fa مقایسه زمان پروترومبین در نمونه های خون وریدی لوله اول و دوم پژوهشي-کامل Research(Original) <div style="text-align: justify direction: rtl"> <span style="font-weight: bold">سابقه و هدف : </span>روش استاندارد جهانی برای اندازه گیری زمان پروترومبین (Prothrombin Time: P.T.)‌ استفاده از لوله دوم خون وریدی در هنگام انجام تست بوده است. علت عدم استفاده از لوله اول تأثیرات نامطلوب حاصله از آزاد شدن ترومبوپالستین بافتی از رگ آسیب دیده ناشی از استفاده از سوزن های چند بار مصرف در گذشته بود. جالب آنکه علی رغم پیدایش روش های مدرن خون گیری تجربه، باز هم با نتایج نامطلوب همراه بوده است. مطالعه جدید در آمریکا با کمک روش های خون گیری مدرن و خون گیر ماهر، کفایت استفاده از همان لوله اول را نشان داده است. ما در این تحقیق ضمن استفاده از روشی کماکان استاندارد در خون گیری، زمان پروترومبین لوله های اول و دوم را نزد دو گروه بیماران با و بدون مصرف وارفارین مورد مقایسه قرار دادیم. <br><span style="font-weight: bold">مواد و روش ها :</span> از دو گروه مساوی پنجاه نفری بیماران با و بدون مصرف وارفارین، دو نمونه خون وریدی متوالی توسط خون گیر ماهر و با کمک سرنگ بال پروانه گرفته شده و تست P.T.‌ برای هر لوله با استفاده از دستگاه کواگولومتر انجام شد. <br> <span style="font-weight: bold">نتایج :</span> در گروه اول به تعداد 50 نفر و تحت درمان با وارفارین متوسط P.T. لوله او 93/18 ثانیه و متوسط P.T.‌ در لوله دوم 26/19 ثانیه (متوسط اختلاف 33/0 ثانیه و 46/0= t‌ ) بود. در گروه دوم که شامل 50 بیمار و بدون درمان با وارفارین بود، متوسط P.T.‌ لوله او 78/12 ثانیه و متوسط P.T. لوله دوم 13 ثانیه (متوسط اختلاف 22/0 ثانیه و 77/0= t ) بدست آمد. <br><span style="font-weight: bold">استنتاج :</span> در هر دو گروه اختلاف بین لوله اول و دوم از دیدگاه آماری و بالینی قابل اغماض بوده و برای انجام تست P.T. با استفاده از خون گیر ماهر می توان از همان لوله اول خون وریدی استفاده کرد. </div> <span style="font-weight: bold">Background and purpose :</span> In The current practice to measure prothrombin time(P.T.). the second tube of venous blood is used for testing. This practice derived from early experience using first tube of venous blood and non-disposable needles which inevitably resulting to release of tissue thromboplastin from traumatized blood vessel wall, and so the estimated time was falsely shortened. Using modern phlebotomy methods by inexperienced phlebotomists were resulted in unfavorable outcome, too. According to a new research , using evacuated tubes by experienced phlebotomists yields accurate results by the first tube. This study designed ti use other standard phlebotomy instruments and to compare P.T. of the first and second tubes within two groups of patients those used and those not used warfarin. <br><span style="font-weight: bold">Materials and Methods :</span> paired blood specimens were collected from two equal groups of patients a groups of fifty patients (Group A) were taking warfarin and another equal group (Group B) were not. Tests were performed on the first and second tubes. <br><span style="font-weight: bold">Results </span>: In Group A mean P.T. of the first and second tubes were 18.93 sec and 19.26 sec , respectively. In Group B mean P.T. of the first tubes was 12.78 sec and that of the second tubes was 13 sec. <br><span style="font-weight: bold">Conclusion :</span> Tte different between P.T. of the first and second tubes was not statistically sinnificant.
