fa بررسی بروز ضایعات بدخیم و پیش بدخیم دهانی در بیماران مراجعه کننده به مراکز درمانی شهر ساری (یک مطالعه ۶ ساله) Investigating the Incidence of Malignant and Premalignant Oral Lesions in Patients Referr ing to Health Centers of Sari: A Six-Year Study دندانپزشکی Dentistry گزارش کوتاه Brief Report سابقه و هدف: بدخیمی‌ها شایع‌ترین علل مرگ در جوامع هستند. با توجه به اهمیت پژوهش‌های اپیدمیولوژیک در پیشگیری و درمان بدخیمی‌ها، مطالعه حاضر با هدف بررسی جوانب اپیدمیولوژیک ضایعات پیش بدخیم و بدخیم دهانی در ساری طی سال‌های ۱۴۰۱-۱۳۹۶ انجام پذیرفت. مواد و روش ‌ها: در مطالعه توصیفی-تحلیلی و مقطعی حاضر، ۲۶۸ بیمار دارای ضایعات پیش بدخیم یا بدخیمی‌های دهانی که دارای معیارهای ورود بودند، بررسی گردید. نمونه‌گیری به‌صورت سرشماری با استفاده از آرشیو مطب خصوصی متخصص‌های آسیب شناسی دهان، بیماری‌های دهان و جراح فک و صورت و همچنین مرکز جامع سرطان مازندران صورت گرفت. اطلاعات دموگرافیک و اطلاعات مربوط به ریسک فاکتورها، نوع، محل و علائم ضایعات توسط چک لیست و معاینه بالینی جمع‌آوری شدند. از آزمون‌های تی مستقل و کای دو جهت آنالیز داده‌ها استفاده شد. داده‌ها توسط نرم‌افزار SPSS23 تجزیه و تحلیل شدند و 0/05 P< معنی‌دار در نظر گرفته شد. یافته‌ها: از مجموع ۲۶۸ بیمار، ۱۷۸ بیمار با ضایعات بدخیم (66/4 درصد) بودند و شایع‌ترین ضایعه بدخیم کارسینوم سلول سنگفرشی دهانی بود. میانگین سنی بیماران16/94±   60/71  سال و بیش‌ترین بروز در مردان و زبان شایع‌ترین مکان درگیری بود. از میان ۹۰ بیمار پیش بدخیم نیز، لیکن پلان و لکوپلاکیا تنها ضایعات یافت شده بودند، میانگین سنی بیماران   50/91± 13/07  سال بود و بیش‌ترین بروز در زنان و مخاط باکال به ثبت رسید. یافته‌ها نشان داد که مصرف سیگار در گروه بدخیم به‌طور معنی‌داری بیش‌تر از گروه پیش بدخیم بود (۰۰۱/۰P<). تغییرات به (OSCC)Oral squamous cell carcinoma  در لکوپلاکیا و لیکن پلان به ترتیب در ۸/۳۰ و ۹/۳ درصد موارد ثبت شد. استنتاج: با توجه به خطر تغییر بدخیمی ضایعات پیش‌ بدخیم دهانی مانند لیکن پلان و لکوپلاکیا و نیز بروز بالای این ضایعات، تشخیص به موقع، پیگیری منظم و درمان اصولی در پیشگیری و کنترل بدخیمی‌های دهانی مهم است.   Background and purpose: Malignancies remain among the most common causes of death worldwide. Given the importance of epidemiological research in the prevention, early detection, and treatment of malignancies, the present study aims to examine the epidemiological characteristics of premalignant and malignant oral lesions in Sari during the years 2017 to 2023. Materials and methods: This descriptive-analytical cross-sectional study investigated 268 patients with premalignant lesions or oral malignancies who met the inclusion criteria. Sampling was performed using a census method, based on records from private offices of oral pathologists, oral medicine specialists, oral and maxillofacial surgeons, and the Mazandaran Comprehensive Cancer Center. Demographic information, as well as data related to risk factors, lesion type, location, and symptoms, were collected using a checklist and clinical examination. Independent t-tests and chi-square tests were used for data analysis. The data were analyzed using SPSS version 23. A P-value of <0.05 was considered statistically significant. Results: Of the total 268 patients, 178 (66.4%) had malignant lesions, with oral squamous cell carcinoma being the most common malignant lesion. The mean age of patients was 60.71 ± 16.94 years, with the highest incidence observed in men; the tongue was the most common site of involvement. Among the 90 patients with premalignant lesions, only lichen planus and leukoplakia were identified. The mean age in this group was 50.91 ± 13.07 years, with the highest incidence recorded in women, and the buccal mucosa was the most common site. The findings showed that smoking was significantly more prevalent in the malignant group compared to the premalignant group (P < 0.001). Transformation to oral squamous cell carcinoma (OSCC) was observed in 30.8% of leukoplakia cases and 3.9% of lichen planus cases, respectively.
