fa بررسی قدرت پیش گویی CRP برای بروز CVA ایسکمیک و هموراژیک مجدد Relationship between C-Reactive Protein as a Prognostic Factor and Recurrent Ischemic and Hemorrhagic Strokes طب اورژانس Emergency Medicine پژوهشي-کامل Research(Original)  سابقه و هدف: سکته مغزی مهم‌ترین و شایع‌ترین بیماری نورولوژیک است. این مطالعه با هدف بررسی ارتباط CRP به عنوان عامل پروگنوستیک با CVA (cerebrovascular accident) ایسکمیک و هموراژیک، انجام پذیرفت. مواد و روش‌ها: این مطالعه از نوع کوهورت بوده و بیماران با سن بیشتر از 18 سال که با علائم CVA ایسکمیک و هموراژیک که طی 6 ساعت اول شروع علائم به اورژانس بیمارستان قائم (عج) شهر مشهد مراجعه کرده بودند، مورد بررسی قرار گرفتند. سطح CRP و همچنین تعیین پیش‌آگهی سطح سرمی CRP در تشخیص شدت آسیب‌های ایسکمیک و هموراژیک در بیماران CVA انجام شد. شدت بیماری CVA براساس مقیاس‌های MRS و NIHSS تعیین گردید. فالوآپ بیماران، 6 ماه بعد در نظر گرفته شد. یافته‌ها: جنسیت و سن در گروه‌های مطالعه تفاوتی نداشت (05/0P>). میانه نمره MRS و NIHSS بیماران هموراژیک به شکل معنی‌داری از بیماران ایسکمیک بیش‌تر بود. آنالیز سطح CRP در مدل رگرسیون تک متغیره نشان داد سطح CRP یک پیش‌آگهی بد در زمینه بروز سکته مغزی می‌باشد (149/0 Beta=، 027/0 P=). میانگین سنی، توزیع جنسیتی، نوع سکته، علائم و بیماری‌های زمینه‌ای در بین گروه مبتلا به سکته مجدد و بدون سکته مجدد تفاوت معنی‌داری نداشت (05/0 P>). از نظر آماری، اختلاف آماری از نظر بروز مجدد سکته مغزی بعد از 6 ماه و وضعیت CRP وجود داشت (009/0 P=). استنتاج: پایش سطح CRP می‌تواند در تشخیص به موقع و کنترل سکته مغزی مجدد موثر باشد.  Background and purpose: Stroke is the most common neurological disease. This study aimed to investigate the relationship between serum C-reactive protein (CRP) as a prognostic factor and ischemic and hemorrhagic cerebrovascular accident (CVA). Materials and methods: This cohort study was performed in patients over 18 years of age with symptoms of ischemic and hemorrhagic CVA attending the emergency department in Mashhad Ghaem Hospital during the first 6 hours of the onset of symptoms. CRP levels and prognosis of serum CRP levels were determined in diagnosing the severity of ischemic and hemorrhagic lesions in CVA patients. The severity of CVA was determined based on MRS and NIHSS scales. Patients' follow-up was considered 6 months later. Results: There were no significant differences between gender and age in study groups (P> 0.05). The median scores for MRS and NIHSS were significantly higher in hemorrhagic patients than ischemic patients. Analysis of CRP level in univariate regression model showed that CRP level is a bad prognosis for stroke (β = 0.149, P= 0.027). Mean age, gender distribution, type of stroke, symptoms, and underlying diseases were not significantly different between the group with and without stroke (P> 0.05). There were significant differences in CRP level and recurrence of stroke after 6 months between the groups studied (P= 0.009).
