fa مقایسه اکسترنال فیکساتور با اینتـرامدولاری نیلینگ در درمان شکستگی‌های باز تیبیا پژوهشي-کامل Research(Original) <div style="text-align: justify direction: rtl"> <span style="font-weight: bold">سابقه و هدف : </span>در حال حاضر درمان استاندارد جهت شکستگی‌های باز تیبیا، اکسترنال فیکساتور می باشد اما با توجه به مدت طولانی بستری شدن و برخی عوارض همراه، برخی از محققین روش مدولاری نیلینگ را به عنوان درمان جراحی جایگزین پیشنهاد کرده‌اند. هدف از این مطالعه مقایسۀ دو روش اکسترنال فیکساتور و اینتـرامدولاری نیلینگ در درمان شکستگی‌های بازتیبیا بوده است. <br><span style="font-weight: bold">مواد و روش ها : </span>این مطالعه به روش کارآزمایی بالینی شاهددار تصادفی، بر روی 113 بیمار که به علت شکستگی باز تیبیای نوع Ï و ÏÏ طبقه‌بندی گاستیلو در طی یک دوره دو ساله انجام شد. 54 بیمار با روش اینترامدولاری نیلینگ و 59 بیمار با روش اکسترنال فیکساتور جراحی شدند. بیماران دو گروه از نظر مدت زمان جوش خوردن و عوارضی چون تأخیر در جوش خوردن، جوش نخوردن و عفونت با هم مقایسه شدند. <br> <span style="font-weight: bold">یافته ها :</span> در این مطالعه 113 بیمار که 95 بیمار مذکر و 18 نفر مونث بودند وارد مطالعه گردیدند. سن بیماران 18 الی 50 سال (متوسط 3/34 سال) بود. از 59 بیمار که با اکسترنال فیکساتور عمل شدند، 8 بیمار دچار تاخیر در جوش خوردن شدند، که از این تعداد، 3 بیمار عارضه جوش نخوردن را تجربه کردند که تا 6 ماه بعد از عمل هیچ‌گونه پیشرفتی در ساخت کالوس نداشتند. از 54 بیمار که با مدولاری نیلینگ درمان شدند، 2 بیمار دچار تاخیر در جوش خوردن و 1 بیمار دچار جوش نخوردن شدند. وضعیت جوش نخوردن و تاخیر در جوش خوردن در دو روش اختلاف معنی‌داری نداشت (05/0p>). <br><span style="font-weight: bold">استنتاج :</span> با توجه به کاهش مدت بستری در روش نیل مدولاری و انجام جراحی مجدد و کاربرد نیل مدولاری در بعضی از بیماران که به روش اکسترنال فیکساتور به خوبی درمان نمی‌شوند توصیه می‌شود از همان ابتدا نیل مدولاری انجام شود. </div> <span style="font-weight: bold">Background and purpose:</span> Ëxternal fixation is currently considered as a standard treatment for open tibial fractures however, due to the long-term hospitalization and some other complications, some researchers have proposed intramedullary nailing as the alternative surgical treatment. This study was conducted to compare the treatment of open tibial shaft fractures by external fixation and intramedullary nailing. <br><span style="font-weight: bold">Materials and methods:</span> This randomized controlled clinical trial was conducted on 113 patients with open tibial fractures type Ï and ÏÏ (Gustilo classification) during a two year period. These patients were divided in two groups and 59 cases were operated using external fixation and 54 fractures were fixed with intramedullary nailing. The patients were compared for complications such as :::union::: time, delay in the :::union:::, non:::union::: and infection. <br> <span style="font-weight: bold">Results:</span> Ône hundred and thirteen patients (95 males and 18 females) with the age range of 18-50 years (average age: 34.3) were enrolled in this study. Âmong the 59 patients treated with external fixator, 8 had delayed :::union:::. Three of these eight patients suffered from the non:::union::: and did not have any progress in callus formation till 6 months after the treatment. Ôn the other hand, among the patients (54 cases) treated with intramedullary nailing, 2 cases suffered from the delayed :::union::: and one had non:::union:::.
