fa بررسی تاثیر تجویز وریدی آتروپین-نئوستیگمین بر پیشگیری از بروز سردرد بعد از بیهوشی نخاعی در جراحی سزارین: یک کارآزمایی بالینی تصادفی Effect of Intravenous Administration of Atropine-Neostigmine in Prevention of Headache after Spinal Anesthesia in Cesarean Section: A Randomized Clinical Trial بیهوشی anesthesia پژوهشي-کامل Research(Original) سابقه و هدف: شایع‌ترین روش بیهوشی در سزارین انتخابی، بیهوشی نخاعی است. از عوارض تاخیری پس از بیهوشی نخاعی، سردرد به دنبال پانکچر دورا می‌باشد. هدف از این مطالعه بررسی تاثیر تزریق وریدی ترکیب آتروپین و نئوستیگمین در پیشگیری از سردرد پس از بیهوشی نخاعی بود. مواد و روش‌ها: این مطالعه کارآزمایی بالینی دوسوکور شاهددار تصادفی شده روی 62 زن باردار کاندید سزارین الکتیو با بیهوشی نخاعی در بیمارستان شهیدبهشتی اصفهان انجام شد. بیماران به‌صورت تصادفی با Random allocation software در دو گروه مساوی تقسیم شدند. بعد از کلامپ بند ناف،  در گروه اول ترکیب mg 5/0 نئوستیگمین و mg 5/0 آتروپین (که با نرمال سالین به حجم 10 ml رسانده شد) و در گروه دوم ml10 نرمال سالین، تزریق وریدی شد. سپس بروز و شدت سردرد در 48 ساعت اول وتا یک هفته بعد از عمل در دو گروه تعیین و مقایسه شد. یافته‌ها: طی این مطالعه، 1بیمار از گروه آتروپین نئوستیگمن(A-N) و 5 بیمار از گروه نرمال سالین(S) دچار سردرد بعد از عمل شدند(2/3 در مقابل 1/16درصد) که می‌تواند از نظر بالینی مهم باشد(195/0=P). فراوانی بروز سردرد در ریکاوری در2 بیمار(1 نفر از هر گروه) و دربخش 4 نفرازگروه(S) بود(11/0=P). میانگین شدت سردرد در تنها بیمار گروه (A-N) 5 و در بیماران گروه(S) (45/0±2/4) بود(18/0=P). فاصله زمانی بین پانکچر دورا تا اولین بروز سردرد در بیمار گروه (A-N) یک ساعت و در بیماران گروه (S) (1/11±8/20) ساعت بود(18/0=P). استنتاج: استفاده ازتک دوز ترکیب نئوستیگیمن و آتروپین(هرکدام 5/0 میلی‌گرم) با کاهش بروز سردرد و شدت آن بعد از پانکچر دورا همراه است، که می‌تواند از نظر بالینی مهم باشد. شماره ثبت کارآزمایی بالینی : 1N20210324050762IRCT   Background and purpose: The most common method of anesthesia in Cesarean section is spinal anesthesia (SA). Post-Dural puncture headache (PDPH) is one of the complications of SA. The aim of this study was to evaluate the effect of intravenous administration of atropine and neostigmine in prevention of PDPH following Cesarean section. Materials and methods: This study was performed in 62 candidates of elective Cesarean section with SA in Isfahan Shahid Beheshti Hospital. Patients were randomly assigned into the two groups (n= 31 per group) using Random Allocation Software. After umbilical cord clamping, experimental group received neostigmine 0.5 mg and atropine 0.5 mg (i.v.), while the control group received 10 ml i.v. infusion of normal saline. The incidence and severity of headache were compared between the two groups in the first 48 hours and followed up for one week. Results: In this study, one patient in experimental group (3.2%) and five patients in control group (16.1%) reported PDPH which can be clinically important (P=0.195). Headache was seen in two patients (1 in each group) at recovery and in four at ward in control group (P=0.11). The mean severity of headache was 5 and 4.2±0.45 in experimental group and control group, respectively (P= 0.18). The interval between spinal anesthesia and first onset of headache was one hour in experimental group and 20.8 ± 11.1 hours in control group (P=0.18).
