fa ارتباط بین بهداشت دهان بر اساس DMFT با سندرم متابولیک در داده‌های فاز ثبت نام جمعیت کوهورت طبری Relationship between oral Hygiene Based on (DMFT) with Metabolic Syndrome in the Data of Registration Phase of the Tabari Cohort Population پرستاری nursing پژوهشي-کامل Research(Original) سابقه و هدف: شیوع سندرم متابولیک در جوامع به طور فزاینده‌ای در حال افزایش است. سندرم متابولیک مجموعه‌ای از اختلالات از جمله چاقی، فشارخون بالا، دیس لیپیدمی و مقاومت به انسولین می‌باشد. مراقبت ناکافی از دندان با بسیاری از بیماری‌های سیستمیک مرتبط است. هم‌چنین شواهد علمی حاکی از وجود ارتباط بین بهداشت دهان و سندرم متابولیک می‌باشد. از این رو مطالعه حاضر با هدف تعیین ارتباط بین بهداشت دهان بر اساس DMFT (دندان‌های پوسیده، کشیده شده، پرشده) با سندرم متابولیک در داده‌های فاز ثبت نام جمعیت کوهورت طبری انجام شد. مواد و روش‌ها: این مطالعه مقطعی وتحلیلی شامل شرکت‌کنندگانی از جمعیت کوهورت طبری بود که در مرحله ثبت‌نام، ارزیابی‌های سلامت دهان و سندرم متابولیک را انجام داده بودند. در مرحله ثبت نام کوهورت طبری اطلاعات 10255 نفر، در گروه سنی 70-35 سال، در طی سال‌های 2017-2015 از دو منطقه شهری و کوهستانی شهرستان ساری، مازندران، ایران جمع‌آوری شد. اطلاعات مربوط به سطح تحصیلات، وضعیت اشتغال، محل زندگی، وضعیت اجتماعی-اقتصادی و تاهل، سابقه اختلالات روانی، سابقه مصرف سیگار یا قلیان، سطح فعالیت بدنی روزانه، مسواک زدن، تعداد دفعات مسواک زدن در روز، استفاده از نخ دندان، تعداد دفعات استفاده از نخ دندان در روز، استفاده از دندان مصنوعی، سال‌های استفاده از دندان مصنوعی، مسواک کردن دندان مصنوعی، استفاده از دهانشویه، تعداد دفعات استفاده از دهانشویه و شاخص DMFT در زمان اجرای TCS از طریق مصاحبه و معاینات شفاهی جمع‌آوری شد. سندرم متابولیک بر اساس معیارهای پانل سوم درمان بزرگسالان (ATP III) تشخیص داده شد. در انتها با نرم‌افزار SPSS نسخه 27، از میانگین، انحراف معیار و درصد برای توصیف داده‌ها، مقایسه متغیرهای کیفی با ابتلا به سندرم متابولیک با آزمون کای اسکوئر و مقایسه متغیرهای کمی با آزمون t مستقل در تحلیل تک متغیره استفاده شد. هم‌چنین تعدیل متغیرهای مخدوشگر، با بهره‌گیری از رگرسیون لجستیک چندمتغیره صورت گرفت. یافته‌ها: در مجموع 9939 نفر در تجزیه و تحلیل نهایی وارد شدند. با آزمون T مستقل بین سندرم متابولیک و DMFT رابطه معنی داری وجود نداشت (4/59=T، 0/156=P)، بطوری که میانگین DMFT در گروه متبلا به سندرم متابولیک 9/07±17/66 و در گروه غیر مبتلا 8/99±16/81 بود. با آزمون کای دو استفاده از دندان مصنوعی(0/001>P)، استفاده از نخ دندان (0/001>P)، سن (0/001>P)، جنس (0/001>P)، وضعیت تاهل(0/05=P)، شغل (0/001>P)، محل سکونت(0/001>P)، بیماری‌های روانی(0/001>P)، وضعیت اجتماعی- اقتصادی(0/005=P)، استعمال سیگار (0/001>P)، BMI (0/001>P)، MET (0/001>P) و سطح تحصیلات (0/001>P) با سندرم متابولیک ارتباط معنی‌دار داشت. پس از تعدیل مخدوش‌گرها در مدل رگرسیون لجستیک چند متغیره، تنها ارتباط تعداد استفاده از نخ دندان دو بار در روز با سندرم متابولیک معنی‌دار شد (0/046=P، 2/213- 1/008 CI- 95 درصد) 493/11/493 OR. استنتاج: یافته‌های این مطالعه نشان داد که بین بهداشت دهان و دندان که با شاخص DMFT اندازه‌گیری می‌شود و وجود سندرم متابولیک در داده‌های مرحله ثبت جمعیت کوهورت طبری رابطه معنی‌داری وجود ندارد. تنها بین استفاده از نخ دندان دو بار در روز با سندرم متابولیک ارتباط معنی دار یافت شد. مطالعات بیشتر برای کشف سایر عوامل بالقوه ای که ممکن است در ایجاد سندرم متابولیک در این جمعیت نقش داشته باشند، ضروری است.   