fa شیوع پرفشاری ریوی پایدار نوزادان و عوامل مرتبط با آن در نوزادان رسیده متولد شده در جنوب شرق ایران سال 1399 Prevalence of Persistent Pulmonary Hypertension of the Newborn and Associated Factors in Term Neonates Born in Southeast of Iran, 2020 قلب کودکان pediatric cardiology پژوهشي-کامل Research(Original)  سابقه و هدف: نوزادان مبتلا به پرفشاری ریوی پایدار(PPHN) سه برابر بیش‌تر از سایر نوزادان در معرض بستری مجدد یا مرگ‌و‌میر پس از ترخیص هستند. این بیماری هزینه بالایی به والدین و نظام سلامت تحمیل می‌کند. در این مطالعه برای اولین بار شیوع و عوامل مرتبط با PPHNدر نوزادان رسیده متولد شده در جنوب شرق ایران بررسی شد. مواد و روش‌ها: در این پژوهش مقطعی پرونده بالینی هریک از 1859 نوزاد متولد‌شده در سال 1399 در بیمارستان افضلی پور کرمان بررسی شد. نوزادان با شرایط بستری از بدو ‌تولد، بیماری‌های مادرزادی قلب و دیگر تشخیص‌های افتراقی PPHN و وجود شواهد بیماری ژنتیکی از مطالعه حذف شدند. سپس نوزادان رسیده با شواهد بالینی PPHN تحت اکوکاردیوگرافی داپلر جهت تایید تشخیص نهایی قرار گرفتند. همچنین داده‌های مربوط به متغیرهای بارداری و زایمان، جنسیت نوزاد، وزن زمان تولد و آپگار دقیقه 1 و 5 و حمایت‌های تنفسی پیشرفته گردآوری شد. داده‌ها با نرم‌افزار آماری 25 SPSS تجزیه و تحلیل شدند. یافته‌ها: از مجموع 229 نوزاد واجد شرایط ورود به مطالعه، 31 نوزاد (11/9درصد) مبتلا به PPHN بودند. فراوانی PPHN با زایمان سزارین و آپگار دقیقه‌پنجم ارتباط آماری معنی‌داری داشت(0/05>P). ارتباط معناداری با سایر عوامل مادری و یا عوامل مربوط به نوزاد یافت نشد(0/05>P). میانگین میزان پارامترهای تعیین‌کننده پرفشاری شریان ریوی شامل جابجایی آنولوس‌تریکوسـپید، سرعت رگورژیتاسیون تریکوسپید، میانگین فشار و قطر شریان ریوی در گروه زایمان سزارین به‌صورت معنادار بیش‌تر از زایمان طبیعی بود(0/05>P). استنتاج: پیشنهاد می‌شود مطالعات تکمیلی آتی بر روی نوزادان جنوب شرق ایران به‌صورت چندمرکزی با بازه‌ زمانی طولانی‌تر و حجم نمونه‌ بیش‌تر اجرا شود تا بررسی دقیق ارتباط وقوع PPHN با عوامل مادری، جنینی و نوزادی فراهم گردد.    Background and purpose: Infants with persistent pulmonary hypertension (PPHN) are readmitted to hospital three times more than other infants or die after discharge. The disease imposes a high cost on families and health systems. In this study, for the first time, the prevalence of PPHN and associated factors were investigated in mature neonates born in southeast of Iran. Materials and methods: In this cross-sectional study, the clinical records of 1859 infants born in Kerman Afzalipour Hospital, 2020 were reviewed. Infants hospitalized from delivery, congenital heart disease, other differential diagnoses of PPHN, and evidence of genetic diseases were excluded from the study. Then, mature infants with clinical evidence of PPHN underwent Doppler echocardiography to confirm the final diagnosis. Information about pregnancy, childbirth, infant gender, birth, 1-minute and 5-minute Apgar scores, and advanced respiratory support were also obtained. Data were analyzed by SPSS V25. Results: Out of 229 neonates meeting the inclusion criteria, 31 (11.9%) had PPHN. Signification relationship was observed between frequency of PPHN and cesarean delivery and 5-minute minute Apgar score (P<0.05). But, no significant association was found between developing PPHN and other maternal or neonatal factors (P>0.05). The mean parameters of PPHN, including tricuspid annular plane systolic excursion (TAPSE), tricuspid regurgitation rate, mean pulmonary artery pressure, and pulmonary artery diameter were significantly higher in infants delivered by cesarean section than those born by normal vaginal delivery group (P<0.05).
