درمان بیماری پیرونی با استفاده از امواج شوک خارج بدنی (ESWT)

<span style="font-weight: bold">سابقه و هدف: </span>از زمان اولین گزارش بیماری پیرونی در سال 1743 تاکنون روش های متعددی جهت درمان طبی و جراحی آن به کار رفته است. اما هیچ یک به طور کامل رضایت بخش نبوده است، هدف از این مطالعه ارزیابی تاثیر استفاده از امواج شوک خارج بدنی در درمان بیماری پیرونی می باشد.<br><span style="font-weight: bold">مواد و روش ها: </span>پس از اخذ رضایت، 12 فرد مبتلا به بیماری پیرونی وارد مطالعه شدند. میانگین سنی بیماران 4+ 52 سال بود. قبل از درمان، درجه کجی آلت پس از ایجاد نعوظ(Erection) مصنوعی با پاپاورین اندازه گیری شد. اندازه پلاک با استفاده از سونوگرافی بررسی و ثبت شد. هم چنین کیفیت نعوظ، وجود یا عدم وجود درد در هنگام نعوظ و مقاربت و مدت زمان بیماری ثبت گردید. جهت هر بیمار 3 بار ESWT با فاصله 2 هفته و هر بار با 2500 موج شوک صورت گرفت. 6 ماه بعد، پارامترهای بررسی شده قبل از درمان مجددا بررسی و ثبت شد.<br><span style="font-weight: bold">یافته ها:</span> پس از درمان در 10 بیمار (83 درصد) درد به طور کامل از بین رفت(P<0.05) . میانگین سایز پلاک قبل و بعد از درمان به ترتیب 3.03±1.2 و 0.99±0.9 سانتی متر بود. این اختلاف از نظر آماری معنی دار بود (P=0.001). میانگین میزان کجی آلت قبل و بعد از درمان به ترتیب 34±5 و 23±6 درجه بود. این اختلاف از نظر آماری معنی دار بود (P=0.001). از 9 بیماری که کیفیت نعوظ شان بد بود، 77 درصد بعد از درمان، بهبودی رضایت بخش را گزارش نمودند. هیچ عارضه جدی به دنبال درمان مشاهده نشد. میانگین زمان پی گیری 8 ماه بود(13-6 ماه).<br><span style="font-weight: bold">استنتاج:</span> ESWT در درمان بیماری پیرونی به خوبی تحمل شده و با عارضه جدی همراه نیست. مطالعه حاضر نشان داد که ESWT در کاهش درد و بهبود وضعیت جنسی بیماران و کاهش کجی آلت موثر است.

<span style="font-weight: bold">Background and purpose :</span> Since the first report of the pyronier’s disease in 1374, different thrapeutic and surgical methods have been used for treatment. However, none resulted in complete satisfactory outcome. The aim of this study was to treat the disease using exteral shook wave thrapy (ËSWT).

<span style="font-weight: bold">Materials and methods:</span> Tweixe patients were included in the study after singning the writhen consent form. The mean age of the patients was 52 ± 4 years. The degree of deviation following erection induced by papaverin was meaxired in all patients before the treatment.
The plaque size was measured and nearded. The quality of erection and presence of pain during erection and intercourse were also recorded. Ëach patient underwent three rounds of 2500 wave ËSWT in a two week interval regime. Sox months later the same parameters were assessed again.
<br><span style="font-weight: bold">Results :</span> Ïn ten patients, the pain was completely vanished after the treatment (P<0.05). the mean size of the plaque before and after the treatment was 3.03 ± 1.2 and 0.99 ± 0.9 cm respectively with a singnificant difference (P=0.001).
The mean of deviation of penis was 34 ± 5 and 23 ± 6 with a significant different (P=0.001) degreen before and after treatment. The quality of erection inproved satisfactority in 77% of the patients. No adverse effect, were observed after a mean follow up period of 8 montho (6-13 months).
<br><span style="font-weight: bold">Çonclusion :</span> ËSWT was well tolerated in pyronier’s disease treatment with no serious side effects and that the method significantly reduced the pain and deviation of the penis and improved the quality of sexual activity of the patients.
