fa مقایسه بیان ایمونوهیستوشیمیایی پودوپلانین بین کارسینوم سلول سنگفرشی دهان و مخاط نرمال دهان Comparing Immunohistochemical Expression of Podoplanin between Oral Squamous Cell Carcinoma and Normal Oral Mucosa آسيب شناسي(پاتولوژی) Pathology پژوهشي-کامل Research(Original) سابقه و هدف: کارسینوم سلول سنگفرشی دهان (OSCC)، ششمین سرطان شایع در جهان است. پودوپلانین یک گلیکوپروتئین شبه موسینی در ضخامت غشا است که در اندوتلیوم لنفاوی بیان شده اما در اندوتلیوم عروق خونی بیان نمی‌شود. هدف از مطالعه حاضر، بررسی بیان ایمونوهیستوشیمیایی پودوپلانین و بررسی لنفانژیوژنز در کارسینوم سلول سنگفرشی دهان در مقایسه با مخاط نرمال دهان بود. مواد و روش‌ها: در این مطالعه آزمایشگاهی، ۴۰ بلوک پارافینه از بایگانی بخش پاتولوژی دهان، فک و صورت دانشکده دندانپزشکی بابل با تشخیص قبلی کارسینوم سلول سنگفرشی دهان و ۴۰ نمونه از مخاط نرمال دهان که طی جراحی دندان عقل نهفته به دست آمده بود با حداقل التهاب از دید بافتی و هیستوپاتولوژی، وارد مطالعه شدند و بیان ایمونوهیستوشیمیایی پودوپلانین در سلول‌های اپی‌تلیالی دو گروه و تعداد عروق لنفاوی در آن‌ها مورد ارزیابی قرار گرفت. جهت آنالیز داده‌ها از آزمون‌های Chi-sqaure، Mann-Whitney، Wilcoxon و Oneway ANOVA استفاده شد. یافته‌ها: اختلاف میانگین تعداد عروق لنفی در ناحیه تهاجم تومور در گروه OSCC (2/02±4/43) و در مرکز تومور (78/72±6/2) به لحاظ آماری معنادار بود (0/001>P). در بررسی بیان پودوپلانین از نظر کیفی بین دو گروه مشخص شد که در گروه OSCC بیان پودوپلانین در 23 مورد (57/5 درصد) بین 51 تا 100 درصد بوده است. در حالی که در گروه کنترل تنها 4 مورد (10 درصد) از نمونه‌ها این میزان بیان را داشته‌اند. بیان پودوپلانین در سلول‌های اپی‌تلیالی در کارسینوم سلول سنگفرشی به‌طور معنی‌داری بیش از مخاط نرمال دهان بود (0/001>P). استنتاج: نتایج مطالعه حاضر نقش پودوپلانین را در پاتوژنز کارسینوم سلول سنگفرشی دهان مطرح می‌نماید. Background and purpose: Oral squamous cell carcinoma (OSCC) is the sixth most common cancer in the world. Podoplanin is a mucin-like integral membrane glycoprotein that is expressed in the lymphatic endothelial cells but not in the blood vascular endothelial cells. The aim of the present study was to investigate the immunohistochemical expression of podoplanin and evaluating lymphangiogenesis in OSCC compared with normal oral mucosa. Materials and methods: In this laboratory study, 40 paraffin blocks archived in the Department of Oral and Maxillofacial Pathology in Babol Dental School with previous diagnosis of OSCC were used. Also, 40 samples of normal oral mucosa obtained during impacted wisdom tooth surgery with minimal inflammation from histological and histopathological point of view were included. Immunohistochemical expression of podoplanin in epithelial cells and the number of lymphatic vessels were evaluated in both groups. Chi-square, Mann-Whitney, Wilcoxon, and Oneway ANOVA were applied for data analysis. Results: Findings showed significant difference in the mean number of lymph vessels between the area of tumor invasion in OSCC group (4.43±2.02) and center of the tumor (6.78±2.72) (P<0.001). In the study of podoplanin expression in terms of quality, it was found that in the OSCC group, podoplanin expression was between 51% and 100% in 23 cases (57.5%), while these expression levels in the control group were seen only in four cases (10%). Podoplanin expression in epithelial cells in squamous cell carcinoma was significantly higher than normal oral mucosa (P<0.001).
