fa تحلیل روند و بررسی عوامل مرتبط با مرگ و میر ناشی از سوء مصرف مواد با استفاده از داده های ثبت شده ملی (1397-1393): مدل رگرسیون نقاط اتصال Substance-Related Mortality in Iran: A Retrospective Study of National Registered Data (2014-2018) Using Joinpoint Regression Analysis روانپزشکی psychiatry پژوهشي-کامل Research(Original) سابقه و هدف: به&lrm;منظور طراحی و اجرای مداخلات پیشگیرانه و درمانی سوءمصرف مواد، این مطالعه با هدف تحلیل روند و بررسی عوامل مرتبط با مرگ و میر ناشی از سوء&lrm;مصرف مواد، اجرا شد. مواد و روش‌ها: در این مطالعه مقطعی، داده&lrm;های مربوط به 6810 مرگ ناشی از سوء&lrm;مصرف مواد ثبت شده در سامانه نظام ثبت و طبقه‌بندی علل مرگ (تعریف شده با معیار ICD-10) و بر اساس گزارش وزارت بهداشت، درمان و آموزش پزشکی و سازمان پزشکی قانونی کشور بود که طی سال&lrm;های 1397-1393 جمع آوری شده  و شامل متغیرهای سن، جنس، نحوه مسمومیت و نوع ماده بود، مورد بررسی قرار گرفت. توصیف داده&lrm;ها و بررسی روند زمانی نوع ماده‌ی مصرفی با آزمون رگرسیون نقاط اتصال و محاسبه درصد تغییرات سالیانه(APC) و میانگین درصد تغییرات سالیانه (AAPC) با نرم&lrm;افزارهای SPSS23 و Joinpoint Regression Program4.9.00 انجام شد. یافته‌ها: میانگین سنی متوفیان 0/17±39/2 سال و درصد عمده&lrm; آن‌ها مردان بودند. بیش&lrm;ترین آمار فوت در مناطق مرکزی و سپس مناطق غربی ایران و مربوط به مسمومیت با مواد افیونی بود. مسمومیت تصادفی و سپس مسمومیت با قصد نامشخص فراوان&lrm;ترین علل بودند. طی این سال&lrm;ها در کل ایران، مرگ با مواد افیونی (0/008=P، 27/2=APC) و محرک&lrm;ها (0/1=P، 10/249=APC) روندی افزایشی داشت و یک نقطه خیز در سال 1393 را نشان داد. در دو گروه مسکن-آرام بخش-ضداضطراب&lrm;&lrm;ها و سایر مواد روند کاهشی یافت شد، اما از نظر آماری معنادار نبود. برای مرگ با مصرف الکل روندی مشاهده نشد. استنتاج: روند رو به رشد مرگ&lrm;های تصادفی، به خصوص ناشی از &lrm;مصرف مواد افیونی، به ویژه در میانسالان مرد قابل توجه است و نیاز به اقدامات لازم برای محدود کردن این روند دارد. تشدید نظارت&lrm;های دارویی و در دسترس قرار دادن و آموزش استفاده از نالوکسان به عنوان یک پادزهر اضطراری برای بیش مصرف بیش مواد افیونی به خانواده&lrm;ها، پلیس و اورژانس در مراحل اولیه مسمومیت با مخدرها پیشنهاد می&lrm;شود. Background and purpose: This study was carried out to analyze the trend of death due to substance abuse and related factors in Iran in order to design and implement preventive and therapeutic interventions for substance related deaths. Materials and methods: In this cross-sectional study, we investigated the data of 6810 deaths caused by drug abuse, registered in the "System of Registration and Classification of Causes of Death", defined by ICD-10 criteria and based on the reports of Ministry of Health and Legal Medicine Organization in 2014-2018. Variables including age, gender, type of poisoning and the type of substance leading to death were examined. Description of these data and examination of the time trend of the raw mortality rate were performed using joinpoint regression analysis and percentage of annual changes (APC) and average percentage of annual changes (AAPC) were calculated in SPSS V23. Results: <a name="_Hlk119970385">The average age of the deceased was 39.2±17.0 years. Majority of them were men. Central regions followed by western regions of Iran accounted for the highest number of deaths that was related to opioid poisoning. Accidental poisoning and poisoning with unknown intention were the most frequent causes. Deaths due to opioids (APC=27.2, P=0.008) and stimulants (APC=109.24, P=0.1) overdose showed an increasing trend and a peak in 2014 in Iran. A decreasing trend was found in two groups of sedative-hypnotic-anxiolytics and other drugs, but it was not statistically significant. No trends were found for alcohol-related deaths.</a> Conclusion: <a name="_Hlk119970290">There is a considerable increasing trend in accidental deaths, especially caused by opioids and mainly in middle-aged men which requires limiting measures.
