fa یافته‌های آندوسکوپیک و پاتولوژیک دستگاه گوارش فوقانی در کودکان ۳ تا ۱۲ سال مبتلا به درد مزمن شکمی بدون علت مشخص Evaluation of Endoscopic and Pathological Findings of the Upper Gastrointestinal Tract in Children with Asymptomatic Chronic Abdominal Pain Aged 3 to 12 Years کودکان children گزارش کوتاه Brief Report سابقه و هدف: درد مزمن و یا راجعه شکم به صورت سه دوره درد شکمی که حداقل برای 3 ماه متوالی اتفاق بیفتد و به اندازه‌ای شدید باشد که بتواند در عملکرد طبیعی کودک تاثیر سوء بگذارد، تعریف می‌شود. وجود درد شکم می‌تواند یک شکایت چالش برانگیز برای بسیاری از متخصصین باشد، زیرا در عین حال که می‌تواند بیانگر یک یافته کاملا خوش خیم باشد، ممکن است بر وجود یک پاتولوژی حاد و بدخیم نیز دلالت کند. مهم‌ترین اقدام پزشک معالج در این مرحله تشخیص ارگانیک و یا عملکردی بودن بیماری می‌باشد. مطالعه حاضر با هدف ارزیابی عملکرد تشخیصی آندوسکوپی دستگاه گوارش فوقانی در تشخیص علل ارگانیک درد مزمن شکم در کودکان و بررسی ارتباط علایم بالینی با یافته‌های پاتولوژیک آندوسکوپی در کودکان 3 تا 12 سال مبتلا به درد مزمن شکمی انجام شده است. مواد و روش‌ها: پژوهش حاضر به صورت یک مطالعه مقطعی بر روی 100 کودک مبتلا به درد مزمن شکم بدون علت پاتولوژیک، که به کلینیک کودکان بیمارستان آیت‌الله موسوی زنجان مراجعه کرده بودند، انجام شد. در ابتدا پس از شرح حال اولیه وبررسی علایم بالینی بیمار، آزمایشات تشخیصی و پاراکلینیکی جهت تعیین علت درد مزمن شکم صورت‌گرفت. درصورت تشخیص علت درد با استفاده از بررسی‌های کلینیکی و پاراکلینیکی، بیمار از مطالعه خارج و درمان متناسب با تشخیص برای وی انجام شد. سپس کودکانی که علت مشخصی برای درد شکم آن‌ها یافت نشد جهت انجام آندوسکوپی به پزشک فوق تخصص گوارش اطفال ارجاع داده شدند. پس از بررسی و مشاهده کامل دستگاه گوارش فوقانی یک نمونه پاتولوژی نیز برای تمام بیماران تهیه شد. یافته‌های آندوسکوپی، پاتولوژی، دموگرافیک و بالینی برای تمام بیماران ثبت شد. داده‌ها در نهایت توسط نرم‌افزار 26SPSS با استفاده از تست‌های آماری کای دو، تست دقیق فیشر، تی تست و من-ویتنی مورد تجزیه و تحلیل آماری قرارگرفت. یافته‌ها: در این مطالعه، میانگین و  انحراف معیار سنی کودکان 2/66±8 سال بود. 63 درصد از آن‌ها دختر و 75 درصد ساکن شهر زنجان بودند. شایع‌ترین شکایت در کودکان بعد از درد شکم، استفراغ (19 درصد) و تهوع (14 درصد) و عدم پاسخ به درمان سرپایی (8 درصد) بود. شایع‌ترین علایم همراه با درد مزمن شکمی به‌ترتیب استفراغ (32 درصد)، پوسیدگی دندان(27 درصد) و یبوست(22 درصد) بود. یافته‌های آندوسکوپی تنها در دو بیمار مبتلا به درد شکم نرمال بود و در 98 درصد حداقل یک ضایعه ارگانیک وجود داشت. شایع‌ترین یافته‌ها در بررسی پاتولوژی به‌ترتیب ریفلاکس معدوی- مروی(GERD I) با 48 درصد فتق هیاتال لغزشی (Small sliding hiatal hernia) با 37 درصد و GERD II (24 درصد) بودند. استنتاج: نتایج پژوهش حاضر نشان می‌دهد که اغلب دردهای مزمن شکمی در کودکان منشا ارگانیک دارد و بهتر است بررسی‌های تکمیلی جهت یافتن علت درد صورت گیرد. شایع‌ترین یافته پاتولوژیک در این کودکان  GERD I, IIو فتق هیاتال لغزشی بود. تظاهرات بالینی بیماری، تنوع علائم بالینی، یافته‌های پاراکلینیکی، تست‌های اولیه و نتایج بیوپسی مخاطی می‌تواند در تشخیص علت درد مزمن کمک‌کننده باشد، لذا با یافتن علت درد می‌توان اقدامات موثر جهت کنترل درد این‌کودکان انجام داد.   Background and purpose: Chronic abdominal pain or recurrent abdominal pain is defined as abdominal pain that occurs in at least three episodes, lasting for a minimum of three consecutive months, and is severe enough to adversely affect the child's normal functioning. Abdominal pain can be a challenging complaint for many specialists because, at the same time, it can indicate a completely benign finding, it can also indicate the presence of an acute and malignant pathology. At this stage, the most important action of the treating physician is to diagnose the disease as organic or functional. The present study was conducted to evaluate the diagnostic performance of upper gastrointestinal endoscopy in diagnosing organic causes of chronic abdominal pain in children and investigating the correlation between clinical symptoms and pathological findings of endoscopy in children aged 3 to 12 years with chronic abdominal pain.  Materials and methods: The present study was conducted as a cross-sectional study on 100 children with chronic abdominal pain without pathological cause who visited the pediatric clinic of Ayatollah Mousavi Hospital in Zanjan, from April to December 2021. At first, after taking the initial history and examining the clinical symptoms of the patient, diagnostic and paraclinical tests were performed to determine the cause of chronic abdominal pain. If the cause of the pain was diagnosed using clinical and paraclinical examinations, the patient was excluded from the study and treated accordingly. Then, the children who could not find a specific cause for their pain were referred to a pediatric gastroenterology specialist for endoscopy. After a complete examination and observation of the upper gastrointestinal tract, a pathology sample was also prepared for all patients, and endoscopic, pathological, demographic, and clinical findings were recorded for all patients. Finally, the data was statistically analyzed by SPSS version 26 software using Chi-2 statistical tests, Fisher's exact test, t-test, and Mann-Whitney. Results: The mean ± standard deviation of the children's age was 8±2.66 years. 63% of the children were girls and 37% were boys. The most common complaints in children after abdominal pain were vomiting (19%) nausea (14%) and non-response to outpatient treatment (8%). The most common accompanying symptoms besides abdominal pain included nausea, vomiting, tooth decay, and constipation. Endoscopic findings were normal in only two patients with abdominal pain, and in 98% there was at least one organic lesion. The most common findings in pathology examination were GERD I at 48%, Small sliding hiatal hernia at 37%, and GERD II at 24% respectively. Conclusion: Our findings indicated that most cases of chronic abdominal pain in children had an organic origin, and it was better to conduct further investigations to identify the cause of the pain. The most common pathological findings in these children were GERD I, II, and sliding hiatal hernia. Clinical manifestations of the disease, diverse clinical symptoms, paraclinical findings, initial tests, and mucosal biopsy results can help diagnose the cause of chronic pain.
