fa بررسی میزان درگیری سی تی اسکن ریه بیماران کووید ۱۹ و ارتباط آن با سطح بیومارکرهای خونی: یک مطالعه گذشته نگر Investigating the Level of Lung Involvement by CT scans in Covid-19 Patients and its Relationship with the Level of Blood Biomarkers: A Retrospective Study عفوني infection گزارش کوتاه Brief Report سابقه و هدف: بیماری کووید 19 یک بیماری با درگیری چند ارگانی می‌باشد که نقش آبشار انعقادی، سیستم ایمنی و التهابی در آن بسیار پر رنگ است. این مطالعه با هدف بررسی ارتباط بیومارکرهایی هم‌چون N-ترمینال پروهورمون پپتید ناتریورتیک مغز (NT-proBNP)، لاکتات دهیدروژناز (LDH)، اینترلوکین 6 (IL-6) و D-dimer با میزان درگیری ریه در بیماران بستری مبتلا به کووید 19 می‌باشد. مواد و روش‌ها: این مطالعه گذشته نگر، در خرداد، تیر و مرداد 1400 در بیمارستان امام سجاد شهر رامسر مازندران انجام گردید که جامعه‌ی آماری آن بیماران بستری مبتلا به کووید 19 با درگیری ریوی بودند. سی‌تی اسکن ریه هر بیمار مورد بررسی قرار گرفت و به میزان درگیری هر ریه نمره 10-0 داده شد. هم‌چنین، علاوه بر اطلاعات دموبیوگرافیک، سطوح بیومارکرهای IL-6، NT-proBNP، D-dimer و LDH از پرونده بیماران استخراج گردید و داده‌های حاصل تحت آنالیز آماری قرار گرفتند. یافته‌ها: 154 بیمار وارد مطالعه شدند. براساس اعداد به‌دست آمده از نمره سی‌تی اسکن، 42 نفر (27/3 درصد) درگیری خفیف، 52 نفر (33/7درصد) درگیری متوسط و 60 نفر (39 درصد) درگیری شدید ریه داشتند. سطوح بیومارکرهای IL-6، D-dimer و LDH در سه گروه یاد شده بر اساس نمره سی تی اسکن دارای تفاوت آماری معنی‌داری بود. هم‌چنین برای نمره محاسبه شده شدت درگیری ریه در سی‌تی‌اسکن و سطوح پلاسمایی IL-6 (0/008P=  و 0/280r=)، LDH (0/001P< و 0/032 r=) و D-dimer (0/008P=  و 0/307 r=) همبستگی مثبت و معنی‌داری یافت گردید. استنتاج: نتایج حاصل از این مطالعه نشان داد که بیومارکرهایی هم‌چون IL-6، D-dimer و LDH می‌توانند با میزان آسیب ریوی بیماری کووید 19 در ارتباط باشند.   Background and purpose: Coronavirus disease 2019 (COVID-19) is a multi-organ illness characterized by significant involvement of the coagulation cascade, immune response, and inflammation. This investigation aimed to explore the significance of various biomarkers, such as interleukin 6 (IL-6), lactate dehydrogenase (LDH), D-dimer, and N-terminal prohormone of brain natriuretic peptide (NT-proBNP), concerning lung complications in COVID-19 patients. Materials and methods: This retrospective study was conducted at Imam-Sajjad Hospital in Ramsar, Mazandaran, from May to August 2021 and focused on all COVID-19 patients with lung involvement who were admitted. An experienced radiologist assessed each patient's computed tomography (CT) scans and assigned a lung involvement score ranging from zero to 10 for each lung. These scores were categorized as mild (0-10), moderate (10-15), or severe (15-20). In addition to demographic data, serum levels of IL-6, LDH, D-dimer, and NT-proBNP were obtained from patient records, and all the achieved data were statistically analyzed. Results: A total of 154 patients were included. The distribution across mild (27.3%), moderate (33.7%), and severe (39%) lung involvement in COVID-19 was observed. Notably, levels of IL-6 (P=0.002), D-dimer (P=0.035), and LDH (P<0.001) exhibited variations among these three groups of COVID-19 lung involvement. Furthermore, statistically significant positive correlations were observed between the calculated lung involvement score and plasma levels of IL-6 (P=0.008, r= 0.280), D-dimer (P=0.008, r= 0.307), and LDH (P<0.001, r= 0.320).
