fa بررسی چگونگی تکمیل فرم ارجاع و بازخورد توسط پزشک خانواده شهری مازندران در سال 1397 Investigating How to Complete the Referral and Feedback form with the Urban Family Doctor of Mazandaranin بهداشت خانواده Family Health گزارش کوتاه Brief Report سابقه و هدف: <a name="_Hlk168572022">پزشک خانواده با استفاده از فرآیند ارجاع، بیماران را به سطوح تخصصی نظام سلامت معرفی می</a>‌کند. به عبارتی دیگر نظام ارجاع می‌تواند به ما اطمینان دهد که مراجعین مراقبت‌های بهداشتی و درمانی به صورت بهینه ومقرون به‌صرفه در سطح تخصصی، خدمات مناسب دریافت می‌کنند. هم‌چنین دسترسی به موقع و ارائه مناسب مراقبت‌های اولیه سلامت از فوائدآن می‌باشد. ارجاع، فرآیند دریافت مشاوره سرپایی تخصصی است که مسیر و ارتباط فرد در سه سطح نظام سلامت را برای تامین جامعیت و تداوم خدمات تعیین می‌کند. این مطالعه با هدف تعیین چگونگی تکمیل فرم ارجاع و بازخورد در پزشک خانواده شهری مازندران در سال 1397 انجام شد. مواد و روش‌ها: این مطالعه مقطعی- توصیفی- تحلیلی می‌باشد که به  بررسی فرم‌های ارجاع بیمه‌شده‌های بیمه تامین اجتماعی توسط پزشکان خانواده شهری و بازخورد متخصصین انجام شد. محقق پس از دریافت کد اخلاق و کسب اجازه از سازمان‌های مرتبط به بررسی فرم‌های ارجاع بیمه شده‌های سازمان تامین اجتماعی سال 1397 ارسالی از طرف پزشکان خانواده شهری به متخصصین در سطح استان پرداخت. جامعه آماری شامل همه فرم‌های ارجاع و پسخوراند تحویلی توسط متخصصین به سازمان بیمه تامین اجتماعی برای دریافت حق العلاج بود. براساس گزارش اداره اسناد پزشکی سازمان بیمه تامین اجتماعی استان مازندران در سال 1397 در مجموع تعداد 151075 برگه ارجاع تکمیل شده برای دریافت حق العلاج متخصصین به آن سازمان تحویل شده است. داده‌های جمع‌آوری شده برای تجزیه و تحلیل آماری به نرم‌افزار اکسل وارد شدند. تحلیل آماری در بخش آمار توصیفی با استفاده از شاخص‌های مرکزی (میانگین، میانه، مد) و شاخص‌های پراکندگی (واریانس، انحراف‌معیار، دامنه تغییرات، ضریب تغییرات) برای متغیرهای کمی و شاخص‌های فراوانی، درصد و شیوع برای داده‌های کیفی انجام شد. داده‌های استخراج شده با استفاده از نرم‌افزار SPSS نسخه 25 تجزیه و تحلیل شدند. یافته‌ها: بررسی 3200 مورد از فرم‌های ارجاع و پس‌خوراند مربوط به پزشکان خانواده و متخصصین نشان می‌دهد که در بین پزشکان خانواده 2321 مورد (72/5 درصد) مرد و 879 مورد (27/5 درصد) زن بودند. سوابق بیماران درج شده در 586 مورد (3/18 درصد) ناقص بود. یافته‌های بالینی در 89 مورد (2/7 درصد) وجود داشت و یافته‌های پاراکلینیک نیز در 22 مورد (0/7درصد) ناقص و در 171 مورد (5/3 درصد) کامل بود. در 884 مورد (27/6 درصد) برگه‌های ارجاع ناخوانا بودند. علت ارجاع در 560 مورد (17/5 درصد) درج شده بود. علت نیاز به مراجعه بعدی در 187 مورد (5/8 درصد) از فرم‌های پس‌خوراند توسط متخصصین نوشته شده بود. علاوه بر این 1454 مورد (45/4 درصد) از برگه‌های پس‌خوراند تکمیل شده توسط متخصصین ناخوانا بودند. استنتاج: <a name="_Hlk168572095">نتایج به‌دست آمده در این مطالعه نشان می</a>‌دهد که وضعیت کیفیت تکمیل فرم ارجاع از جانب پزشکان خانواده و همچنین وضعیت ارائه پسخوراند از سوی پزشکان متخصص مناسب نیست. برای بهبود این وضعیت اجرای برنامه‌های آموزشی حضوری و غیرحضوری برای هر دو گروه و استفاده از فرم‌های ارجاع الکترونیک می تواند در دستور کار قرار گیرد.   Background and purpose: The process by which family physicians refer patients to specialized levels of the healthcare system is presented. In other words, the referral system can assure us that the clients of health care services are optimally allocated at the specialized level of services. In addition to providing proper time and providing proper health care for health benefits. The referral process is a specialized outpatient consulting process that determines the course and communication of the individual in three levels of the health system to provide the integrity and continuity of services this study aimed to determine the quality of referral and feedback forms in the medical family physician of Mazandaran province in 2019. Materials and methods: This descriptive cross-sectional study was conducted to examine the form of insurance referral of social security insurance by physicians of urban families and feedback from experts. After receiving the code of ethics and obtaining permission from the relevant organization the researcher paid the social security organization 1397 issued by the physicians in the province. The statistical population consisted of all forms of reference and the feedback from experts to the social security insurance organization to get the right According the report of the department of medical record department of the social security insurance organization of Mazandaran province in1397 has been handed over to that organization in a total of 151075 reference sheets completed to obtain the right of the specialists. the collected data were entered into Excel software for statistical analysis was performed in descriptive statistics using central dices (mean, median, mode) and dispersion index(variance, standard deviation, range of changes, coefficients of variation ) for quantitative variables, and index, percentage, and prevalence for qualitative data. The data were analyzed by using spss-25 software. Results: The study of 3200 cases of referral and referral forms of family physicians and specialists showed that among the Physicians 2321(72.5) were male and 879(27.5) were female. The patient's records in 586(%18.3) were incomplete. Clinical findings Were found in 98(2.7%) cases and paraclinical findings were incomplete in 22 cases (0.7%) and complete in 171 cases (5.3%). in 884 cases (27.6%) the papers were illegible the cause of referral was written in 560(17.5%). the reason for further referral in 187 cases (5.8%) was written by experts. In addition, 1454 cases (45.4%) of the return sheets completed by specialists were illegible.
