This study aimed to determine one-year survival rate and discontinuation causes of IUD use in Kermanshah, Iran. Materials and methods: In this cohort study, data were collected from patients with IUD using a questionnaire containing demographic and IUD related variables. Patients were followed for one year or until the time that IUD removed. Then, the data were analyzed using Cox regression model. Kaplan-Meier method was used to determine the duration of IUD use. Results: The mean age of the 274 studied women was 28.2 ± 6.2 years. 15% of them had removed their IUD and the most common reason for withdrawal was related to bleeding and spontaneous removal. The highest IUD removal rate was occurred in the first 6 months and the mean duration of IUD use in people who had IUD removal was 4.2 ± 3.8 months. A significant correlation was found between previously used of IUD and IUD removal. So that, those who did not have a history of IUD use have a greater rate ratio (P = 0.04). In this study, the 6-month survival rate of IUD was 88.7% and one-year survival was 85%. Conclusion: The results show that bleeding and spontaneous removal is the main reasons for discontinuation of this method therefore, it is necessary to devise strategies to reduce removal rate and increase the time maintenance of IUD.
