fa ارزیابی گلوکوم در بیماران مبتلا به سندرم سودواکسفولیشن چشمی در بیمارستان بوعلی سینا ساری (1398-1397) Glaucoma Evaluation in Patients with Ocular Pseudoexfoliation Syndrome at Sari Bu Ali Sina Hospital (2019-2020) چشم Eye پژوهشي-کامل Research(Original) سابقه و هدف: سندرم سودواکسفولیشن (PEX) یک بیماری شایع مرتبط با سن است که با ترشح مواد فیبریلار از اجزای داخل چشمی به ویژه در سگمان قدامی چشم مشخص می‌شود. این وضعیت منجر به انسداد شبکه ترابکولار، اختلال در خروج مایع زلالیه و متعاقبا افزایش فشار داخل چشمی (IOP) می‌شود. این عوامل سبب افزایش خطر ابتلا به گلوکوم و آسیب به ساختارهای داخلی چشم و کاهش دید غیرقابل برگشت می‌شود. بنابراین، غربالگری و ارزیابی افراد مبتلا به این سندرم ممکن است امکان شناسایی به موقع خطر گلوکوم و اجرای اقدامات پیشگیرانه یا درمانی مناسب را فراهم کند. از این رو، بررسی سندرم PEX به‌منظور سنجش خطر ابتلا به گلوکوم و اعمال اقدامات مناسب برای حفظ بینایی و سلامت چشم از اهمیت بالایی برخوردار است. لذا هدف از این مطالعه تعیین میزان شیوع گلوکوم در بیماران مبتلا به سندرم PEX برای ارزیابی دقیق خطر ابتلا به گلوکوم بود. این ارزیابی ممکن است شامل نظارت بر فشار داخل چشمی، اقدامات پیشگیرانه و انجام درمان‌های مختص این بیماری باشد. مواد و روش‌ها: این مطالعه مقطعی بر روی بیماران بالاتر از ٤٠ سال مبتلا به سندرم PEX چشمی مراجعه‌کننده به بخش چشم پزشکی بیمارستان بوعلی سینا ساری، مازندران، ایران، در فاصله زمانی بهمن ١٣٩٧ تا اسفند ١٣٩٨ انجام شد. ابتدا معاینات کامل چشم پزشکی شامل رفرکشن با اتورفرکتومتر تاپکن، تعیین حدت بینایی با E چارت اسنلن، اسلیت لمپی و فاندوسکوپی با لنز ٩٠ برای تمام بیماران انجام شد. سپس فشار داخل چشمی(IOP) از طریق تونومتر گلدمن اندازه‌گیری شد. در افراد مشکوک به گلوکوم، زاویه چشم با استفاده از گونیوسکوپ سه آینه‌ای جهت بررسی ترابکولار مشورک انجام شد و نسبت کاپ به دیسک عصب بینایی (cup/disk) با Cirrus HD-NFL OCT (Carl Zeiss Meditec, Inc.) تعیین شد. براساس یافته‌های معاینات بالینی، بیماران در دو گروه گلوکوم و غیرگلوکوم قرار گرفتند. داده‌ها در نرم‌افزار SPSS نسخه ٢٢ تجزیه وتحلیل شدند. جهت توصیف داده‌ها از میانگین، انحراف‌معیار، تعداد و درصد استفاده شد. برای مقایسه میانگین‌های متغیرهای کمی از آزمون t-test و برای مقایسه میانگین‌های متغیرهای کیفی آزمون کای-دو در دو گروه مبتلا و عدم مبتلا به گلوکوم استفاده شد. سطح معنادار آماری ٠٥/٠ P<در نظر گرفته شد. یافته‌ها: از ١٠٠ بیمار (١٨٠ چشم) با میانگین سنی 6/97±73/69 سال، 27/77 درصد مبتلا به گلوکوم بودند که در ٩٠ درصد موارد گلوکوم درگیری دوطرفه مشاهده شد. ٥٤ درصد از بیماران مبتلا به گلوکوم، مردان با میانگین سنی 7/03± 73/88 سال بودند. بیماران گلوکومی فشار داخل چشمی و نسبت کاپ به دیسک بالاتری نسبت به موارد غیر گلوکومی نشان دادند (0/001>P). در گروه گلوکوم، چشم‌های چپ در مقایسه با چشم‌های راست فشار داخل چشمی بالاتر (4/75±29/12 در مقابل 5/65±28/86؛ 0/001>P) و نسبت cup/disk بالاتری (0/12±0/53 در مقابل 0/15±0/51؛ 0/001>P) را نشان دادند. استنتاج: سندرم سودواکسفولیشن (PEX) شیوع قابل‌توجهی از گلوکوم، عمدتاً با درگیری دوطرفه را نشان می‌دهد. این سندرم می‌تواند یک عامل خطر مهم برای توسعه گلوکوما و شروع افزایش فشار داخل چشمی به ویژه در مردان مسن باشد. این یافته‌ها چشم‌‌انداز ارزشمندی را برای تحقیقات آینده ارائه می‌کنند و درک ما را از رابطه بین سندرم PEX چشمی و گلوکوم بهبود می‌بخشد، که ممکن است در بهبود نتایج درمان بیماران و استراتژی‌های مدیریت بالینی موثر باشد.   