fa آیا مصرف بیس فسفونات ها کنترااندیکاسیون درمان های دندانپزشکی است؟ Is using Bisphosphonate a Contraindication for Dental Treatments? دندانپزشکی Dentistry مروری Review بیس فسفونات‌ها دسته‌ایی از داروها هستند که برای درمان بیماری‌های خاص استخوانی مانند مالتیپل میلوما و سرطان‌های متاستاتیک استخوانی استفاده می‌شوند. اگرچه مکانیسم عمل دقیق بیس فسفونات‌ها شناخته نشده، ولی نقش این داروها در سرکوب رگ‌زایی و ریمادلینگ استخوانی ثابت شده است. استئونکروزفکین (BRONJ) عارضه‌ی جانبی در بیمارانی است که بیس فسفونات دریافت می‌کنند، این عارضه به ندرت رخ می‌دهد ولی در صورت وقوع، مشکلات جدی برای بیماران به وجود می‌آورد و حتی می‌تواند منجر به شکستگی‌های پاتولوژیک در مندیبل شود. شیوعBRONJ در بیمارانی که بیس فسفونات داخل وریدی مانند زالندرونات و پامیدرونات مصرف می‌کنند، به‌صورت چشم‌گیری از بیمارانی که بیس فسفونات خوراکی مصرف می‌کنند، بیش‌تر است. احتمال وقوع BRONJ در مندیبل 2 برابر ماگزیلاست و 60-70 درصد بیماران، به دنبال جراحی‌های دندانی به BRONJ مبتلا می‌شوند. علائم و نشانه‌ها ممکن است قبل از وقوع استئونکروز مشاهده شوند که شامل درد، لقی دندان‌ها و زخم‌های مخاطی است. ویژگی‌های تصویربرداری در BRONJ اختصاصی نیست. رادیوگرافی‌های پری‌اپیکال و پانورامیک شاخص‌های غربال‌گری اولیه هستند، درحالی‌که CT و MRI اطلاعات کامل‌تری در اختیار ما می‌گذارند. درمان‌های غیر جراحی شامل داروهای ضد درد، آنتی بیوتیک‌ها و دهان‌شویه‌های ضد میکروبی و درمان‌های جراحی شامل دبریدمان و ریسکشن است که در مراحل پیشرفته‌تر توصیه شده است. Bisphosphanates include a group of medicines used in treatment of specific bone diseases, such as multiple myeloma and osteo- high metastatic cancers. Though the exact mechanism of such medicines has not yet been known, the role these medicines play in the suppression of innervations and bone remodeling has been proved. Bisphosphonate related osteonecrosis of jaws (BRONJ) is a complication caused by using Bisphosphanates. It is a rare infliction, but, when it occurs, it brings about grave consequences for the patients including pathological fractures in the mandible. The BORNJ prevalence in patients who take intravenous Bisphosphates (BPs), such as Zolendronate, and Pamidronate is significantly higher than that in patients taking such medicines orally. The probability of BRONJ onset in mandible is twice higher than in maxilla; and some 60-70% of the patients are reported to be inflicted with BRONJ following dental surgeries. The signs and symptoms may occur prior to the presence of osteonecrosis, which include pain, loose teeth, and membranous lesions. The imaging features in BRONJ are non-specific. Peri-apical and panoramic imaging are the primary screening indices, while CT scanning and MRI provide us with more accurate information.
