fa مقایسه دو دوز مختلف انفوزیون رمی ‌فنتانیل بر روی نمره آپگار نوزادان و بیداری حین بیهوشی عمومی در مادران تحت عمل سزارین انتخابی پژوهشي-کامل Research(Original) <div style="text-align: justify direction: rtl"> <span style="font-weight: bold">سابقه و هدف:</span> رمی ‌فنتانیل یک داروی مخدر کوتاه اثر است که علی ‌رغم عبور سریع از جفت، در بدن جنین نیز به سرعت متابولیزه می‌ گردد و اثرات سوء کمتری بر جنین اعمال می ‌کند. هدف از انجام این مطالعه بررسی مقایسه ‌ای دو دوز مختلف انفوزیون رمی فنتانیل بر روی آپگار اسکور نوزادان و بیداری مادران به هنگام بیهوشی عمومی در سزارین الکتیو می ‌باشد.<br><span style="font-weight: bold">مواد و روش ها:</span> این کارآزمایی بالینی دوسوکور بر روی 90 مادر 18 تا 35 ساله کاندید سزارین انتخابی انجام شد. مادران با روش تخصیص تصادفی به 3 گروه مساوی شامل انفوزیون رمی فنتانیل با دوز 0.2 mg/kg/min (گروه مورد)، انفوزیون نرمال سالین با همان حجم و سرعت (گروه شاهد) و انفوزیون رمی فنتانیل با دوز 0.1 mg/kg/min (گروه مورد) تقسیم شدند. نمره آپگار نوزادان در دقایق 1 و 5 پس از تولد ثبت شد. فشارخون و ضربان قلب مادران، قبل از شروع القا بیهوشی و بلافاصله قبل و بعد از لوله گذاری تراشه و 5 دقیقه بعد از آن اندازه ‌گیری و ثبت شد. داده‌ ها توسط آزمون ‌هایT-test ،Anova وRepeated measures مورد ارزیابی قرار گرفت.<br><span style="font-weight: bold">یافته ها: </span>میانگین فشار سیستولیک و دیاستولیک و ضربان قلب مادر قبل از القا، یک دقیقه پس از لوله‌ گذاری، 5 دقیقه پس از لوله ‌گذاری و نیز پس از خروج جنین در گروه‌ های مورد کمتر از گروه شاهد بوده است و از لحاظ آماری تفاوت معنی ‌داری داشت (p<0.05). میانگین آپگار نوزادان در دقایق اول و پنجم در سه گروه از لحاظ آماری تفاوتی معنی ‌داری نداشت (p<0.05).<br><span style="font-weight: bold">استنتاج:</span> به نظر می ‌رسد که داروی رمی فنتانیل دارای اثرات کنترل کننده و محافظتی بر تغییرات نوسانی فشارخون و ضربان قلب مادر بوده و اثرات سوء آشکاری بر مادر و جنین اعمال نمی ‌کند. این یافته ‌ها می ‌تواند تاییدی بر استفاده گسترده ‌تر از این داروی مخدر سیستمیک در کمک به بیهوشی در عمل سزارین باشد. </div> <span style="font-weight: bold">Background and purpose:</span> Remifentanil is a short-acting opiod that is metabolized rapidly in the fetus despite its rapid passage through the placenta. Therefore, it induces less adverse effects on the fetus. The aim of this study was to compare the effect of two different doses of remifentanil infusion on Âpgar score of neonates and mothers' awareness during general anesthesia in elective Çesarean section. <br><span style="font-weight: bold">Materials and methods:</span> This double-blind clinical trial was performed on 90 participants aged 18-35 years who were candidates for elective Çesarean section. They were randomly allocated into three equal groups as follows: the first case group received a remifentanil infusion of 0.2 µg/kg/min, the control group was infused with the same volume and rate of normal saline, finally, the second case group received a remifentanil infusion of 0.1 µg/kg/min. Âpgar scores of neonates were recorded in the 1st and 5th minutes after birth. Systolic and diastolic blood pressure and heart rates of mothers were recorded before induction, just before and after tracheal intubation and 5 min after that. Data were analyzed using SPSS17 through t-test, ÂNÔVÂ test and Repeated measures. <br><span style="font-weight: bold">Results:</span> Ïn the case groups, systolic and diastolic blood pressure and heart rates of mothers before induction, one minute and five minutes after intubation and after fetus delivery were less than that of the control group and the differences among the groups were statistically significant (P<0.05). However, there were no significant differences between the mean of Âpgar scores of the neonates one minute and five minutes after birth (P>0.05). <br><span style="font-weight: bold">Çonclusion: </span>Ït seems that remifentanil has supportive and controlling effects on oscillatory changes in blood pressure and heart rate of mothers and also it does not have any significant adverse effects on the mother and the fetus.
