fa مقایسه‌ تاثیر کرم بابونه ( Matricaria chamomilla L) و کالندیت ‌ای بر روند التیام زخم جراحی در موش صحرائی Comparison of the Effects of Matricaria Chamomilla L and Calendite-E Cream on the Healing Process of Surgical Wounds in Rats جراحی عمومی General surgery پژوهشي-کامل Research(Original) سابقه و هدف: تسریع درفرآیند التیام زخم اهمیت زیادی دارد. زخم‌های پوستی با آسیب سلولی همراه هستند و می‌توانند بر عملکرد فیزیولوژیک پوست تاثیر بگذارند. فرایند التیام زخم چند مرحله‌ای است که با استفاده از سلول‌ها، سایتوکین‌ها و سیستم عروقی رخ می‌دهد. فرایند التیام زخم در فازهای هموستاز، التهاب، پرولیفراسیون و تکثیر سلولی و اصلاح ساختار و شکل پوست انجام می‌گیرد. استفاده از فراورده‌های گیاهی در ترمیم زخم‌ها به دلیل خواص ضد التهابی، ضد باکتریایی و آنتی‌اکسیدانی آن‌ها مورد توجه محققین قرار دارد. در طب سنتی ایران به‌طور جداگانه برای مصارف پوستی بابونه و هم‌چنین سرخارگل (Echinacea purpurea) و گل همیشه بهار (Calendula officinalis)شواهدی وجود دارد، ولی اثرات هم‌افزایی این دو فراورده بر التیام زخم مشخص نیست. لذا این مطالعه با هدف مقایسه‌ تاثیر کرم بابونه (Matricaria chamomilla L) و کرم کالندیت-‌ای (Calendite-E) که ترکیبی از گل همیشه بهار و سرخارگل می‌باشد بر روند التیام زخم جراحی انجام شد. مواد و روش‌ها: این مطالعه مداخله‌ای- تجربی بر روی 32 سر موش صحرائی نر انجام شد. رت‌ها بعد از بیهوشی و ایجاد زخم جراحی به چهار گروه 8 تایی کنترل (بدون مداخله)، بابونه (کامیل کرم) حاوی 5/0 درصد اسانس و 5 درصد عصاره بابونه، کالندیت-‌ای با ترکیب و نسبت عصاره هیدروالکلی گل‌های تازه گیاه همیشه بهار (1 گرم) و عصاره هیدروالکلی سرخارگل(اکیناسه) (1/1 گرم) و کنترل (اوسرین) تقسیم شدند. در سه گروه مداخله و کنترل پایه (اوسرین) بعد از تمیز کردن زخم با نرمال سالین، کرم‌ها روزانه به‌صورت یک لایه نازک بر روی سطح کامل زخم با آبسلانک مالیده می‌شد و برای گروه بدون مداخله کنترل بعد از تمیز کردن زخم با نرمال سالین اقدام خاصی صورت نمی‌گرفت. اندازه‌گیری مساحت زخم در روزهای 0، 3، 7 و 21 با استفاده از نرم‌افزار IC measure انجام شد. مساحت زخم به ترتیب در روزهای 0، 3، 7 و 21 و یافته‌های هیستوپاتولوژیکی در 7و21 روز مقایسه شدند. میانگین و انحراف‌معیار مساحت زخم و امتیاز هیستوپاتولوژیک بین گروه‌ها با آزمون ANOVA و کروسکال والیس و آزمون تعقیبی Dunn مقایسه شدند. از نرم‌افزار SPSS 26 جهت تحلیل داده‌ها استفاده شد. یافته‌ها: مساحت زخم در روز سوم در بین گروه‌ها تفاوت نداشت ولی در روز هفتم، تفاوت معنی‌داربود (0/020P=). میانگین مساحت زخم در گروه کالندیت-ای نسبت به گروه کنترل (0/008P=) و گروه اوسرین (0/019P=) به‌طور معنیداری کم‌تر بود. التیام زخم در روز بیست ویکم فقط در کالندیت-ای و کامیل کرم به‌طور کامل حاصل شد. شاخص‌های هیستوپاتولوژی در روز هفتم در گروه کامیل کرم از نظر بازسازی سلول‌های پوششی نسبت به گروه‌های دیگر بهتر بود(038/0=P). در روز بیست و یکم از نظر بازسازی سلول‌های پوششی و روند فیبروپلازی گروه کالندیت-ا‌ی و از نظر گستردگی واکنش فیبروتیک، میزان ادم و آماس و ارزیابی خونریزی هم کالندیت-ای و هم کامیل کرم تفاوت معنی‌داری با گروه کنترل داشت (0/05P<). استنتاج: کالندیت-ای و کامیل کرم بروی فاکتورهای التیامی زخم اثرات مثبتی داشتند و عنایت به در دسترس و ارزان بودن آن، به‌عنوان فراورده گیاهی بدون عارضه و مناسب، برای بهبود زخم پیشنهاد می‌شود.   Background and purpose: Accelerating the wound healing process is of paramount importance. Cutaneous wounds are associated with cellular damage and can impair the physiological function of the skin. Wound healing is a multifaceted process that involves cells, cytokines, and the vascular system. This process occurs in distinct phases: hemostasis, inflammation, proliferation, and remodeling of the skin's structure and architecture. The utilization of plant-derived products in wound healing has garnered significant interest among researchers due to their anti-inflammatory, antibacterial, and antioxidant properties. In Persian traditional medicine, there is evidence supporting the topical application of chamomile (Matricaria chamomilla L.), Echinacea purpurea, and Calendula officinalis individually; however, the synergistic effects of these two products on wound healing remain unclear. Therefore, this study aimed to compare the efficacy of chamomile cream and Calendite-E cream, a combination of marigold and Echinacea, on the healing process of surgical wounds. Materials and methods: 32 male rats were divided into four groups of 8: control, Kamil Cream, Calendit E, and Oserin. Wound area and histopathological findings were examined and compared on days 0, 3, 7, 21, and 7 and 21, respectively. Among the many properties of Matricaria chamomilla L in Persian traditional medicine, its anti-inflammatory, antibacterial, and antioxidant properties are confirmed. 32 male rats were divided into four groups (8 rats in each group) including the control, chamomilla cream containing 0.5% essential oil, and 5% chamomile extract, calendar-E and control were divided with Oserin base cream. The wound area was compared on days 0, 3, 7, and 21, and histopathological findings on 7 and 21 days, respectively. Results: There was no difference between the groups on the wound area on the third day, but on the 7th day, there was a significant difference (P=0.020). So the average wound area in the Calendit E group was significantly lower than the control group (P=0.008) and also compared to the Oserin group (P=0.019). Unlike other groups, wound healing was completely achieved on the 21st day in Calandit E and Kamil Cearm. In terms of histopathological indicators, on the seventh day, in the Kamil cream group, the restoration of the epithelial cells was better than the other groups (P=0.038) on the 21st day, in terms of regeneration of the lining cells and the process of fibroplasia of the Calendit-E group, and in terms of the extent of the fibrotic reaction, the amount of edema and swelling, and the assessment of both Calendit-E and Kamil cream bleeding, it had a significant effect (P<0.05).
