fa طراحی و ارزیابی کیت آنتی بادی ضد کراتین [AKA] در بیماران آرتریت روماتویید پژوهشي-کامل Research(Original) <div style="text-align: justify direction: rtl"> <span style="font-weight: bold">سابقه و هدف:</span> آرتریت روماتویید از بیماری های روماتیسمی منتشر خود ایمن است. اتو آنتی بادی های کشف شده در این بیماری از نظر تشخیص و پیش آگهی با ارزش می باشند. یکی از این آنتی بادی ها، آنتی بادی ضد کراتین (AKA). است که با کراتین فیبری در اپیدرم و ضمائم اپیدرمال و لایه های شاخی شده اپی تلیوم مری رت واکنش می دهد. ابتدا کیت AKA-IFA طراحی، حساسیت و ویژگی آن محاسبه می شود سپس شیوع آنتی بادی را در بیماران آرتریت روماتویید (RF) بررسی و در پایان نتایج AKA با نتایج آزمایش RF مقایسه شد.<br><span style="font-weight: bold">مواد و روش ها:</span> با استفاده از آنتی ژن های پروتیینی لایه شاخی مری رت و استفاده از آنتی هیومن IgG کونژوگه به ماده فلورسنت، کیت AKA طراحی گردید. برای سنجش حساسیت و ویژگی کیت، 52 بیمار مبتلا به آرتریت روماتویید با استفاده از معیار کالج روماتولوژی امریکا با میانگین سنی15.8±48.0 انتخاب شد و نتایج آزمایش AKA در آنها با گروه کنترل بیمار (23 بیمار مبتلا به بیماری های روماتیسمی و غیر مبتلا به آرتریت روماتویید) با میانگین سنی 16.9±32.5 و گروه کنترل سالم 30 نفر با میانگین سنی 16.9±32.1 مورد مقایسه قرار گرفت. جهت تعیین دقت کیت، روش Inter assay و Inter assay مورد استفاده قرار گرفت. RF بیماران بررسی و نتایج آن با نتایج AKA شده مقایسه گردید.<br><span style="font-weight: bold">یافته ها:</span> از 52 بیمار آرتریت روماتویید، 39 مورد (75 درصد) و در گروه کنترل بیمار 3 مورد (13 درصد) و در گروه کنترل سالم 1 مورد (3.3درصد) AKA مثبت بودند. کیت طراحی شده AKA از دقت 100 درصد به روشن Inter assay و 98 درصد به روش Inter assay برخوردار بوده و حساسیت و ویژگی تست AKA در رقت 1.10 سرم به ترتیب 75 و 92.5 درصد بود. در حالی که حساسیت و ویژگی RF به ترتیب 88.5 و 86.8 درصد محاسبه شد.<br><span style="font-weight: bold">استنتاج: </span>نتایج نشان داد، بهترین حساسیت و ویژگی آزمایش برای بیماران آرتریت روماتویید، تیتر۱۰/۱ است به عبارتی رقت۱۰/۱ به عنوان حداقل تیتر معنی دار (Cut off) در تشخیص و تایید آرتریت روماتویید می باشد و با توجه به دقت کیت طراحی شده، سنجش –IFA AKA دارای اعتبار علمی بالایی در تشخیص و تایید بیماری آرتریت روماتویید می باشد. </div> <span style="font-weight: bold">Background and purpose:</span> Rheumatoid Arthritis (RA) is a systemic auto immune Rheumatoid diseases. Auto antibodies are detected in this disease, with diagnostic and prognostic properties. One of them is AKA which reacts with the fibrous keratin in epiderm and the stratum corneum of rat esophageal epithelium. Hence at first AKA – IFA kit was designed, then its sensitivety and specificity was measured, and the titer of Anti body was evaluated in RA patients, and finaly the results of AKA were compared with that of RF test. <br><span style="font-weight: bold">Materials and method:</span> AKA-IFA kit was designed with protein antigens in the stratum corneum of rat esophageal and anti human IgG conjugated to FITC. 52 patients with RA (mean age 48.0 ±15.8) according to the American College of Rheumatology (ACR) criteria were selected for measuring the sensitivity and specificity. The results of AKA test in sera of RA patients were campared with 23 sera of patient control groupe (mean age 32.5 ±16.4) and 30 sera of healthy control groupe (mean age 32.1 ±16.9). Inter and intra assay method was used to determinig precision of AKA kit. RF test was also performed and it's results compared with result of designed AKA kit. <br><span style="font-weight: bold">Results: </span>AKA were found in 75% of patients with RA (39/52), 13% of patient control groupe (3/23) and 3.3% of healthy control groupe .(YBO) The designed AKA kit by inter and intra assay method had 100% and 98% percision respectively. The sensitiviety and specificity of AKA in (1/10) serum dilution was 75% and 92.5% respectively but sensitiviety and specificity of RF was 88.5% , 86.8% respectively. <br><span style="font-weight: bold">Conclusion:</span> According to the results, IFA– AKA test could be diagnostic and confirmative for RA , And AKA in (1/10)serum dilution has the best diagnostic Value for RA.
