fa بررسی فراوانی اتیولوژیک و الگوی مقاومت آنتی بیوتیکی پاتوژنهای مسبب آرتریت سپتیک در بیماران بستری در بیمارستان امام رضا (ع) مشهد در یک بازه پنج ساله Epidemiology and Antibiotic Resistance Patterns of Pathogens Causing Septic Arthritis in Hospitalized Patients at Mashhad Imam Reza Hospital: A Five-Year Retrospective Study عفوني infection گزارش کوتاه Brief Report سابقه و هدف: آرتریت سپتیک یکی از اورژانس‌های جدی حوزه‌ی پزشکی است که در صورت تأخیر در تشخیص و درمان، می‌تواند منجر به تخریب مفصل و عوارض سیستمیک قابل توجه شود. شناخت عوامل اتیولوژیک و مقاومت آنتی‌بیوتیکی این بیماری نقش مهمی در بهبود تصمیم‌گیری درمانی دارد. این مطالعه با هدف، بررسی الگوی اتیولوژی و مقاومت دارویی عوامل مولد آرتریت سپتیک در بیماران بستری در بیمارستان امام رضا (ع) مشهد طی سال‌های ۱۳۹۰ تا ۱۳۹۵، انجام پذیرفت. مواد و روش ‌ها: این پژوهش به‌صورت کوهورت گذشته‌نگر انجام شد. پرونده‌های بیماران با تشخیص قطعی آرتریت عفونی بر اساس معیارهای بالینی و آزمایشگاهی بررسی گردید. داده‌های جمعیت ‌شناختی، ویژگی‌های بالینی، نتایج کشت‌ها و الگوی مقاومت آنتی‌بیوتیکی استخراج و میان دو گروه سنی کودکان (زیر ۱۵ سال) و بزرگسالان (۱۵ سال و بالاتر) مقایسه شد. سطح معنی‌داری زیر 05/0 در نظر گرفته شد. یافته‌ها: در مجموع، ۲۱۶ بیمار با میانگین سنی 22/42 ± 27/53 سال وارد مطالعه شدند که ۱۵۸ نفر بزرگسال و ۵۸ نفر کودک بودند. شیوع بیماری در مردان بیش‌تر گزارش شد. فراوانی تب در گروه کودکان بیش‌تر و درد در آن‌ها کم‌تر از بزرگسالان بود. زانو شایع‌ترین مفصل درگیر بود و نرخ کشت خون مثبت در کودکان بیش‌تر مشاهده شد. استافیلوکوک اورئوس به عنوان فراوان‌ترین عامل میکروبی در کودکان شناسایی شد و نسبت به سفتریاکسون، لینزولاید و جنتامایسین بیش‌ترین حساسیت را نشان داد؛ در حالی که بالاترین مقاومت نسبت به پنی‌سیلین (100 درصد) و ایمی‌پنم (50 درصد) گزارش گردید. استنتاج: با توجه به شیوع بالای استافیلوکوک اورئوس در تمام گروه‌های سنی، توصیه می‌شود این باکتری در طراحی رژیم درمانی اولیه و تجربی بیماران مبتلا به آرتریت سپتیک به‌ویژه در مناطق با مقاومت دارویی بالا مد نظر قرار گیرد.   Background and purpose: Septic arthritis is a serious medical emergency that, if not diagnosed and treated promptly, can lead to irreversible joint damage and severe systemic complications. Accurate knowledge of the causative microorganisms and their antimicrobial resistance patterns is crucial for informing effective treatment strategies. This study was conducted to evaluate the distribution of pathogens and their resistance profiles in patients with septic arthritis hospitalized at Imam Reza Hospital, Mashhad, from 2011 to 2016. Materials and methods: This retrospective cohort study involved a review of medical records of patients with a confirmed diagnosis of septic arthritis, based on clinical and laboratory criteria. Demographic characteristics, clinical manifestations, culture findings, and antibiotic susceptibility data were extracted and analysed according to two age groups: children (<15 years) and adults (≥15 years). Statistical analyses were performed, with significance set at P < 0.05. Results: A total of 216 patients, with a mean age of 22.42 ± 27.53 years, were included; of these, 158 were adults and 58 were children. The condition was more prevalent in males. Fever occurred more frequently in children, whereas joint pain was more prominent in adults. The knee was the most commonly affected joint, and positive blood cultures were observed more often in children. Staphylococcus aureus was the predominant pathogen, demonstrating high sensitivity to ceftriaxone, linezolid, and gentamicin, while exhibiting complete resistance to penicillin (100%) and partial resistance to imipenem (50%). Conclusion: S. aureus is the predominant causative pathogen of septic arthritis across all age groups.
