fa مقایسه عوارض عفونی عمل جراحی هیسترکتومی آبدومینال در دو رژیم آنتی بیوتیکی 24 ساعته و 7 روزه پژوهشي-کامل Research(Original) <div style="text-align: justify direction: rtl"> <span style="font-weight: bold">سابقه و هدف:</span> هیسترکتومی یکی از رایج ترین اعمال جراحی و عفونت بعد از عمل نیز شایع ترین عارضه بعد از آن است و شامل عفونت دستگاه تنفسی، عفونت دستگاه ادراری، عفونت زخم، عفونت کاف واژن، آبسه ها و ... می باشد. مصرف آنتی بیوتیک پروفیلاکسی یکی از راه های پیشگیری از عفونت بعد از عمل هیسترکتومی است. این بررسی به منظور مقایسه عوارض عفونی عمل جراحی هیسترکتومی آبدومینال در دو رژیم آنتی بیوتیک 24 ساعته و 7 روزه صورت گرفته است.<br><span style="font-weight: bold">مواد و روش ها:</span> این مطالعه به صورت کارآزمایی بالینی کنترل شده تصادفی بوده است. دراین بررسی، 306 بیمار که دارای شرایط ورود به مطالعه بودند به صورت تصادفی به دو گروه تقسیم شدند. 6 بیمار به دلیل نیاز به دستکاری مثانه و حالب و روده از مطالعه خارج شدند. 300 بیمار باقی مانده به صورت تصادفی در دو گروه آنتی بیوتیک 24 ساعته و 7 روزه قرار گرفتند. هر دو گروه از جهت عوارض تب دار بعد از عمل در طی مدت بستری و تا دو هفته بعد از عمل پیگیری شدند.<br><span style="font-weight: bold">یافته ها:</span> دو گروه از نظر سن، میزان تحصیلات،BMI و هموگلوبین قبل از عمل و تعداد روزهای بستری قبل از عمل و زمان عمل جراحی متناظر بودند. در گروه آنتی بیوتیک 24 ساعته (150 نفر)، 15 نفر (10 درصد) دچار عفونت بعد از عمل شدند و در گروه آنتی بیوتیک 7 روزه (150 نفر )، در 12 نفر (8 درصد) عفونت بعد از عمل مشاهده شد که تفاوت آماری معنی داری نداشت(p<0.05) .از نظر روزهای بستری بعد از عمل نیز تفاوت معنی داری بین دو گروه مشاهده نشد (P=0.131).<br><span style="font-weight: bold">استنتاج: </span>به دلیل عدم اختلاف عوارض تب دار بعد از عمل در مصرف آنتی بیوتیک 24 ساعته و 7 روزه می توان مصرف آنتی بیوتیک پروفیلاکسی بعد از هیسترکتومی را تنها به 24 ساعت محدود کرد و از مصرف بدون علت آنتی بیوتیک خوراکی در منزل بعد از ترخیص که متاسفانه در کشور ما متداول است، جلوگیری کرد. </div> <span style="font-weight: bold">Background and purpose:</span> Hysterectomy is one of the most common surgery in women and is followed by post-operative infections of respiratory and urinary tracts, wound, volva and boils. Prophylaxis with antibiotics is a preventive measure against post-hysterectomy infections. This comparitive study was conducted to compare the abdominal post-hysterectomy infections managed with 24 h-versus 7 days-antibiotic medication regimen. <br> <span style="font-weight: bold">Materials and methods : </span>The study design was a randomized controlled clinical trial. 506 patients matching the criteria of the study were selected and randomly allocated into two groups. 6 patients were excluded from the study because of the need for intervension of bladder pelvis and intestine. 300 patients were divided randomly in two groups of 24 h-and 7 days-antibiotic therapy, both groups were followed up for febrile complications while admitted and for up to two weeks later. <br> <span style="font-weight: bold">Results: </span>Two groups were matched for age, level of education, BMÏ, hemoglobin prior to operation number of the admitted days and duration of surgery.
