fa از چالش تا فرصت: بررسی زیرساخت‌ها و دیدگاه ارائه‌دهندگان خدمت در راه‌اندازی مشاوره شیردهی از راه دور From Challenges to Opportunities: Exploring Infrastructure and Healthcare Providers’ Perspectives on Telelactation Counselling مامائی Midwifery پژوهشي-کامل Research(Original) سابقه و هدف: مشاوره شیردهی یکی از مؤثرترین مداخلات در ارتقای سلامت مادر و نوزاد است، اما محدودیت‌های زمانی و مکانی می‌تواند دسترسی مادران به خدمات تخصصی را کاهش دهد. ارائه مشاوره شیردهی از راه دور به‌عنوان یک رویکرد نوین می‌تواند این محدودیت‌ها را برطرف کند. این مطالعه با هدف بررسی امکان‌سنجی ارائه این خدمت از دیدگاه ارائه‌ دهندگان خدمات سلامت انجام شد. مواد و روش ‌ها: این مطالعه توصیفی- مقطعی، بر روی ۵۰ نفر از کارکنان مراکز جامع و پایگاه‌های سلامت شهرستان ملارد در سال 1402، به روش سرشماری انجام شد. داده‌ها با پرسشنامه استاندارد امکان‌سنجی خدمات سلامت از راه دور گردآوری و با استفاده از آمار توصیفی و آزمون‌های استنباطی (همبستگی پیرسون، t مستقل و من-ویتنی) در نرم‌افزار SPSS-23 تحلیل شدند. یافته‌ها: در این مطالعه میانگین سن شرکت‌ کنندگان 95/4± ۳۰٫۳۰ سال بود. ارائه ‌دهندگان خدمت در تمامی حیطه‌ها شامل نگرش، قصد رفتاری، درک سودمندی، سهولت استفاده، سازگاری و امنیت داده‌ها سطح مطلوبی از آمادگی را نشان دادند، به‌طوری‌ که در میان حیطه‌ها، بالاترین میانگین به «اعتماد در مرکز مشاوره از راه دور » با69/3± 28/29 و «ارتباط با بیمار » با63/3 ± 36/27 اختصاص داشت. در ارزیابی زیرساخت‌ها، تنها ۴۸ درصد از امکانات فنی و تجهیزاتی مورد نیاز در مراکز فراهم بود. تحلیل‌های استنباطی نیز نشان داد که نمره پذیرش ارتباط معنی‌داری با سن و سابقه کار نداشت، اما در پرسنل پایگاه‌های سلامت (0/014= P) و افراد دارای سابقه قبلی تله‌مدیسین (0/007= P) به طور معناداری بالاتر بود. استنتاج: با وجود نگرش مثبت و آمادگی رفتاری مطلوب ارائه ‌دهندگان خدمت برای به‌کارگیری مشاوره شیردهی از راه دور، زیرساخت‌های موجود به شدت ناکافی است. این عدم توازن نشان می‌دهد که چالش اصلی استقرار این خدمت نه در پذیرش نیروی انسانی، بلکه در محدودیت‌های تجهیزاتی و فنی نهفته است، لذا موفقیت اجرای این برنامه مستلزم اولویت‌دهی به تامین زیرساخت‌ها، ارتقای تجهیزات و پشتیبانی سازمانی است.   Background and purpose: Breastfeeding counselling is one of the most effective interventions for improving maternal and neonatal health; however, geographical and time-related barriers often limit mothers’ access to specialized services. Telelactation counselling has emerged as a promising approach to addressing these barriers. This study aimed to assess the feasibility of implementing telelactation counselling from the perspective of healthcare providers. Materials and methods: This cross-sectional study was conducted using a census sampling approach among 50 healthcare providers working in comprehensive health centres and health posts in Malard County. Data were collected using a standardized Telehealth Feasibility Questionnaire and analysed using descriptive statistics and inferential tests, including the Pearson correlation, independent-samples t-test, and Mann-Whitney U test, in SPSS version 23. Results: The mean age of the participants was 30.30 ± 4.95 years. Healthcare providers demonstrated a high level of readiness across all assessed domains, including attitude, behavioural intention, perceived usefulness, perceived ease of use, compatibility, and data security. The highest mean scores were observed for “trust in the telehealth centre” (29.28 ± 3.69) and “patient communication” (27.36 ± 3.63). Regarding infrastructure, only 48% of the required technical and equipment resources were available at the centres. Inferential analyses revealed that the total acceptance score was not significantly associated with age or work experience; however, it was significantly higher among health post staff than among health centre staff (P = 0.014) and among participants with previous telemedicine experience (P = 0.007). Conclusion: Despite healthcare providers’ positive attitudes and favourable behavioural readiness towards implementing remote breastfeeding counselling, substantial infrastructure limitations remain. This discrepancy suggests that the primary challenge to implementing this service lies not in workforce acceptance but in the availability of appropriate technical infrastructure and equipment.
