fa بررسی فراوانی استافیلوکوکوس اورئوس در مایع مفصلی بیماران مبتلا به آرتریت سپتیک بستری در بیمارستان های آموزشی درمانی دانشگاه علوم پزشکی مازندران Assessment of the Frequency of Staphylococcus aureus in Synovial Fluid Samples from Patients with Septic Arthritis in the Teaching Hospitals of Mazandaran University of Medical Sciences ميکروبيولوژي Microbiology گزارش کوتاه Brief Report سابقه و هدف: آرتریت باکتریایی از آرتریت‌های شناخته شده‌ای است که به-سرعت باعث تخریب مفاصل می‌شود. ورود باکتری به مفاصل ممکن است از طریق جراحی یا به ندرت حین آسپیراسیون و تزریق ایجاد شود. آرتریت سپتیک، یک بیماری عفونی مهم مفصلی است که می‌تواند در صورت عدم درمان به موقع، تخریب مفاصل و عوارض شدید را به دنبال داشته باشد. شایع‌ترین عامل این بیماری، استافیلوکوکوس اورئوس است. سن بیمار، سیستم ایمنی تضعیف شده و بیماری‌های قبلی مرتبط با مفاصل، از فاکتورهای مستعد کننده این بیماری به شمار می آیند. فرد مبتلا به آرتریت سپتیک در بیش‌تر مواقع دارای تب بالا، تورم، التهاب، درد و محدودیت حرکتی در مفصل‌های درگیر می‌باشد. این مطالعه با هدف بررسی فراوانی استافیلوکوکوس اورئوس در مایع مفصلی بیماران مبتلا به آرتریت سپتیک بستری در بیمارستان‌های آموزشی درمانی دانشگاه علوم پزشکی مازندران انجام شده است. مواد و روش ‌ها: این پژوهش به صورت توصیفی- مقطعی، در سال 1402-1403 روی 230 نمونه مایع مفصلی بیماران مبتلا به آرتریت سپتیک انجام شده است. نمونه‌ها از محیط‌های کشت مخصوص مورد بررسی قرار گرفتند و تشخیص نهایی از طریق کشت و تست‌های آزمایشگاهی برای شناسایی ژن nuc انجام شد. داده‌ها با نرم‌افزار SPSS تجزیه و تحلیل شدند. یافته‌ها: از میان 230 بیمار، 5/66 درصد مرد و 33/5 درصد زن بودند. شایع‌ترین مفصل درگیر زانو (53/5 درصد) و پس از آن شانه و آرنج بودند. از مجموع 20 مورد کشت مثبت، 12 مورد (60 درصد) استافیلوکوکوس اورئوس شناسایی شد. ارتباط معنی‌داری بین سن بیماران و نتیجه کشت مثبت مشاهده شد (0/043= P). استنتاج: استافیلوکوکوس اورئوس به عنوان شایع‌ترین عامل عفونی در آرتریت سپتیک شناخته شده است. نتایج این مطالعه اهمیت تشخیص سریع و دقیق عفونت‌های مفصلی برای پیشگیری از عوارض بلند مدت را تأیید می‌کند. توصیه می‌شود پژوهش‌های آینده به شناسایی الگوهای مقاومت آنتی‌بیوتیکی بپردازند.   Background and purpose: Bacterial (septic) arthritis is a severe and rapidly progressive form of arthritis that can lead to irreversible joint destruction. Bacteria may enter the joint during surgical procedures or, less commonly, through joint aspiration or injection. Advanced age, a compromised immune system, and a history of joint disease are recognized predisposing factors for this condition. Patients with septic arthritis typically present with high fever, joint swelling, inflammation, pain, and restricted range of motion in the affected joint(s). This study aimed to determine the prevalence of Staphylococcus aureus in the synovial fluid of patients diagnosed with septic arthritis and hospitalized in the teaching hospitals affiliated with Mazandaran University of Medical Sciences. Septic arthritis represents a serious infectious disease of the joints that can cause severe complications and permanent damage if not promptly diagnosed and treated. The most common causative pathogen is S. aureus. Materials and methods: This descriptive cross-sectional study, conducted from 2023 to 2024, included 230 synovial fluid samples obtained from patients diagnosed with septic arthritis. The samples were cultured on specific media, and final identification of the isolates was performed using standard microbiological techniques and molecular testing for the nuc gene. The data were analyzed using SPSS software. Results: Among the 230 patients included in the study, 66.5% were male and 33.5% were female. The knee joint was the most frequently affected site (53.5%), followed by the shoulder and elbow joints. Out of 20 culture-positive samples, 12 cases (60%) were identified as S. aureus. A significant association was found between patient age and positive culture results (P = 0.043). Conclusion: S. aureus was identified as the most prevalent causative agent of septic arthritis.
