and purpose: Coronavirus disease 2019 (COVID-19) emerged as a global public health crisis, resulting in substantial mortality, particularly among hospitalized patients. This study aimed to examine the epidemiological characteristics, factors associated with mortality, and the role of underlying diseases among patients with COVID-19 admitted to hospitals in Mazandaran Province, Iran, in 2020. Materials and methods: In this cross-sectional study, data from 45,651 patients were extracted from the Health Care Monitoring Center system of Mazandaran University of Medical Sciences. Inclusion criteria consisted of a confirmed diagnosis based on a positive polymerase chain reaction test or compatible clinical symptoms and imaging findings. Statistical analyses were performed using the chi-square test, independent samples t-test, and both univariate and multivariate Cox proportional hazards regression models using SPSS software, version 25. Results: The mean age of the patients was 58 years, with the highest proportion of cases (21%) occurring in the 60-69-year-old group. The overall mortality rate was approximately 8.8% and was significantly higher among male patients (9.5%) than female patients (8.2%). The highest mortality rates were observed in individuals aged over 80 years (19.7%), patients with renal failure (20.5%), those with cancer (......%?), and patients admitted to the intensive care unit (32.5%). Peripheral oxygen saturation below 93% was associated with a mortality rate of 19.5%, while intubation was associated with a mortality rate of 51.3%. Multivariate Cox regression analysis identified age greater than 50 years, male sex, underlying diseases (particularly renal failure, cancer, cardiac disease, diabetes, and chronic pulmonary disease), substance abuse, and hypoxemia as independent risk factors for mortality. Conclusion: Advanced age, male sex, underlying diseases, and impaired respiratory function constitute the principal predictors of death among hospitalized COVID-19 patients.
