بررسی میزان پراکندگی و الگوی مقاومت آنتی بیوتیکی سویه های انتروکوک جدا شده از نمونه های بالینی بیمارستان های آموزشی شهر همدان

چکیده
سابقه و هدف: انتروکوکهای دارای مقاومت دارویی چندگانه، به عنوان پاتوژنهای مهم بیمارستانی در حال افزایش میباشند. آگاهی از الگوی مقاومت ضد میکروبی در تدوین دستورالعملهای درمان عفونتهای انتروکوکی ضروری میباشد. در این مطالعه مقاومت ضد میکروبی انتروکوکهای ایزوله شده از بیمارستانهای آموزشی شهر همدان مورد بررسی قرار گرفت.
مواد و روشها: این تحقیق بر روی 242 سویه انتروکوک فکالیس و انتروکوک فاسیوم جدا شده از نمونههای بالینی بیماران بستری و سرپایی در بیمارستانهای آموزشی شهر همدان در طی سال های 1393-1391 انجام گرفت. گونههای شناسایی شده به روش بیوشیمیایی، با استفاده از روش مولکولی PCR تایید شدند. الگوی مقاومت دارویی با استفاده از روش دیسک دیفیوژن انجام شد و حداقل غلظت مهارکنندگی (Minimum InhibitoryConcentration MIC:) آنتیبیوتیکهای ونکومایسین و تیکوپلانین به روش Microdilution Broth انجام گرفت.
یافتهها: ز 280 سویه انتروکوک، 175 سویه (5/62 درصد) به عنوان گونه انتروکوک فکالیس، 67 سویه (24 درصد) به عنوان گونه انتروکوک فاسیوم و 38 سویه (5/13 درصد) تا حد جنس انتروکوک شناسایی شدند. هیچگونه مقاومتی نسبت به آنتیبیوتیکهای نیتروفورانتوئین و لینه زولید در بین سویههای انتروکوک فکالیس مشاهده نشد و تمامی سویههای انتروکوک فاسیوم به آنتی بیوتیک لینه زولید حساس بودند. میزان مقاومت به آنتیبیوتیکهای آمپیسیلین، ونکومایسین، تیکوپلانین، اریترومایسین، سیپروفلوکساسین، نورفلوکساسین، جنتامایسین و نیتروفورانتوئین در سویههای انتروکوک فاسیوم بیشتر از سویههای انتروکوک فکالیس شناسایی شد.
استنتاج: ﺑﺎ ﺗﻮﺟﻪ ﺑﻪ ﻓﺮاواﻧﯽ ﺑﺎﻻی ﺳﻮﯾﻪﻫﺎی اﻧﺘﺮوﮐﻮک ﻣﻘﺎوم ﺑﻪ وﻧﮑﻮﻣﺎﯾﺴﯿﻦ و همچنین مقاومت در حال افزایش انتروکوکها نسبت به اکثر آنتیبیوتیکهای معمول، ﺿﺮورت ﮐﺎرﺑﺮد اﻗﺪاﻣﺎت ﮐﻨﺘﺮﻟﯽ و ﭘﯿﺸﮕﯿﺮاﻧﻪ وﺟﻮد دارد. ﺑﻪ ﻣﻨﻈﻮر ﺗﺠﻮﯾﺰ ﻣﻨﺎﺳﺐ آﻧﺘﯽبیوتیک، اﻧﺠﺎم آزﻣﻮن آﻧﺘﯽﺑﯿﻮﮔﺮام ﺑﺮای ﻫﺮ ﺑﯿﻤﺎر ﻗﺒﻞ از درﻣﺎن ﭘﯿﺸﻨﻬﺎد ﻣﯽﮔﺮدد.
Background and purpose: Multi-resistant enterococci are important nosocomial pathogens that are shown to have high prevalence in recent years. Knowledge of antimicrobial resistance pattern is essential to formulate treatment guidelines for infections caused by enterococci. The aim of this study was to investigate the antimicrobial resistance among enterococci isolated from Hamedan hospitals.
Material and methods: The study was carried out during 2012-2014 in 242 Enterococcus faecalis and Enterococcus faecium Strains isolated from clinical specimens of teaching inpatients and outpatients hospitals in Hamedan. Identified species by biochemical methods were confirmed by PCR. Antibiotic resistance was performed by disk diffusion. MIC (Minimum Inhibitory Concentration) of vancomycin and Teicoplanin were determined by Microdilution Broth method.
Results: A total of 280 enterococcal isolates were studied of which, 175 (62.5%) were identified as Enterococcus faecalis, 67(24%) as Enterococcus faecium, and 38(13.5%) as Enterococcus spp. No resistance to Linezolid and Nitrofurantoin was detected among E.faecalis isolates. All isolates of E. faecium were susceptible to Linezolid. Resistance rates to ampicillin, Vancomycin, Teicoplanin, ciprofloxacin, erythromycin, Norfloxacin, gentamicin and Nitrofurantoin were found higher in E.faecium isolates than those of the E.faecalis group.
Conclusion: Due to the high prevalence of Vancomycin-resistant enterococci and also increasing resistance of enterococcus to most common antibiotics, there is a need for preventive and control measurements. Therefore, antibiotic susceptibility testing is recommended for all patients before prescription of appropriate antibiotics.
