بررسی میزان اعتبار رضایت های اخذ شده از بیماران بستری در بیمارستان های آموزشی شهر ساری، سال 1387

<span style="font-weight: bold">سابقه و هدف:</span> کیفیت داده های ثبت شده در پرونده بیماران موضوع جدیدی نیست. کیفیت مصادیق متعددی دارد که در این مقاله از مصداق اعتبار داده در ارتباط با یکی از اساسی ترین وظایف بیمارستان ها قبل از ارایه درمان، که همان اخذ رضایت بیمار به عنوان یکی ارکان حقوق وی می باشد، پرداخته شد.<br><span style="font-weight: bold">مواد و روش ها:</span> این بررسی به صورت توصیفی و به صورت مرور پرونده در محدوده زمانی فروردین تا اسفند 1386 انجام شد. متغیرهای مورد بررسی شامل: سن، جنس، نوع مراجعه، وضعیت هوشیاری، حال عمومی بیمار در بدو ورود به بیمارستان، عنوان فرد رضایت دهنده (رضایت اخذ شده)، نسبت شاهد با بیمار، اعتبار رضایت، رضایت استفاده از اطلاعات پزشکی، اعتبار رضایت استفاده از اطلاعات پزشکی و میزان تحصیلات پذیرش کننده بود. با توجه به اینکه این مطالعه به صورت مقدماتی انجام شد، لذا از هر بیمارستان به ازای تعداد کارکنان پذیرش کننده، 30 پرونده استخراج شد. اعتبار رضایت نامه ها بر اساس مقایسه رضایت اخذ شده با استانداردها مورد بررسی قرار گرفت. جهت بررسی ارتباط بین متغیرها از آزمون آماری کای دو استفاده شد و سطح معناداری P<0.05 درنظر گرفته شد.<br><span style="font-weight: bold">یافته ها:</span> بر اساس یافته های این پژوهش مشخص شد که از 540 بیمار مورد بررسی 266 نفر (49.4 درصد) مرد و بقیه زن بودند. دامنه سنی از نوزاد1 روزه تا 89 سال سن متغیر بود. از نظر نوع مراجعه 258 نفر (47.8 درصد) به صورت اورژانس و بقیه الکتیو بودند. از منظر وضعیت هوشیاری، 477 نفر (88.5 درصد) هوشیار بودند و حال عمومی 442 نفر (82 درصد) خوب بود. از کل رضایت نامه های اخذ شده 403 مورد (74.6 درصد) در مقایسه با استانداردها اعتبار نداشتند. همچنین در بخش رضایت استفاده از اطلاعات پزشکی بیمار، فقط رضایت 125 مورد (23.1 درصد) اعتبار داشت.<br><span style="font-weight: bold">استنتاج:</span> با توجه به یافته های این پژوهش، اغلب رضایت های اخذ شده اعتبار نداشتند. بنابراین باید تدابیر لازم برای افزایش اعتبار به کار گرفته شود.

<span style="font-weight: bold">Background and purpose:</span> Quality of data in patient record is not a new issue. Data quality requires many details, which in this article we analyzed for validity of consent. This is a patients right and hospitals must pay attention to it, prior to providing their services.

<span style="font-weight: bold">Materials and methods: </span>This is a descriptive chart review research. Variables are age, sex, type of admission, degree of understanding, general condition at time of admission, and the person who signed the consent for chart, using medical information, validity of consent and education of admission clerk. This was a pilot study within each hospital, according to the number of clerks, thus, 30 records were reviewed. Validity of consent confirmed with comparison of consents with the standards. Data was analyzed with SPSS software, X2 statistics were used for determining the relation between variables and P<0/05 was significant.
<br><span style="font-weight: bold">Results: </span>Results showed that of the 540 records which were reviewed, 266 (49/4%) of the patients were males and remaining were females. Range of age was from 1 day to 89 years of age. Type of admission for 258 (47/8%) was emergency and the others were admitted electively. Degree of conscious for 477(88/5%) were awake and 442(82%) were in good general condition. The 403 (74/6%) consent for care were not valid and 125(23/1%) of the consent for using data of medical records in research was also not valid.
<br><span style="font-weight: bold">Conclusion: </span>The results of this research indicated that most consent is not reliable. Every effort must be made to ensure the development of consent validity.