Background and purpose: Pseudoexfoliation syndrome (PEX) is a common age-related disorder, characterized by the secretion of fibrillar material from intraocular components, particularly along the anterior segment structures. This condition leads to obstruction of the trabecular meshwork impairing aqueous humor outflow and subsequently increasing intraocular pressure (IOP). These factors contribute to an increased risk of developing glaucoma, damage to the internal ocular structures, and irreversible vision loss. Therefore, screening and evaluating individuals with this syndrome may allow timely identification of glaucoma risk and implementation of appropriate preventive or therapeutic measures. Consequently, it is crucial to screen for PEX syndrome to assess glaucoma risk and take necessary actions to preserve vision and ocular health. This study aims to determine the prevalence of glaucoma in patients with PEX syndrome to accurately assess the risk of developing glaucoma. This assessment may include monitoring of intraocular pressure, preventive measures, and specific treatments. Materials and methods: This cross-sectional study was conducted on patients over 40 years old diagnosed with pseudoexfoliation (PEX) syndrome referred to the ophthalmology department of Bu-Ali Sina Hospital in Sari from February 2019 to March 2020. Initially, comprehensive ophthalmic examinations, including autorefraction with a Topcon autorefractometer, visual acuity determination with Snellen E charts, slit lamp examination, and fundoscopy with a 90D lens, were performed for all patients. Subsequently, intraocular pressure (IOP) was measured using a Goldmann tonometer. In individuals suspected of glaucoma, an angle examination using a three-mirror goniolens to assess the trabecular meshwork, and cup/disc ratio using Cirrus HD-NFL OCT (Carl Zeiss Meditec, Inc.) was performed. Based on clinical findings, patients were categorized into glaucoma and non-glaucoma groups. The data were analyzed using SPSS version 22 software. Descriptive statistics, including mean, standard deviation, frequency, and percentage, were used to describe the data. For comparing the means of quantitative variables, the t-test was utilized, while the chi-square test was used for comparing the means of qualitative variables between the glaucoma and non-glaucoma groups. A statistically significant level of P<0.05 was considered. Results: Among of the 100 patients (180 eyes), 27.77% were diagnosed with glaucoma, with bilateral involvement noted in 90% of glaucoma cases. 54% of glaucoma patients were male with a mean age of 73.88±7.03 years. Glaucoma patients exhibited higher intraocular pressure (IOP) and cup/disc ratio compared to non-glaucoma cases (P<0.001). Within the glaucoma group, the left eyes demonstrated higher IOP (29.12±4.75 versus 28.86±5.65, P<0.001) and a higher cup/disc ratio (0.53±0.12 versus 0.51±0.15; P<0.001) compared to the right eyes. Conclusion: Pseudoexfoliation syndrome (PEX) is associated with a significant prevalence of glaucoma, particularly with bilateral involvement in most cases. This syndrome may be an important risk factor for the development of glaucoma and the onset of elevated intraocular pressure, especially in elderly men.
