تاثیر اجرای برنامه اصلاح شده "آموزش جایگزین پرخاشگری" بر میزان هموگلوبین گلیکوزیله نوجوانان مبتلاء به دیابت تیپ یک
The Effect of Modified Aggression Replacement Training on Glycosylated Hemoglobin Levels in Adolescents with Insulin-dependent Diabetes

<p dir="RTL">سابقه و هدف: محدودیتهای فراوان نوجوانان دیابتی موجب بروز واکنشهای رفتاری منفی مانند پرخاشگری میشود که افزایش هموگلوبین گلیکوزیله آنان را در پی دارد. لذا بررسی مداخلاتی برای کاهش پرخاشگری و بهبود هموگلوبین گلیکوزیله این نوجوانان حایز اهمیت است. این مطالعه با هدف تعیین تاثیراجرای برنامه اصلاح شده آموزش جایگزین پرخاشگری (<span dir="LTR">ART</span>) بر میزان هموگلوبین گلیکوزیله نوجوانان مبتلا به دیابت وابسته به انسولین انجام گرفت.</p>
<p dir="RTL">مواد و روشها: طی یک کارآزمایی بالینی 70 نوجوان دیابتی وابسته به انسولین مراجعهکننده به مرکز دیابت پارسیان مشهد به صورت دردسترس انتخاب و به طور تصادفی به دو گروه مداخله (35 نفر) و کنترل (35 نفر) تقسم شدند. برنامه به مدت شش جلسه 5/1 تا دو ساعته در فواصل پنج روز از یکدیگر در گروههای 8 تا 10 نفره در محل دانشکده پرستاری و مامایی مشهد برگزار گردید. آزمایش هموگلوبین گلیکوزیله قبل از مداخله و دو ماه پس از اتمام مداخله انجام شد. دادهها با استفاده از نسخه 5/11 نرم افزارآماری <span dir="LTR">SPSS</span> و آزمونهای آماری تی مستقل و تی زوجی مورد تجزیه و تحلیل قرار گرفتند.</p>
<p dir="RTL">یافتهها: در این مطالعه میزان هموگلوبین گلیکوزیله نوجوانان مبتلا به دیابت قبل از مداخله در دو گروه تفاوت معنیداری نداشت (457/0=<span dir="LTR">p</span>)، امادر مرحله بعداز مداخله میزان هموگلوبین گلیکوزیله به طورمعنیداری در گروه مداخله (<span dir="LTR">8/7</span><span dir="LTR">±</span><span dir="LTR">1/4</span>) نسبت به گروه کنترل (<span dir="LTR">9/6</span><span dir="LTR">±</span><span dir="LTR">1/5</span>) کاهش داشت (017/0 ><span dir="LTR">p</span>).</p>
<p dir="RTL">استنتاج: برنامه اصلاح شده <span dir="LTR">ART</span> میتواند بر کاهش هموگلوبین گلیکوزیله نوجوانان دیابتی وابسته به انسولین اثربخش باشد. اجرای این برنامه به عنوان بخشی از درمان و مراقبت جامع دیابت توصیه میشود.</p>

<p dir="ltr"><strong>Background and purpose:</strong> Adolescents with diabetes are faced with some constraints and may show some negative reactions such as aggressive behavior. These behaviors increase the level of glycosylated hemoglobin in these patients. It is important to assess some interventions that reduce aggression and improve glycosylated hemoglobin level in diabetic adolescents. This study aimed at determining the effect of implementation of the modified aggression replacement training program (ART) on glycosylated hemoglobin levels in adolescents with insulin-dependent diabetes.</p>
<p dir="ltr"><strong>Materials and methods:</strong> A clinical trial was conducted in 70 adolescents suffering insulin-dependent diabetes attending Parsian Diabetes clinic in Mashhad, 2014. They were divided into two groups: a control (n= 35) and intervention (n= 35) group. The intervention program comprised five sessions of 1.5-2 hours. The sessions were held at five days intervals and each group consisted of 8-10 individuals. Glycosylated hemoglobin tests were performed before the intervention and two months later. Data was analyzed in SPSS by student t-test and Chi-square test.</p>
<p dir="ltr"><strong>Results:</strong> Before the intervention the glycosylated hemoglobin levels were not significantly different between the two groups (P=0.457). But in post-intervention phase, the glycosylated hemoglobin levels decreased significantly in intervention group (8.7±1.4) compared to the control group (9.6±1.5) (P<0.017).</p>
<p dir="ltr"><strong>Conclusion:</strong> The modified ART could be effective in reducing glycosylated hemoglobin levels in adolescents with insulin-dependent diabetes. Therefore, it is recommended as part of a comprehensive treatment for diabetes.</p>
