fa مقایسه اختلال عملکرد جنسی به دنبال جراحی پروستاتکتومی باز و TURP قبل و بعد از عمل Comparing Sexual Dysfunction Following Open Prostatectomy and Transurethral Resection of the Prostate جراحي کليه،مجاري ادراري و تناسلي(اورولوژی) urology پژوهشي-کامل Research(Original) سابقه و هدف: بزرگی پروستات بر روی پیشابراه فشار آورده و موجب مشکلات ادراری می&lrm;گردد که درمان آن، دارویی و جراحی می&lrm;باشد. از آن&lrm;جایی که اثرات جراحی برداشت پروستات بر عملکرد جنسی بیماران با بزرگی خوش&lrm;خیم پروستات هنوز مورد بحث محققین بوده و نتایج ضد و نقیض می&lrm;باشند، مطالعه حاضر با هدف مقایسه عملکرد جنسی بیماران پروستاتکتومی باز و TURP قبل و بعد از عمل انجام گردید. مواد و روش‌ها: این مطالعه مقطعی بر روی 100 بیمار دارای بزرگی خوش&lrm;خیم پروستات در سن 75-50 سال که سال 1398 در مراکز دانشگاهی شهر بابل براساس سایز پروستات تحت عمل جراحی قرار گرفتند، در دو گروه 50 نفری TURP و پروستاتکتومی باز انجام شد. عملکرد جنسی بیماران از طریق پرسشنامه استاندارد IIEF-15، قبل از پروسیجر و سپس 3 و 6 ماه پس از عمل در دو گروه ارزیابی و با استفاده از آزمون های آماری کای اسکوئر، تست دقیق فیشر و T-test و آنالیز واریانس تجزیه و تحلیل شدند. یافته‌ها: میانگین سنی بیماران در گروه TURP، 27/6±42/71 سال و در گروه پروستاتکتومی باز، 91/5 ± 40/73 سال بود. نمره کلی پرسشنامه IIEF5، قبل، سه و شش ماه بعد در گروه پروستاتکتومی باز به&lrm;ترتیب 82/10±40/37، 21/9±74/37 و 31/9 ± 22/38 و در گروه TURP به ترتیب نمرات، 62/7±40/23، 38/7 ± 34/37 و 11/7 ± 29/37 بود که اختلاف بین دو پروسیجر معنی&lrm;دار نبود(05/0<P). اختلال شدید عملکرد جنسی 6 ماه پس از عمل به روش TURP و باز به ترتیب در 5 نفر (10 درصد) و 8 نفر (16 درصد) دیده شد. اختلال عملکرد خفیف در روش TURP مشاهده نشد اما در روش باز در 2 نفر (4 درصد) دیده شد. استنتاج: نتایج مطالعه نشان داد که روش باز و TURP بر عملکرد جنسی تأثیری ندارد. Background and purpose: Enlarged prostate places pressure on the urethra and causes urinary problems, the treatment of which is medication and surgery. The effect of transurethral resection of the prostate (TURP) on sexual function is still controversial in patients with benign prostate enlargement. This study aimed at comparing sexual function in patients with open prostatectomy and TURP before and after surgery. Materials and methods: This cross-sectional study was performed in 100 patients with benign prostatic hyperplasia aged 50-75 years who underwent surgery in 2019 in hospitals affiliated with Babol University of Medical Sciences based on prostate size in two groups of 50 TURP (n=50) and open prostatectomy (n=50). Patients' sexual function was evaluated and compared between the two groups using the International Index of Erectile Function (IIEF-15), before and three and six months after the surgery. Data analysis was done applying Chi-square, Fisher's exact test, t-test, and analysis of variance. Results: The mean age in TURP group was 71.42 ± 6.27 and in open prostatectomy group was 73.40±5.91 years. The total scores for the IIEF-15 before, three, and six months after the open prostatectomy group were 37.40 ±10.82, 37.74 ±9.21, and 38.22 ± 9.31, respectively, and in TURP group were 23.40±7.62, 37.34±7.38, and 37.29±7.11, respectively. The scores showed no significant differences in sexual function between the two procedures (P>0.05). Intense sexual dysfunction was seen in five (10%) patients and eight (16%) patients, six months after TURP and open prostatectomy, respectively. Mild dysfunction was not observed after TURP, but it was seen in two (4%) patients after open prostatectomy.
