fa بررسی میزان IgE اختصاصی بر علیه کاندیدا آلبیکنس در مبتلایان به درماتیت آتوپیک مراجعه کننده به درمانگاه پوست بیمارستان بوعلی سینا ساری 1385-1383 پژوهشي-کامل Research(Original) <div style="text-align: justify direction: rtl"> <span style="font-weight: bold">سابقه و هدف: </span>کاندیدا آلبیکنس به عنوان یکی از اعضای کوچک طبیعی ساکن (Microflor) بدن انسان می تواند باعث رهاسازی دایمی مواد حساسیت زا در افراد حساس و در نتیجه سبب تشدید بیماری های حساسیتی مزمن نظیر التهاب ارثی پوست (Atopic Dermatitis) شود. هدف از مطالعه حاضرتعیین میزان شیوع IgE اختصاصی برعلیه کاندیدا آلبیکنس در بیماران مبتلا به التهاب ارثی پوست مراجعه کننده به بیمارستان بوعلی ساری طی سال های 85-1383 می باشد.<br><span style="font-weight: bold">مواد و روش ها:</span> 120 بیمار مبتلا به التهاب ارثی پوست (52 مرد و 68 زن) مراجعه کننده به بیمارستان بوعلی ساری وارد مطالعـه شدند. طیف سنی بیماران 4 ماه تا 60 سال با میانگین 9/12 سال بود. معیارهای تشخیص برای بیماری، معیارهای هانیفین و راجکا (Hanifin & Rajka) بود. سرم این بیماران با استفاده از اسباب های اندازه گیری IgE کلی (RADIM) و بر اساس دستورالعمل کارخانه سازنده اندازه گیری شد و برای تعیین میزانIgE اختصاصی از اسباب های (Kits) طراحی شده به وسیله IgE خاص حساسیت انسان(ALerCHEK'S Allergen specific human) و بر اساس دستور کارخانه سازنده استفاده گردید.<br><span style="font-weight: bold">یافته ها:</span> از مجموع 120 بیمار مورد مطالعه، در 37 نفر (8/30 درصد) سطح IgE کلی بیش از IU/ml 100 و در 44 نفر (7/63) درصد 20-100 IU/ml و در 39 نفر (5/32 درصد) کمتر از100IU/mlبود. از 68 بیمار زن، 20 نفر (4/29 درصد) و از 52 بیمار مرد 17 نفر (7/32 درصد) دارای IgE کلی بیش از 100IU/ml بودند. از مجموع 120 بیمار مورد مطالعه 9 نفر (5/7 درصد) IgEاختصاصی بر علیه کاندیدا آلبیکنس را نشان دادند که از این تعداد 6 نفر (6/66 درصد) زن بودند.<br><span style="font-weight: bold">استنتاج:</span> نتایج مطالعه حاضر در مقایسه با سایر تحقیقات، هماهنگی نسبی میزان IgE کلی را در بیماران نشان می دهد. اما درصد بیماران مبتلا به التهاب ارثی پوستبا IgE اختصاصی بر علیه کاندیدا آلبیکانس در مطالعه حاضر پایین تر بود که به نظر می رسد عوامل مختلفی نظیر سن بیماران مورد مطالعه، شدت بیماری، روش ارزیابی IgE اختصاصی و همچنین سطح IgE کلی در آن تاثیرگذار باشد. </div> <span style="font-weight: bold">Background and purpose:</span> Candida albicans (C. albicans) as a micro flora of the human could be responsible for a continuous release of allergen and may be responsible for chronic atopic dermatitis (AD) in sensitive patients. Thus, in this study, we analyzed AD patients for total IgE and specific IgE, against C. albicans. <br><span style="font-weight: bold">Materials and Methods:</span> A total of 120 AD patients (male 52 and female 68) were introduced in this study. The age range varied from 4 months to 60 years (mean about 12.9 years). Serum total IgE was assayed by ELISA kit (RADIM). Solid phase was captured by sandwich ELISA assay, using a micro well format for the determination of serum specific IgE to C. Albicans was used according to the manufacturer’s instructions, (ALerCHEK Allergen specific human IgE). <br><span style="font-weight: bold">Results: </span>Of the 120 AD patients, 37 subjects (30.8%) had total IgE higher than 100 IU/mL, 44 subjects (63.7 %) 20-100IU/mL and 39 subjects (32.5%) less than 20 IU/mL. 9 (7.5%) of the patients had specific IgE against C. albicans. Among the patients who were positive for specific IgE to C. albicans, 6 (66.7%) were women. <br><span style="font-weight: bold">Conclusion:</span> The result of our study on serum total IgE in AD patients is concordant with other studies from different countries. In comparison to other studies, our AD patients showed less frequency of specific IgE against Candida albicans.
