fa بررسی اثر افشانه محلول لیدوکائین 2 درصد با یک روش استفاده جدید در محل زخم بر میزان درد واضطراب بیماران مبتلا به سوختگی حاد در زمان تعویض پانسمان Analgesic and Anti-anxiety Effects of the "Direct Lidocaine Spraying with new Method on Burn Wound due to dressing change بیهوشی anesthesia پژوهشي-کامل Research(Original)    سابقه و هدف: تعویض مکرر پانسمان در سوختگی با دردهای شدید همراه است. در این مطالعه اثر افشانه لیدوکائین 2 درصد بر میزان درد و اضطراب بیماران هنگام تعویض پانسمان بررسی می‌گردد. مواد و روش‌ها: 168 بیمار زن و مرد 20 تا 40 سال دچار سوختگی حاد با در جه b 2 و 3 با وسعت 15-20 از بیمارستان شهید مطهری در سال 1397 انتخاب شدند. گروه مداخله افشانه داروی لیدوکائین و گروه شاهد افشانه آب مقطر دریافت نمود. معیار درد، VAS، معیار اضطراب، RASS و ضربان قلب در زمان‌های 0-3-10 و سپس هر ده دقیقه، ارزیابی گردید. یافته‌ها: میانگین ضربان قلب و فشارخون دیاستول، در دو گروه، تا دقیقه دهم بعد از مداخله، به‌ترتیب برابر 3±27/85 و 5±29/87 بود و در فواصل دقیقه صفر تا ده، در گروه کنترل، کم‌تر از گروه شاهد بود. فشار خون سیستولیک، ضربان قلب و میزان اضطراب در دقایق 10 تا 60 در گروه مداخله کم‌تر از گروه شاهد بود. در دقایق 10 و60 میانگین نمره درد و اضطراب در گروه مداخله کم‌تر از گروه شاهد بود. نمره درد در زمان‌های فوق 30/1±81/4، 93/0±43/6، 41/1±10/6 و 68/1±29/8 و نمره اضطراب 66/1±78/14، 12/2±14/17، 58/0±92/9 و 21/2±29/17 بود. استنتاج: یافته‌های این مطالعه نشان داد که اسپری لیدوکائین در غلظت مصرفی می‌تواند به طور قابل‌توجهی میزان درد و اضطراب را کاهش دهد بدون اینکه منجر به تغییر قابل توجهی در فشار خون سیستولیک و ضربان قلب بیماران شود.   Background and purpose: Burn patients often experience severe pain due to frequent dressing changes. The purpose of this study is to investigate the effect of 2% lidocaine spray on the level of pain and anxiety of patients during dressing change. Materials and methods: A total of 168 patients, including both males and females aged between 20 and 40 years, were selected from Shahid Motahari Hospital in the year 2017. The participants in the intervention group were given lidocaine spray, whereas the participants in the control group were given distilled water spray. Pain scale, Visual Analogue Scale (VAS), anxiety scale, Richmond Agitation-Sedation Scale (RASS), and heart rate were assessed at 0-3-10 minutes and subsequently every ten minutes. Results: The average heart rate and diastolic blood pressure in the two groups, up to the tenth minute after the intervention, were 85.27±3 and 87.29±5, respectively, and in the intervals from zero to ten minutes, in the control group, it was lower than the control group. The systolic blood pressure, heart rate and anxiety levels in the 10 to 60 minutes were lower in the intervention group than in the control group. In 10 and 60 minutes, the average score of pain and anxiety in the intervention group was lower than the control group. The pain score in the above times was 4.81±1.30, 6.43±0.93, 6.10±1.41, and 8.29±1.68 and the anxiety score was 14.78±1.66, 2.12. It was 17.14±14, 9.92±0.58 and 17.29±2.21.
