fa مرور نظام‌مند پویایی‌های پنهان ترخیص بر خلاف توصیه پزشک در ایران A Systematic Review of the Underlying Drivers of Discharge Against Medical Advice in Iran مديريت خدمات بهداشتي و درماني Health Services Management مروری Review سابقه و هدف: ترخیص بر خلاف توصیه پزشک (DAMA) یکی از چالش‌های عمده نظام‌های سلامت است که با پیامدهای نامطلوب بالینی، اخلاقی و اقتصادی همراه است. در ایران، مطالعات متعددی به این پدیده پرداخته‌اند؛ با این حال، شواهد موجود پراکنده بوده و به‌ندرت دیدگاه‌های بیماران و کارکنان نظام سلامت را به‌طور یکپارچه بررسی کرده‌اند. این مطالعه با هدف، مرور نظام‌مند DAMA در ایران از دیدگاه بیماران و کارکنان نظام سلامت، انجام پذیرفت. مواد و روش ‌ها: این مرور نظام‌مند، مطابق با PRISMA 2020 انجام شد. جستجوی جامع در PubMed، Scopus، Web of Science، SID و MagIran بدون محدودیت زمانی صورت گرفت. فرمول جستجوی PubMedشامل ((("Discharge Against Medical Advice"[Mesh]) OR ("Agains t MedicalAdvice"[tiab]) OR ("DAMA"[tiab])) AND ("Iran"[Mesh] OR "Iran"[tiab])) NOT ("Editorial"[pt] OR "Letter"[pt] OR "Comment"[pt])، بود. عملگرهای بولی AND، OR و NOT به کار رفتند. مطالعات کمی، کیفی و آمیخته مرتبط با DAMA در ایران وارد شدند. کیفیت مطالعات با ابزارهای NOS، JBI، CASP و MMAT ارزیابی گردید. دو داور مستقل داده‌ها را استخراج و با رویکرد تماتیک سنتز کردند. یافته‌ها: در مجموع ۵۰ مطالعه (۴۲ کمی، ۶ کیفی، ۲ آمیخته) وارد مرور شدند. ترکیب تماتیک چهار تم اصلی، فشارهای اقتصادی و ناامنی مالی به‌عنوان محرک اصلی DAMA، گسست‌های ارتباطی و فرسایش اعتماد در روابط بیمار-ارائه‌ دهنده، تجربه‌های هیجانی منفی از بستری و تلاش برای باز پس‌گیری عاملیت و تعارض‌های حرفه‌ای، اخلاقی و ساختاری از دیدگاه کارکنان سلامت، را آشکار ساخت. یافته‌ها نشان داد که DAMA در ایران پدیده‌ای چند بعدی و چند صدایی است. استنتاج: DAMA در ایران حاصل تعامل عوامل فردی، ارتباطی، حرفه‌ای و ساختاری است. مداخلات مؤثر نیازمند راهبردهای یکپارچه‌ای هستند که هم‌زمان به تجربه بیماران، چالش‌های کارکنان و اصلاحات ساختاری بپردازند.   Background and purpose: Discharge Against Medical Advice (DAMA) is a major challenge for health systems, with significant adverse clinical, ethical, and economic outcomes. In Iran, numerous studies have examined this phenomenon; however, the available evidence is fragmented and seldom integrates the perspectives of both patients and healthcare providers. This study aimed to systematically review DAMA in Iran from the perspectives of patients and healthcare providers. Materials and methods: This systematic review was conducted according to the PRISMA 2020 guidelines. A comprehensive search was performed across PubMed, Scopus, Web of Science, SID, and MagIran without any time restriction. The PubMed search formula was: ((("Discharge Against Medical Advice"[Mesh]) OR ("Against Medical Advice"[tiab]) OR ("DAMA"[tiab])) AND ("Iran"[Mesh] OR "Iran"[tiab])) NOT ("Editorial"[pt] OR "Letter"[pt] OR "Comment"[pt]). Boolean operators AND, OR, and NOT were applied. Quantitative, qualitative, and mixed-methods studies addressing DAMA in Iran were included. Study quality was assessed using the NOS, JBI, CASP, and MMAT assessment tools. Two independent reviewers extracted the data and synthesized the findings using a thematic approach. Results: A total of 50 studies met the eligibility criteria, including 42 quantitative, 6 qualitative, and 2 mixed-methods studies, and were included in the review. Thematic synthesis revealed four main themes: (1) economic pressures and financial insecurity as the primary driver of DAMA; (2) communication breakdowns and the erosion of trust in patient-provider relationships; (3) negative emotional experiences during hospitalisation and attempts to regain personal agency; and (4) professional, ethical, and structural conflicts from the perspective of healthcare providers. Overall, the findings demonstrated that DAMA in Iran is a multidimensional and multifaceted phenomenon. Conclusion: DAMA in Iran results from the interaction of individual, communicative, professional, and structural factors.
