fa ارتباط سطح کلسیم خون با شدت و پیامد بیماری در مبتلایان به کووید-19 Association between Blood Calcium Levels and Disease Severity and Outcomes in COVID-19 Patients داخلی Internal medicine گزارش کوتاه Brief Report سابقه و هدف: درمان بیماران کووید-19 بیش‌تر جنبه حمایتی داشت. احتمال تداوم طولانی مدت این بیماری، شناخت عوامل پیش بینی کننده برای شناسایی شدت بیماری و بهبود عواقب آن را مورد توجه قرار داد. این مطالعه با هدف بررسی، ارتباط سطح کلسیم خون با شدت و پیامد بیماری کووید-19، انجام پذیرفت. مواد و روش ‌ها: در این مطالعه مقطعی - توصیفی، اطلاعات ۱۶۷ بیمار بستری در بیمارستان امام خمینی ساری با تشخیص کووید-19، در سال 1400، ثبت شد. یافته‌ها: <a name="_Hlk163364161">میانگین سنی بیماران 17/14 </a>± 57/35 سال و نمایه توده بدنی (BMI) 07/ 5 ± 28/08کیلوگرم بر متر مربع بود. از مجموع بیماران، ۱۱۶ بیمار (69/5 درصد) به فرم شدید بیماری دچار بودند و شایع‌ترین علامت دیس‌پنه گزارش شد. میانگین کلسیم سرمی mg/dl 0/88 ± 8/45بود و 75 بیمار (44/9 درصد) هایپوکلسمی داشتند. تحلیل آماری نشان داد که هیچ ارتباط معنی‌داری بین سطح کلسیم با متغیرهایی از جمله سن، جنسیت، تعداد روز بستری در بیمارستان یا بستری در بخش مراقبت های ویژه (ICU)، لکوپنی، میزان درگیری ریوی، شدت بیماری، و میزان مرگ و میر وجود ندارد. تنها فاکتور التهابی پروتئین واکنشگر C (CRP) در گروه بیماران با هایپوکلسمی به طور معنی‌داری بالاتر از گروهی که هایپوکلسمی نداشتند، بود (0/036=P). استنتاج: کلسیم سرمی به تنهایی در مدیریت و ارزیابی بیماران کووید-19 توصیه نمی‌شود، زیرا سطح سرمی کلسیم نتوانست به عنوان یک نشانگر پیش‌آگهی برای شدت و میزان مرگ و میر بیماری کووید-19 استفاده شود. بنابراین، برای پیش‌بینی بهتر شدت بیماری و پیامدهای آن، نیازمند بررسی عوامل متعدد و همزمان است.   Background and purpose: The treatment of COVID-19 patients has been mainly supportive. The possibility of the long-term persistence of the disease necessitates the identification of predictive factors to determine disease severity and improve patient outcomes. This study investigated the association between blood calcium levels and the severity and clinical outcomes of COVID-19. Materials and methods: In this descriptive, cross-sectional study, data from 167 patients hospitalized at Sari Imam Khomeini Hospital, who were diagnosed with COVID-19 in 2021, were collected. Results: The mean age of the patients was 57.35 ± 17.14 years, and the body mass index (BMI) was 28.08 ± 5.07 kg/m². Among the total patients, 116 (69.5%) had severe disease, with dyspnea being the most common symptom reported. The mean serum calcium level was 8.45 ± 0.88 mg/dL, and 75 patients (44.9%) presented with hypocalcemia. Statistical analysis revealed no significant association between serum calcium levels and variables including age, gender, length of hospital and/or ICU stay, leukopenia, degree of pulmonary involvement, disease severity, or mortality. The only inflammatory factor, C-reactive protein (CRP), was significantly higher in the hypocalcemia group than in the non-hypocalcemia group (P = 0.036). Conclusion: Serum calcium alone is not sufficient for the management and assessment of COVID-19 patients, as serum calcium levels cannot serve as a prognostic marker for disease severity or mortality.
