fa بررسی فراوانی ژن های مقاومت میکروبی مولد متالوبتالاکتاماز در ایزوله های پسودوموناس ائروژنوزا در بیمارستان سوختگی و روانپزشکی زارع، ساری Investigation the Frequency of Metallo-Beta-Lactamase-Producing Microbial Resistance Genes in Pseudomonas Aeruginosa Isolates in Zare Burn and Psychiatric Hospital, Sari ميکروبيولوژي Microbiology گزارش کوتاه Brief Report سابقه و هدف: پسودوموناس آئروژینوزا یکی از عوامل مهم در ایجاد عفونت‌های بیمارستانی مانند عفونت ادراری، باکتریمی، پنومونی و عفونت زخم سوختگی است. با توجه به اهمیت ویژه کارباپنم‌ها در درمان عفونت‌های مقاوم به آنتی‌بیوتیک‌های مرسوم و همچنین تجویز غیر منطقی و تجربی آنتی‌بیوتیک‌ها لازم است شیوع و الگوی مقاومت به آنتی‌بیوتیک‌ها در پاتوژن‌های ایجاد کننده عفونت‌های بیمارستانی تعیین گردد تا گسترش مقاومت میکروبی، میزان مرگ و میر ناشی از عفونت‌های بیمارستانی به حداقل برسند. لذا مطالعه حاضر با هدف تعیین فراوانی ژن‌های متالوبتالاکتاماز در ایزوله های پسودوموناس ائروژنوزا در در بیمارستان سوختگی و روانپزشکی زارع، ساری صورت گرفت. مواد و روش‌ها: در این مطالعه توصیفی- مقطعی، 150 ایزوله پسودوموناس آئروژینوزا از نمونه‌های بالینی بیماران در بخش سوختگی بیمارستان آموزشی– درمانی زارع، شهرستان ساری جمع‌آوری شدند. حساسیت آنتی‌بیوتیکی به روش دیسک دیفیوژن انجام شد. شیوع ژن‌های بتالاکتاماز blaVIM-1، bla IMP-1 در ایزوله‌ ها با استفاده از PCR انجام شد. یافته‌ها: <a name="_Hlk150336187">طبق نتایج حساسیت آنتی‌بیوتیکی، بیش‌ترین درصد مقاومت به ترتیب به جنتامایسین 85 درصد،</a> سفتازیدیم 84 درصد، ایمی‌پنم 83 درصد، سیپروفلوکساسین 80 درصد، سفیپیم 79 درصد، آمیکاسین 78 درصد، مروپنم 71 درصد، کلیسیتین 10 درصد مشاهده شد. فراوانی ژن‌های blaVIM-1، bla IMP-1 به ترتیب 45 و 55 درصد در ایزوله‌ها شناسایی شدند. استنتاج: فراوانی سویه‌های پسودوموناس آئروژینوزا حامل ژن متالوبتالاکتاماز (MBLs) در بیمارستان زارع در شهرستان ساری در حال افزایش است. در بخش مراقبت‌های ویژه و بخش سوختگی بیمارستان‌ها باید از تجویز تجربی و بی‌رویه کارباپنم‌ها خودداری نمود. <pre style="text-align:justify; text-indent:28.05pt"> Background and purpose: Pseudomonas aeruginosa is a significant contributor to hospital-acquired infections, including urinary infections, bacteremia, pneumonia, and burn wound infections. Due to the significant role of carbapenems in treating infections that are resistant to conventional antibiotics, it is necessary to identify the prevalence and resistance patterns of pathogens that cause hospital infections. This will help to prevent the spread of microbial resistance, and subsequently, reduce the death rate caused by such infections. Therefore, the aim of the present study is to determine the occurrence of metallo-beta-lactamase genes in Pseudomonas aeruginosa isolates in Sari hospitals.</pre> Materials and methods: This cross-sectional study included 150 isolates of Pseudomonas aeruginosa as clinical samples that were collected from patients from the burn department of Zare Hospital, Sari city. To determine the antibiotic sensitivity, the disk diffusion method was used. Additionally, PCR method was employed to examine the prevalence of beta-lactamase genes blaVIM-1 and blaIMP-1 in the isolates. Results: Based on the antibiotic sensitivity results, it was found that the highest resistance was observed to gentamicin, ceftazidime, imipenem, ciprofloxacin, amikacin, meropenem, cefepime, ceftriaxone, and calcitin. The data analysis also revealed that the frequencies of blaVIM-1 and bla IMP-1 genes were detected in isolates of 45% and 55%, respectively. Conclusion: The frequency of Pseudomonas aeruginosa strains carrying the MBLs genes is increasing in Zare Hospital in Sari.
