fa مقایسه‌ی استحکام شکست دندان‌های اندو شده ترمیم شده با روش بایومیمتیک و سه نوع پست و کور Comparison of the Fracture Strength of Endodontically Treated Teeth Restored with the Biomimetic Method and Three Types of Post-and-Core Systems دندانپزشکی Dentistry گزارش کوتاه Brief Report سابقه و هدف: در دندان‌های درمان ریشه شده، درمان پست و کور و به دنبال آن، پوشش تاج با روکش درمان انتخابی است. تحقیق حاضر با هدف مقایسه‌ی استحکام شکست دندان‌های ترمیم شده با استفاده از چهار روش مختلف (بایومیمتیک، فایبر پست، پست و کور ریختگی نیکل-کروم (NiCr)، و پست و کور ریختگی Non-pieuous gold (NPG) )، انجام پذیرفت. مواد و روش ‌ها: در این مطالعه آزمایشگاهی، چهل دندان پرمولر دوم تک ‌ریشه‌ای ماگزیلا انتخاب و تاج آن‌ها از mm 2 بالای CEJ قطع شد. درمان ریشه‌ی استاندارد در نمونه‌ها انجام شد و دندان‌ها در 4 گروه تقسیم شدند. در گروه‌های پست ریختگی NiCr، NPG و فایبر پست، روکش‌های تمام فلزی ساخته شده و با گلاس‌آینومر سمان شدند. نمونه‌های بایومیمتیک از طریق قرار گرفتن فایبرهای رزینی تقویتی و به طور مستقیم ترمیم شدند و سپس با روکش‌های تمام فلزی باز سازی شدند. نیروهای 45 درجه‌ای در دستگاه universal به روکش‌ها وارد شده و استحکام شکست دندان‌ها محاسبه و الگوهای شکست آن‌ها تعیین شد. استحکام گروه‌ها با آزمون آنالیز واریانس مقایسه شده و مقایسات دو به دو با آزمون Tukey انجام شد. الگوهای شکست با آزمون chi-square بررسی شدند. یافته‌ها: بیش‌ترین مقادیر استحکام شکست به ترتیب در پست‌های NPG (4622/37 نیوتن)، فایبرپست (4534/22 نیوتن)، <a name="_Hlk199761302">NiCr</a> (3794/45 نیوتن) و روش بایومیمتیک (2329/58 نیوتن) دیده شد. استحکام پست‌های (0/001 = p) NPG، فایبر (0/001=P) و NiCr (0/025=P) به طور معنی‌داری بیش از روش بایومیمتیک بوده و بیش‌ترین و کم‌ترین فراوانی شکست‌های غیرقابل ترمیم به ترتیب در گروه‌های فایبرپست (80/0درصد) و بایومیمتیک (50/0درصد) دیده شد. استنتاج: روش بایومیمتیک برای تقویت دندان‌ها استحکام شکست کم‌تری نسبت به سیستم‌های پست-کور ایجاد کرده ولی فراوانی شکست‌های قابل ترمیم در آن بیش‌تر از گروه‌های دیگر مورد آزمایش بوده است.   Background and purpose: In endodontically treated teeth, the use of post-cores and subsequent full-coverage crown restorations is a commonly proposed treatment modality. This study aimed to compare the fracture resistance of restored teeth using four different methods: biomimetic approach, fiber post, nickel-chromium (NiCr) cast post and core, and non-precious gold (NPG) cast post and core. Materials and methods: In this experimental study, 40 maxillary single-rooted second premolars were selected and decoronated 2 mm above the cementoenamel junction (CEJ). Standard root canal treatments were performed, and the teeth were randomly divided into four groups. In the NiCr, NPG, and fiber post groups, metal crowns were fabricated and cemented using glass-ionomer cement. Biomimetic specimens were restored using indirectly applied modified resin-based fiber. A load at a 45 angle was applied to all specimens using a universal testing machine, and the fracture resistance of the teeth was measured. Fracture patterns were also assessed. Fracture data were analyzed using one-way analysis of variance (ANOVA), and pairwise comparisons were conducted using Tukey’s post hoc test. Failure patterns were analyzed using the Chi-square test. Results: The highest fracture resistance values were observed in teeth restored using NPG posts (4622.37 N), followed by fiber posts (4534.22 N), NiCr posts (3794.45 N), and the biomimetic method (2329.58 N), respectively. The fracture resistance of teeth restored with NPG (P< 0.001), fiber (P< 0.001), and NiCr (P=0.025) posts was significantly higher than that of teeth restored using the biomimetic method. The highest incidence of unrestorable fractures was found in the fiber post group (80.0%), followed by NiCr (70.0%), NPG (50.0%), and the biomimetic method (50.0%). Conclusion: Using the biomimetic method for restoring endodontically treated teeth resulted in lower fracture resistance than the studied post-core systems.
