fa تحلیل روند سال های از دست رفته عمر قبل و بعد کووید 19 در شمال ایران Analyzing The Trend of Years of Life Lost (YLL) Before and After COVID-19 in Northern Iran مديريت خدمات بهداشتي و درماني Health Services Management پژوهشي-کامل Research(Original) سابقه و هدف: کرونا ممکن است سبب تغییر در سال‌های از دست رفته عمرهم شده باشد. این مطالعه با هدف تحلیل روند مرگ و میر و سال های از دست رفته عمر قبل و بعد کووید 19 در شمال ایران، انجام پذیرفت. مواد و روش‌‌ها: این مطالعه مقطعی، بر اساس داده های تمامی مرگ‌های ثبت شده در استان مازندران انجام شد. برای تحلیل داده‌ها از آزمون روند کوکران آرمیتیج و آزمون من ویتنی در سطح معنی داری کم‌تر از 0.01 استفاده شد. آنالیزها داده‌ها به وسیله نرم‌افزارهای 14 Stata، 2013Excell  و23 SPSS انجام گرفت. یافته‌ها: <a name="_Hlk189563518"></a>میزان مرگ خام طی سال های مطالعه به ترتیب 424، 419، 432، 474، 575، 603، 496 در یک صد هزار نفر و میانگین YLL به ترتیب 19/7، 19/3، 18/6، 18/1 17/2، 17/8، 17/8 بوده است که حاکی از کاهش YLL در پیک پاندمی کرونا در این استان می باشد.<a name="_Hlk189450795"></a> <a name="_Hlk189515232"></a>آسیب‌پذیری بیش‌تر افراد مسن، کاهش مرگ به‌دلیل علل خارجی مرگ و مرگ ناشی از حوادث ترافیکی، بیش‌تر بودن مرگ مردان و مرگ در مناطق شهری از دیگر یافته‌ها بود. استنتاج: با این که کرونا سبب افزایش همه شاخص‌های مرگ مطالعه شد، ولی کاهش میانگین سال‌های عمر از دست رفته به علت مرگ زودرس و تغییر روند آن در دوره کرونا در این استان اتفاق افتاده است. پایین تر بودن سال‌های عمر از دست رفته به علت مرگ زودرس در شهرها می‌تواند به دلیل دسترسی بیش‌تر به خدمات درمانی باشد. توصیه می‌شود، با توجه به در دسترس بودن سامانه ثبت و طبقه بندی علل مرگ در سطح کشور، استفاده از این ظرفیت اطلاعاتی برای رصد وقایع مرتبط و تغییر شاخص‌های بهداشتی در راستای پایش مداوم و انجام اقدامات حمایتی و جبرانی در بحران هایی نظیر کرونا به این مهم توجه شود.   Background and purpose: The COVID-19 pandemic may have influenced the average years of life lost (YLL). This study aimed to analyze trends in mortality and YLL before and after the onset of COVID-19 in northern Iran. Materials and methods: This cross-sectional study was conducted using data from all recorded deaths in Mazandaran Province. The Cochran–Armitage trend test and the Mann–Whitney test were applied for data analysis, with a significance level set at P < 0.01. All analyses were performed using softwares of Stata version 14, Excel 2013, and SPSS version 23. Results: The crude death rates during the study period were 424, 419, 432, 474, 575, 603, and 496 per 100,000 population, while the corresponding average YLL were 19.7, 19.3, 18.6, 18.1, 17.2, 17.8, and 17.8, respectively, indicating a decrease in YLL during the peak of the COVID-19 pandemic in this province. The findings also indicated greater vulnerability among the elderly, a decline in deaths due to external causes and traffic accidents, a higher incidence of male deaths, and a greater number of deaths occurring in urban areas. Conclusion: Although COVID-19 increased all mortality indicators studied, the average number of years of life lost due to premature death decreased and its trend changed during the COVID-19 period in this province.
