fa بررسی شیوع ضایعات پره‌نئوپلاستیک معده در نمونه‌های پاتولوژی بیماران مبتلا به دیس ‌پپسی مراجعه ‌کننده برای اندوسکوپی به بیمارستان فیروزآبادی (1401-1397) Prevalence of Preneoplastic Gastric Lesions in Pathology Samples of Dyspeptic Patients Undergoing Endoscopy at Firoozabadi Hospital (2018–2022) آسيب شناسي(پاتولوژی) Pathology پژوهشي-کامل Research(Original) سابقه و هدف: سرطان معده یکی از شایع‌ترین سرطان‌ها می‌باشد و از عوامل مهم مرگ‌ومیر در جهان و ایران است. ضایعات پره‌نئوپلاستیک معده (شامل آتروفی، متاپلازی روده‌ای، دیسپلازی) در پیشرفت به سمت آدنوکارسینوم معده نقش مهمی دارند. این مطالعه با هدف بررسی، شیوع این ضایعات پره‌نئوپلاستیک را در بیماران با علایم دیس‌پپسیک مراجعه کننده به بیمارستان فیروزآبادی، انجام پذیرفت، تا داده‌هایی برای پیشگیری از سرطان معده ارائه دهد. مواد و روش ‌ها: مطالعه مقطعی گذشته‌نگر، روی 327 بیمار دیس‌پپسیک تحت آندوسکوپی در بیمارستان فیروزآبادی (1401 – 1397) انجام شد. معیارهای ورود در این مطالعه شامل سن بالای 18 سال، وجود علایم دیس‌پپسی، انجام نمونه‌برداری از مخاط معده و کامل بودن پرونده بیمار می‌باشد. بیماران با تشخیص سرطان معده، سابقه جراحی معده، حاملگی یا عدم رضایت بیمار از مطالعه خارج شدند. ضایعات پره‌نئوپلاستیک و هلیکوباکتر پیلوری با سیستم آپدیت‌ شده سیدنی جهت گزارش پاتولوژی ارزیابی، و داده‌ها با SPSS نسخه 26 تحلیل شدند. یافته‌ها: از 327 بیمار (54/7 درصد مرد، 45/3درصد زن، میانگین سنی 51/63 ± 19/08 سال)، 82/6 درصد (270 نفر) عفونت هلیکوباکتر پیلوری داشتند (0/006= P). متاپلازی روده‌ای در 4/21 درصد (0/001< P)، دیسپلازی در 1/8 درصد (0/228= P) و آتروفی معده در 0/61درصد (0/00= P) دیده شد. زخم معده 20 درصد (0/001< P)، التهاب حاد 6/71 درصد (0/001< P) و التهاب مزمن 22 درصد (0/706=P) گزارش شد. متاپلازی روده‌ای با هلیکوباکتر پیلوری (0/01= P) و سن (0/001= P) ارتباط داشت. استنتاج: شیوع بالای هلیکوباکتر پیلوری، التهاب حاد و متاپلازی روده‌ای در بیماران دیس‌پپسیک بر ضرورت غربالگری و درمان این عفونت برای پیشگیری از سرطان معده در ایران تأکید دارد. تشخیص و مداخله زود هنگام می‌تواند از پیشرفت ضایعات پره‌نئوپلاستیک جلوگیری کند.   Background and purpose: Gastric cancer is one of the most common malignancies and a major cause of cancer-related mortality worldwide, including Iran. Gastric preneoplastic lesions (including atrophy, intestinal metaplasia, and dysplasia) play an important role in the progression to gastric adenocarcinoma. The aim of this study was to determine the prevalence of these preneoplastic lesions in patients with dyspeptic symptoms referred to Firoozabadi Hospital during 2018–2022. The findings are intended to provide data to support gastric cancer prevention strategies. Materials and methods: This retrospective cross-sectional study was conducted among 327 dyspeptic patients who underwent endoscopy at Firoozabadi Hospital. Inclusion criteria included age over 18 years, presence of dyspeptic symptoms, gastric mucosal biopsy, and complete medical records. Patients with a diagnosis of gastric cancer, history of gastric surgery, pregnancy, or unwillingness to participate were excluded. Preneoplastic lesions and Helicobacter pylori infection were evaluated based on the Updated Sydney System in pathology reports, and data were analyzed using SPSS version 26. Results: Of 327 patients (54.7% male, 45.3% female; mean age 51.63 ± 19.08 years), 82.6% (270 patients) had H. pylori infection (p=0.006). Intestinal metaplasia was observed in 21.4% (p<0.001), dysplasia in 1.8% (p=0.228), and gastric atrophy in 0.61% (p=1.00). Peptic ulcers (20%; p<0.001), acute inflammation (71.6%; p<0.001), and chronic inflammation (22%; p=0.706) were reported. Intestinal metaplasia was significantly associated with H. pylori (p=0.01) and age (p=0.001). Conclusion: The high prevalence of Helicobacter pylori infection, acute inflammation, and intestinal metaplasia in dyspeptic patients underscores the necessity of screening and treatment of this infection to prevent gastric cancer in Iran.
