fa بررسی ناهنجاری های آناتومیک مفصل زانو و ارتباط آن با مفصل هیپ Evaluation of the Knee Anatomical Deformities and Its Relationship with Hip Joint ارتوپدی،تروماتولوژی Orthopedic & Traumatology گزارش کوتاه Brief Report سابقه و هدف: آسیب و اختلالات مفصل زانو به دو دسته کلی آسیب‌های ضربه‌ای و غیرضربه‌ای شامل بد شکلی‌های اسکلتی همچون زانو پرانتزی(ژنوواروم) و زانو ضربدری(ژنو والگوم) تقسیم می‌شوند. ژنو والگوم و ژنو واروم از جمله شایع‌ترین ناهنجاری‌های ناحیه مفصل زانو می‌باشد که می‌تواند به‌طور مستقیم بر کیفیت زندگی اجتماعی افراد نقش داشته باشد. با توجه به اهمیت تعیین میزان آن در جامعه جوانان، این مطالعه با هدف بررسی ارتباط بین وضعیت ژنو والگوم و ژنو واروم با شاخص‌های آنتروپومتریک بدن در دانش‌آموزان مدارس ابتدایی سطح شهرستان دزفول در سال 1401، تعیین گردید. مواد و روش‌ها: این مطالعه مقطعی-تحلیلی، درسال1401 درسطح مدارس ابتدایی شهرستان دزفول بر روی 400 نفر از دانش‌آموزان مدارس دخترانه و پسرانه بین سنین 7 تا 11 سال انجام شده است. پس از اخذ رضایت و دریافت اطلاعات دموگرافیک شامل سن، جنس، قد، وزن، شاخص توده بدنی، سوابق خانوادگی و فاصله بین تروکانترهای بزرگ استخوان ران از طریق پرسشنامه، برای ارزیابی تغییر شکل وضعیت اندام تحتانی، شرکت‌کنندگان باید به‌صورت ایستاده به نحوی که زانو در حالت اکستنشن کامل باشد و قوزک‌های دو پا به گونه‌ای به هم چسبیده باشد که استخوان کشکک هر دو سمت رو به سمت جلو باشند. در این حالت فاصله بین دو اپی‌کندیل داخلی استخوان‌های فمور(برای ارزیابی ژنو واروم) و فاصله بین دو قوزک داخلی (برای ارزیابی ژنووالگوم) به‌وسیله خط‌کش مخصوص اندازه‌گیری و ثبت گردید. یافته‌ها: نتایج توزیع فراوانی نشان داد2/5 درصد از افراد مورد مطالعه دارای ژنو والگوم (پا ضربدری) و 11/5درصد از آن‌ها دارای ژنو ‌واروم(پا پرانتزی) بودند. این آمار در مجموع نشان دهنده وجود اختلالات اسلکتی به میزان 14 درصد در جمعیت مورد مطالعه بود. هم‌چنین نتایج این مطالعه نشان داد که تنها 2/2 درصد از افراد مورد مطالعه دارای سابقه خانوادگی عارضه‌های اسکلتی عضلانی همچون ژنو والگوم و ژنو واروم بوده و 97/5درصد از آن‌ها سابقه خانوادگی نداشتند که نشان‌دهنده نبود ارتباط معنی‌دار بین بروز اختلالات و سابقه خانوادگی بود(0/187P=). بررسی‌ها نشان داد بین میانگین سن و مشاهده اختلال ژنو واروم و ژنو والگوم ارتباط معنی‌داری وجود دارد به نحوی که با افزایش سن، افراد بیش‌تری درگیر اختلال بودند(0/001P<). هم‌چنین بین افزایش قد و وزن و بروز این اختلالات ارتباط معنی‌داری مشاهده گردید(0/001P=). جالب توجه است که نتایج این مطالعه ارتباط معنی‌داری بین فاصله بین مفصل هیپ با ناهنجاری‌های ژنو والگوم و ژنو واروم نشان دادند(0/05P<). استنتاج: اگرچه با توجه به جامعه آماری مورد مطالعه میزان اختلالات در جمعیت نمونه درصد خیلی بالایی نداشت، اما بررسی‌های آماری در این مطالعه ارتباط معنی‌داری بین شاخص‌های دموگرافیک و اختلالات مفصل زانو را تایید کرده است. وجود ارتباط بین قد، وزن، سن و سایر شاخص‌های مهم آناتومیک همانند فاصله بین مفصل هیپ با ژنو واروم و ژنو والگوم احتمالا نشان‌دهنده روند رو به افزایش مشاهده این اختلالات در جامعه با رشد و تغییرات فرم سیستم اسکلتی باشد که نیازمند انجام مطالعات با جامعه آماری وسیع‌تری است. نتایج این تحقیق می‌تواند به عنوان یک زنگ خطری برای سلامت جامعه، مورد استفاده سیاست‌گذاران سلامت جامعه قرار گیرد تا با تدوین برنامه‌ها و استراتژی‌های مناسب از میزان بروز این اختلال مخصوصا در سنین پایین جامعه جلوگیری به‌عمل آید.   Background and purpose: Knee joint disorders are divided into two general categories of traumatic and non-traumatic injuries, including skeletal deformities such as genu valgum (knocked knee) and genu varum (bowed legs). Genu valgum and genu varum are two of the most common deformities of the knee joint. These deformities can directly affect the quality of people's social lives. Considering the importance of determining its rate in the youth community, this study aims to determine the relationship between genu valgum and genu varum with anthropometric indices in elementary school students of Dezful City in 2022. Materials and methods: The current research was cross-sectional-analytical, which was conducted in 2022 at the level of elementary schools in Dezful City on 400 students of girls' and boys' schools between 7 and 11 years old. Patient demographic information, including age, gender, height, weight, family history, and hip joint width, was gathered using a questionnaire. To evaluate the deformation of the lower limb, the participants must stand in a way that the knee is in full extension and the ankles of both legs should be close together so that the patella bones on both sides are facing forward. In this position, deformation of the knee joint (genu valgum and genu varum) was determined by measuring the distances between the medial condyles of the femur and the medial malleolus of the ankle, and its relationship with demographic information was evaluated. Results: The frequency distribution showed that 2.5% of the studied subjects had genu valgum, and 11.5% of them did not have genu varum. These results showed the existence of skeletal disorders in knee joints at the rate of 14% in the studied population. Also, the results of this study showed that only 2.2% of the studied population had a family history of musculoskeletal disorders such as genu valgum and genu varum, and 97.5% of them had no family history, which indicated the absence of a significant relationship between the occurrence of disorders and family history (P=0.187). Results showed that there was a significant relationship between the average age and the observation of genu varum and genu valgum disorder (P<0.001). Also, a significant relationship was observed between height and weight and genu varum and genu valgum (P=0.001). It is noteworthy that a significant relationship was observed between the width of the hip joint with these abnormalities (P<0.05). Conclusion: Although the rate of disorders in the sample population was not very impressive, investigations in this study have confirmed a significant relationship between demographic indicators and knee joint disorders. The existence of a relationship between height, weight, age, and other important anatomical indicators, such as the distance between the hip joint and genu varum and genu valgum, probably indicates the increasing trend of observing these disorders in society with growth and changes in the skeletal framework, which requires further evaluation.
