fa بررسی یافته‌های بالینی، اپیدمیولوژیک و آزمایشگاهی بیماران مبتلا به CCHF در استان مازندران در سال‌های 97-1393 Clinical, Laboratory, and Epidemiological Findings in Patients with Crimean-Congo Hemorrhagic Fever in Mazandaran Province, 2014-2018 عفوني infection گزارش کوتاه Brief Report  سابقه و هدف: تب هموراژیک کنگو-کریمه (CCHF) یکی از تب‌های خونریزی دهنده ویروسی است، که توسط ویروس‌های گروه نایرو (Nairovirus group) ایجاد می‌شود. بررسی اپیدمیولوژیک و علایم بالینی بیماری در هر منطقه می‌تواند به شناسایی کانون‌های مستعد بیماری و تشخیص اولیه بیماری در موارد همه‌گیری کمک کند، لذا هدف از این مطالعه بررسی علایم بالینی، اپیدمیولوژیک و آزمایشگاهی بیماران مبتلا به CCHF در استان مازندران در سال‌های 97-1393 می‌باشد. مواد و روش‌ها: جمعیت مورد بررسی در این مطالعه مقطعی اپیدمیولوژیک، بیماران بستری شده درکل بیمارستان‌های استان مازندران در سال‌های 97-1393 بود که تشخیص CCHF در آن‌ها تأیید شده بود. 135 نفر به‌صورت سرشماری انتخاب شدند و سپس علائم بالینی و خصوصیات اپیدمیولوژیک و یافته‌های آزمایشگاهی بیماران مذکور مورد بررسی قرار گرفت. یافته‌ها: در مطالعه حاضر، 23درصد بیماران سن زیر 30 سال داشتند و 80/7 درصد آن‌ها، مرد بودند. در 33/3 درصد از موارد، افراد دارای مشاغل پرخطر برای ایجاد CCHF بودند و در 39/3 درصد از موارد سابقه تماس اخیر با دام وجود داشت. فصل ایجاد بیماری در 45/9 درصد بهار، 42/2 درصد تابستان، 5/9 درصد پاییز و 5/9 درصد زمستان بود. علائم بیماران مورد بررسی شامل 88/1 درصد تب، 11/1 درصد راش پوستی، 23 درصد درد شکمی، 28/9 درصد خونریزی، 7/4 درصد اپیستاکسی، 52/6 درصد سردرد، 69/7 درصد دردهای عضلانی- اسکلتی بود و سرفه و منوراژی در هیچ بیماری مشاهده نشد. ترومبوسیتوپنی در 51/9 درصد بیماران وجود داشت و در مورد نتایج سایر آزمایشات بیماران در 29/6 درصد هیچ یافته مثبتی نبود، اما در 13/3 درصد افزایش آنزیم‌های کبدی، 5/2 درصد آنمی، 27/4 درصد لکوپنی، 9/6 درصد لکوسیتوز، 3/7 درصد افزایش آنزیم‌های کبدی همراه با آنمی، 11/1 درصد افزایش آنزیم‌های کبدی همراه با لکوپنی مشاهده شد. استنتاج: با توجه به یافته‌های به‌دست آمده، چنین استنتاج می‌شود که خصوصیات بالینی و اپیدمیولوژیک و آزمایشگاهی CCHF در بیماران بستری شده در بیمارستان‌های استان مازندران مشابه سایر مطالعات انجام شده در این زمینه می‌باشد. با این وجود بررسی دوره‌ای این یافته‌ها در بیمارن توصیه می‌گردد.    Background and purpose: Crimean-Congo hemorrhagic fever (CCHF) is one of the viral hemorrhagic fevers caused by Nairovirus group. Epidemiological investigation and clinical symptoms of the disease in each region can help to identify disease-prone foci and early diagnosis of the disease in epidemic cases. Therefore, the purpose of this study was to investigate the clinical, epidemiological, and laboratory symptoms of patients with CCHF in Mazandaran province in 2014-2018. Materials and methods: In this cross-sectional epidemiological survey, 135 patients with proven CCHF in Mazandaran province were enrolled in a census manner. The clinical, epidemiological, and laboratory findings in these patients were determined. Results: In this study, 23% were younger than 30 years of age and 80.7% were men. In hospitalized patients, 33.3% had high risk jobs linked to the disease and 39.3% had livestock exposure. The patients were most affected in spring (45.9%) and summer (42.2%) and less during fall (5.9%), and winter (5.9%). Clinical manifestations, including fever, rash, abdominal pain, bleeding, epistaxis, headache, musculoskeletal pain, cough, and menorrhagia were observed in 88.1%, 11.1%, 23%, 28.9%, 7.4%, 52.6%, 69.7%, 0%, and 0% of the infected subjects, respectively. Thrombocytopenia were seen in 51.9% of the patients and about other laboratory results there were no any specific findings in 29.6%. According to laboratory findings, LFT raise, anemia, leukopenia, leukocytosis, LTF raise plus anemia, and LFT raise plus leukopenia were seen in 13.3%, 5.2%, 27.4%, 9.6%, 3.7%, and 11.1%, respectively.
