fa سیستم اطلاعات ایمنی بیمار: اهداف، ساختار و وظایف پژوهشي-کامل Research(Original) <div style="text-align: justify direction: rtl"> ایمنی بیمار یکی از ارکان اصلی کیفیت مراقبت بهداشتی است. بهبود ایمنی بیمار مستلزم شناسایی وقایع تهدیدکننده ایمنی بیمار، تحلیل روند این وقایع و تدوین راه‌حل‌های اصلاحی برای بهبود سیستم می‌باشد. سازمان‌های مراقبت‌های بهداشتی بدون داده‌ها و اطلاعات ایمنی بیمار نمی‌توانند در مورد ایمنی مراقبت‌های ارایه شده قضاوت کنند. به‌ همین دلیل از سیستم اطلاعات ایمنی بیمار به‌ منظور شناسایی و ثبت وقایع تهدیدکننده ایمنی بیمار، تعیین و تحلیل علل این وقایع، فرضیه‌سازی در مورد روش‌های بهبود و در نهایت انجام و پایش برنامه‌های بهبود استفاده می‌شود. با توجه به اهمیت شناخت اجزاء مختلف سیستم اطلاعات ایمنی بیمار، این مطالعه با هدف تبیین اجزاء مختلف این سیستم انجام گردیده است. در این مطالعه مروری، اطلاعات مورد نیاز برای شناسایی و تبیین اجزای مختلف سیستم اطلاعات ایمنی بیمار از روش جستجوی منابع در پایگاه‌های اطلاعاتی Web of Science ، (Medline) Pubmed ، Ôvid ، Science Direct و Google Scholar به‌دست آمد. در سیستم اطلاعات ایمنی بیمار، انواع داده‌های قابل گردآوری،‌ خطاها و وقایع تهدیدکننده تحت پوشش، روش گردآوری داده و افراد مشارکت‌کننده در گردآوری داده‌ بسیار متنوع بود. علاوه بر این، روش‌های مشخصی (از جمله شاخص‌های ایمنی بیمار) برای تحلیل و تفسیر داده‌ها تعریف شده‌اند که این تحلیل‌ها باید توسط متخصصین موضوعی انجام شده و در نهایت، خروجی سیستم (گزارش‌های آماری، توصیه‌ها، هشدارها و برنامه‌های اصلاحی) به کاربران آن (از جمله ارایه‌کنندگان مراقبت، مدیران و کاربران خارجی) اطلاع‌رسانی شود تا در امور مختلف به‌ خصوص اصلاح فرایندها و یادگیری از خطاها مورد استفاده قرار گیرند. بنابراین، برای طراحی چنین سیستمی در ایران لازم است چارچوب مناسبی با لحاظ کردن این موارد طراحی گردد. </div> Patient safety is the key element of quality in healthcare. Ïmproving patient safety involves identifying the incidents, analyzing the trend of events and developing corrective solutions for promoting the system. Health care organizations can not judge the safety of the care without data and information related to patient safety. Therefore, patient safety information system (PSÏS( is used in identifying and recording patient safety incidents, determining and analyzing their causes, hypothesizing corrective solutions, and finally, implicating and monitoring the improvements. Âccording to the importance of this system, this study was conducted to explain the different components of the PSÏS. Ïn this review study, we reviewed the materials searched via Web of Science, Pubmed (Medline), Ôvid, Science Direct and Google Scholar. There were varieties of data collection methods, the incidents, and participants involved in data collection in patient safety information system. Moreover, some specific methods such as patient safety indicators were defined for data analysis. These analyses should be done by the experts in the field and the output of the PSÏS including statistical reports, alerts, recommendations and corrective actions should be provided for internal or external users such as healthcare executives and healthcare providers so that they can use them for improving the processes and learning from the incidents.
