fa آسیب کلیوی حاد و بیماری مزمن کلیوی اصلی‌ترین عامل خطر مرگ‌ومیر بیماران بستری‌شده به علت COVID-19 Severe Acute Kidney Injury and Chronic Kidney Disease are Paramount Risk Factors for Mortality in Hospitalized Cases with COVID-19 نفرولوژی Nephrology پژوهشي-کامل Research(Original) سابقه و هدف: گونۀ جدید ویروس کرونا (COVID-19) می‌تواند موجب آسیب حاد کلیه (AKI) شود و بیماران دارای بیماری زمینه‌ای کلیه پروگنوز بدتری دارند. در این مطالعه، یافته‌های بالینی و آزمایشگاهی بیماران مبتلابه کووید-۱۹ و عوامل خطر مرگ و بستری در بخش مراقبت‌های ویژه (ICU) ارزیابی شده است. مواد و روش‌ها: این مطالعه مشاهده‌ای بر روی ۲۹۶ بیمار مبتلابه کووید-۱۹ با و بدون بیماری زمینه‌ای کلیوی انجام شده است. از مجموع ۲۹۶ مورد بیمار بستری‌شده، ۳۷ نفر فوت کردند (میانگین سنی: 66/6±17/6). یافته‌ها: شایع‌ترین علائم اولیۀ بیماران فوت‌شده این موارد بود: تنگی نفس (37/8 درصد)، سرفه (21/6 درصد)، سن بالا، استفاده از انسولین، بیماری دستگاه عصبی مرکزی (CNS)، شوک عفونی، هیپوآلبومینمی (0/0001>P) و میزان بالای INR (0/0001>P). میانۀ زمان مرگ موارد بستری‌شده در ICU بیش‌تر از بیماران فاقد بیماری مزمن کلیه (CKD) بود (OR 3/08، 0/006= P). در افراد فوت‌شده از آزیترومایسین و سفتریاکسون به میزان کم‌تر و بیش‌تر از مروپنم استفاده شده بود (0/0001>P). استنتاج: این مطالعه نشان می‌دهد که خطر مرگ‌ومیر و بستری در ICU در بیماران دارای CKD بیش‌تر از بیماران فاقد CKD است. ارزیابی اثرات درمانی آزیترومایسین و سفتریاکسون و عوارض جانبی رسیژن و مروپنم در بیماران فوت‌شدۀ مبتلابه کووید نشان داده شد. اضافه بر این، نشان داده شد که آلبومین پایین، INR، نوتروفیل و گلبول‌های سفید (WBC) بالا به‌طور قابل‌توجهی خطر مرگ‌ومیر این بیماران را افزایش می‌دهد. Background and purpose: Novel coronavirus (COVID-19) can cause acute kidney injury (AKI), and underlying kidney diseases worsen the prognosis of COVID-19 patients. Materials and methods: This observational study examined the clinical and laboratory outcomes of patients with COVID-19 and assessed the major risk factors for death and intensive care unit (ICU) hospitalization in COVID-19 patients. The sample consisted of 296 COVID-19 patients with or without underlying kidney diseases. In total, 37 patients died (mean age: 66.6 ± 17.6). Results: Most patients who passed away initially suffered from shortness of breath (37.8%), cough (21.6%), old age, insulin use, central nervous system (CNS) disease, septic shock, hypoalbuminemia (P<0.0001), and high international normalized ratio (P<0.0001). Median time to death was higher in patients hospitalized in the ICU than in those without chronic kidney disease (OR=3.08; P=0.006). The use of azithromycin and ceftriaxone was lower than the use of Recigen and meropenem in deceased people (P<0.0001). Conclusion: The risk of mortality and ICU hospitalization was higher in patients with CKD than in those without CKD. The results revealed the therapeutic effects of azithromycin and ceftriaxone and the side effects of Recigen and meropenem in deceased COVID-19 patients.