Background and purpose: The prevalence of metabolic syndrome in societies is increasing. Metabolic syndrome is a cluster of medical conditions that include obesity, hypertension, dyslipidemia, and insulin resistance. Poor dental care is linked to numerous systemic disorders. Scientific research supports the presence of a relationship between oral hygiene and metabolic syndrome. Therefore, the present study was conducted to determine the relationship between oral hygiene based on DMFT (decayed, missing, filled teeth) and metabolic syndrome in the registration phase data of the Tabari cohort population. Materials and methods: This study was cross-sectional and analytical and included participants from the Tabari cohort population who had performed oral health and metabolic syndrome assessments during the registration phase. Throughout the registration period of the Tabari cohort study (TCS), data was gathered from 10255 individuals ranging from 35 to 70 years old. This data was obtained from both urban and mountainous parts of Sari, Mazandaran, Iran, during the years 2015 and 2017. Data regarding education level, occupation status, residential location, socio-economic status, marital status, mental health history, smoking or hookah habits, daily physical activity, dental hygiene practices (such as tooth brushing and flossing frequency), denture usage and care, mouthwash usage, and DMFT index were gathered during the implementation of TCS through interviews and oral examinations. Metabolic syndrome was diagnosed based on the criteria of the third adult treatment panel (ATP III). Ultimately, SPSS version 27 software was utilized to describe the data using mean, standard deviation, and percentage. The qualitative variables were compared with the metabolic syndrome using the chi-square test, while the quantitative variables were compared using the independent t-test in univariate analysis. Furthermore, the process of adjusting for confounding variables was carried out through the utilization of multivariate logistic regression. Results: 9,939 people were included in the final analysis. Independent T-test result showed that there was no significant relationship between metabolic syndrome and DMFT (P=0.156, T=4.59), so the average DMFT in the group with metabolic syndrome was 17.66±9.07, whereas in the non-affected group, it was 16.81±8.99. Based on chi-square test results, significant relationships with metabolic syndrome were found for the use of dentures (P<0.001), use of dental floss (P<0.001), age (P<0.001), gender (P<0.001), marital status (P=0.05), occupation (P<0.001), place of residence (P<0.001), psychiatric disorders (P<0.001), socio-economic status (P=0.005), smoking (P<0.001), BMI (P<0.001), MET (P<0.001), and education level (P<0.001). Upon adjustment of confounders in the multivariable logistic regression model, only the number of flossing twice a day was significantly associated with metabolic syndrome (OR 1.493 [95% CI 1.008–2.213], P=0.046). Conclusion: The findings of this study showed that there is no significant relationship between oral and dental hygiene measured by the DMFT index and the presence of metabolic syndrome in the population registration stage data of the Tabari cohort. There was a significant relationship identified alone between flossing twice daily and metabolic syndrome.
