fa تاثیر توکسین بوتولینیوم در درمان مبتلایان به فیشر آنال مزمن آیدیوپاتیک پژوهشي-کامل Research(Original) <div style="text-align: justify direction: rtl"> <span style="font-weight: bold">سابقه و هدف: </span>فیشر آنال مزمن ایدیوپاتیک، بیماری طولانی مدتی است که با دفع دردناک مدفوع و خونریزی همراه است. هدف از انجام این مطالعه مقایسه تاثیر تزریق توکسین بوتولینیوم (BT) و جراحی در درمان بیماران مبتلا به این بیماری می باشد.<br><span style="font-weight: bold">مواد و روش ها </span>: در یک کارآزمایی بالینی 40 بیمار مبتلا به CIAF ، به طور مساوی (هر گروه 20 نفر) تحت تزریق 20 واحد BT در دو طرف فیشر یا جراحی اسفنکتروتومی جانبی داخلی (LIS) قرار گرفتند. بیماران از نظر عوارض و بهبودی یک هفته، یک، دو و چهار ماه پس از درمان پیگیری شدند.<br><span style="font-weight: bold">یافته ها:</span> میزان بهبودی در گروه LIS در ماه اول، 80 درصد (16 نفر) بود در حالی که گروه BT بهبودی نداشت (P<0.05) در ماه چهارم 17 نفر از گروه جراحی و 11 نفر از گروه BT بهبود یافته بودند (P=0.082)شدت درد در هفته اول در هر دو گروه به طور معناداری کاهش یافت (P<0.05) این کاهش در گروه BT تا ماه اول ادامه داشت و پس از آن تا پایان پیگیری کاهش معناداری نیافت. ولی به طور کلی میزان درد در تمام مراحل پیگیری در گروه LIS به طور معناداری کمتر از گروه BT بود (P<0.05) عوارض جانبی در دو گروه تفاوت معنادار نداشت.(P<0.05) <br><span style="font-weight: bold">استنتاج: </span>بهبودی در بیماران پس از تزریق BT با LIS تفاوت معناداری نداشته ولی دیرتر ظاهر می شود. BT می تواند به عنوان اولین قدم درمانی برای بیماران CIAF انجام شود. جهت مشاهده اثرات درمانی آن پیگیری بیماران حداقل تا 4 ماه ضروری است. </div> <span style="font-weight: bold">Background and purpose:</span> Anal fissure is a chronic condition characterized by painful defecation and rectal bleeding. The aim of this study was to compare the injection of botulinum toxin (BT), versus with surgical lateral internal sphincterotomy (LIS). <br> <span style="font-weight: bold">Materials and Methods:</span> In this clinical trial, 40 patients enrolled with chronic idiopathic anal fissure (CIAF) and were divided into 2 groups (20 BT group and 20 LIS group). Both groups were compared according to complications and healing process during one week, following, 2 and 4 months after interventions. <br><span style="font-weight: bold">Results: </span>There were no statistical significant differences between 2 groups as a viewpoint of duration of disease, pain and other symptoms at the beginning of the study. The rate of healing in the LIS group was greater than the BT group within the first month (16/0), and second month (16/7) (p<0.05), however, there were no significant differences between the two groups in the 4th month (17/11) of the study (p>0.05). In comparison with the BT group, the severity of pain in the LIS group decreased significantly. There were no significant differences between the two groups regarding complications. <br><span style="font-weight: bold">Conclusion:</span> Healing in the BT group was approximately similar to the LIS yet it appears to occur slower than LIS. Therefore, according to the results of the study, injection of the BT is recommended within the first step of CIAF treatment.
