fa بررسی عوامل پاراکلینیکی و دموگرافیک موثر در موفقیت درمان ناباروری به روش IVF میکروانجکشن در زنان نابارور Investigation of Paraclinical and Demographic Factors Affecting the Success of Infertility Treatment by IVF Microinjection Method in Infertile Women زنان و زایمان و نازایی Gynecology and Infertility پژوهشي-کامل Research(Original) سابقه و هدف: ناباروری به عنوان معضل بهداشت باروری در سراسر دنیا مطرح شده است. شیوع ناباروری در مناطق مختلف جهان براساس مطالعه‌های انجام شده متفاوت است و شیوع آن در ایران بالاتر از میانگین جهانی است. روش‌های مختلفی برای درمان ناباروری وجود دارد که یکی از مهم‌ترین آن‌ها، روش IVF میکروانجکشن است که به‌عنوان یکی از آخرین روش درمانی برای زوج‌های نابارور به‌شمار می‌رود. در موفقیت‌آمیز بودن این روش عوامل مختلفی نقش دارند که بسیاری از آن‌ها ممکن است هنوز شناخته شده نباشند، لذا این مطالعه با هدف بررسی عوامل پاراکلینیک و دموگرافیک موثر بر درمان ناباروری به روش IVF میکروانجکشن در مرکز درمان ناباروری بیمارستان امام خمینی ساری، انجام پذیرفت. مواد و روش‌ها: داده‌های این مطالعه مقطعی، با استفاده از اطلاعات پرونده‌های زنان ناباروری که در مرکز درمان ناباوری بیمارستان امام خمینی ساری تحت درمان قرار دارند، استخراج شد. اطلاعات پرونده‌هایی جمع‌آوری شدند که زوجین ایرانی بودند، برای آن‌ها تشخیص ناباروری قطعی باشد و مشکل ناباروری مربوط به زن باشد. عوامل پاراکلینیکی و دموگرافیک موثر بر موفقیت درمان ناباروری به روش IVF میکروانجکشن با استفاده از مدل رگرسیون لجستیک بررسی شدند. ابتدا با استفاده از مدل تک متغیره، متغیرهای مهم شناسایی شدند و سپس مدل چند متغیره، برازش داده شد. برای تحلیل داده‌ها از نرم‌افزار SPSS21 استفاده شد. یافته‌ها: به‌طورکلی 64 نفر (19/9 درصد) به روش IVF میکروانجکشن موفق به درمان شدند. زنانی که موفق به درمان شده‌اند در زمان تشخیص ناباروری میانگین سنی کم‌تری نسبت به زنانی داشتند که موفق به درمان نشده‌اند(0/015=P). هم‌چنین نسبت موفقیت درمان در زنان ناباروری که سابقه بیماری‌های روانپزشکی داشتند بیش‌تر از زنان ناباروری بود که سابقه بیماری‌های روانپزشکی نداشتند (0/005=P). هم‌چنین در فاصله کم‌تر از یکسال تشخیص ناباروری زنان، افزایش سن زن در زمان تشخیص ناباروری، شانس موفقیت درمان به روش IVF میکروانجکشن را کم‌تر می‌کند. میزان موفقیت درمان برای زنانی که کم‌تر از یک سال تحت درمان بودند (22/4-13/3 درصد) 17/5 درصد و برای زنانی که بیش‌تر از یک سال تحت درمان بودند (44/6-20/3درصد) 31/5درصد بود. استنتاج: موفقیت کلی درمان به روش IVF میکروانجکشن در این مطالعه، نسبت به مطالعات مشابه کم‌تر بود، هر چند در زنانی که کم‌تر از یک سال تحت درمان بودند، این میزان، کم‌تر از زنانی بود که بیش‌تر از یکسال تحت درمان بودند و نشان می‌دهد که ادامه درمان ناباروری به این روش در مدت زمان بیش‌تری پس از شروع درمان، شانس موفقیت درمان را بیش‌تر می‌کند. سن زن در هنگام تشخیص ناباروری یکی از مهم‌ترین عوامل در درمان ناباروری به روش IVF میکروانجکشن است و هرچه ناباروری در سن پایین‌تری تشخیص داده شود و درمان‌هایی مثل IVF میکروانجکشن زودتر شروع شد، شانس موفقیت در درمان و باروری بیش‌تر می‌شود. لذا مراجعه زود هنگام به متخصص می‌تواند نقش زیادی در موفقیت درمان داشته باشد. موفقیت درمان به روش IVF میکروانجکشن در زنانی که سابقه بیماری‌های روانپزشکی داشتند بیش‌تر از زنان دیگر بود که می‌تواند ناشی از اصرار آن‌ها بر درمان باشد.   Background and purpose: Infertility has been raised as a reproductive health problem all over the world. Based on the other studies the prevalence of infertility in different regions of the world is different, and its prevalence in Iran is higher than the global average. There are various methods for treating infertility, one of the most important of which is the IVF microinjection method, which is considered one of the last treatment methods for infertile couples. Because various factors play a role in the success of this treatment, many of which may not be known yet, so this study was conducted to investigate the paraclinical and demographic factors affecting infertility treatment by IVF microinjection method in the infertility treatment center of Imam Khomeini Hospital in Sari. Materials and methods: The data of this cross-sectional study were extracted using information from the files of infertile women who are being treated at the Infertility Treatment Center of Imam Khomeini Hospital in Sari. Information was collected on cases that were Iranian couples, married women whose diagnosis of infertility was definite, and the problem of infertility was related to the woman. Paraclinical and demographic factors affecting the success of infertility treatment by IVF microinjection method were investigated using a logistic regression model. First, using a univariate model, important variables were identified, and a multivariate model was fitted. SPSS21 software was used for data analysis. Results: 64 people (19.9%) were successfully treated using the IVF microinjection method. Women who were successfully treated had a lower average age at the time of diagnosis of infertility than women who were not treated successfully (P= 0.015). Also, the treatment success rate in women who had a history of psychiatric diseases is higher than in women who did not have (P= 0.005). Also, less than one year after the diagnosis of infertility, an increase in the age of the woman at the time of diagnosis of infertility, reduces the chances of successful IVF microinjection treatment. The success rate of treatment for women who were treated for less than one year was 17/5% (13/3% - 22.4%), and for women who were treated for more than one year was 31.5% (20/3% - 44.6%). Conclusion: The overall success rate of microinjection IVF treatment in this study was lower than in similar studies, although, in women who had been treated for less than one year, this rate was lower than in women who had been treated for more than one year. So, continuing infertility treatment by IVF microinjection method for a longer period after the start of treatment increases the chance of treatment success. A woman's age when infertility is diagnosed is one of the most important factors in the treatment of infertility, and the younger the infertility is diagnosed and the sooner treatments are started, the higher the chance of success in treatment and fertility. Therefore, early referral to a specialist can affect the success of the treatment.
