بررسی رشد کودکان از بدو تولد تا دو سالگی در مناطق روستایی شهرستان بابل در سال 1377

<span style="font-weight: bold">سابقه و هدف: </span>ارزیابی پایش رشد کودکان یکی ازمهمترین منابع اطلاعاتی در تشخیص اختلال رشد و شروع سو تغذیه در کودکان است. لذا هدف از این مطالعه تعیین روند رشد کودکان از بدو تولد تا 2 سالگی در مناطق روستایی شهرستان بابل و مقایسه آن با منحنی استاندارد NCHS می باشد.<br><span style="font-weight: bold">مواد و روش ها:</span> این بررسی بر اساس اطلاعات موجود (Existing data) در سیستم پایش رشد کودکان تحت پوشش خانه های بهداشت شهرستان بابل درسال 1377 انجام گرفته است. نمونه گیری به روش خوشه ای بوده است که 429 کودک از میان متولدین 73 تا 75 از طریق دفتر ثبت موالید انتخاب شده اند. صدک های قد و وزن بر حسب سن و جنس محاسبه و با استاندارد NCHS مقایسه گردید.<br><span style="font-weight: bold">یافته ها:</span> نتایج پایش رشد وزنی کودکان مورد مطالعه در مقایسه با صدک های استاندارد نشان می دهد که وزن 5/67 درصد کودکان در یک ماهگی بین صدک پنجاهم تا نود و هفتم استاندارد بوده است. با افزایش سن این رقم کاهش یافته است، به طوری که در 6 ماهگی 2/45 درصد، در 12 ماهگی 6/42 درصد و در 24 ماهگی 2/44 درصد کودکان وزن شان بین صدک پنجاهم و نود و هفتم استاندارد بوده است. وقتی که ارقام فوق را در دو جنس مقایسه می کنیم، تقریبا برای دختران 5 تا 20 درصد کمتر از پسران است. این نتایج نشان می دهد با افزایش سن، کودکانی که وزن آنها بالای صدک پنجاهم استاندارد بوده است به پایین تر از صدک پنجاهم استاندارد افت کرده است. نتایج همچنین نشان می دهد که صدک پنجاهم وزن پسران تا 12 ماهگی و برای دختران تا 6 ماهگی بالاتر از صدک مشابه در NCHS است، ولی در سنین بعد تا 2 سالگی زیر صدک پنجاهم استاندارد قرار می گیرد. همچنین اختلال در رشد قدی در سنین پایین تر 1 تا 3 ماهگی آغاز می شود، به طوری که در یک ماهگی صدک پنجاهم قد نزدیک به استاندارد است ولی از 3 ماهگی به بعد از صدک پنجاهم NCHS انحراف پیدا می کند. به طورکلی در صدک های وزن و قد کودکان جامعه پژوهش در مقایسه با صدک های NCHS اختلاف معنی داری مشاهده گردیده است (P<0.05).<br><span style="font-weight: bold">استنتاج:</span> نتایج دلالت می کند که قضاوت در مورد رشد شیرخوران ایرانی با استفاده از استانداردهای غیرایرانی صحیح نمی باشد. بنابراین تهیه منحنی های رشد کشوری در ایران ضروری به نظر می رسد.

<span style="font-weight: bold">Background and purpose:</span> The evaluation of growth monitoring in children is one of the important sources of information to detect malnutrition. The objective of this study was to determine the trend of infant growth from birth to two years in rural communities of Babol and to compare it with NÇHS standard.
<br><span style="font-weight: bold">Materials and methods:</span> This study was based on the existing data in growth monitoring system available in Babol communities under coverage of health centers. 429 registered births between 1996 to 1998 were selected using cluster sampling technique. Percentiles of height and weight were calculated and compared with NÇHS standard.

<span style="font-weight: bold">Results: </span>Results show that, the weight in 67.5% of infants was between 50th and 97th percentile of NÇHS standard at first month. This figure declined by increasing the age in such a way that, in the weight percentile was between 50th and 97th NÇHS 45.2% at 6 months, 42.6% at 12 months and 44.2% at 24 months.
When these figures were compared between boys and girls, 5% to 20% were less in girls than the boys. The results also show that the 50th percentile of weight for boys up to 12 months and for girls up to 6 months is higher than 50th percentiles of NÇHS standard, but then after, up to the age of 2 years becomes lower than 50th percentile of standard.
Âlso height growth disorder starts at the age under 1 to 3 months, in such a way that at first month, the 50th percentile of height is close to NÇHS, but after 3 months deviates from NÇHS. Â significant difference was observed between 50th percentile of height and weight of the cases under our study and NÇHS (P<0.05).

<span style="font-weight: bold">Çonclusion:</span> The results indicate that Ïranian infants growth can not be evaluated with NÇHS standard. Hence it is necessary to provide national and local growth curves for Ïranian children.
