(041/0(OR=1.67, 95% CI= 1.01-2.73 p=.

<CONTENT>Background and purpose: Endothelial-derived nitric oxide (NO) is a major contributor in regulating myocardial function and has been implicated in development of coronary artery disease (CAD) as the most common type of cardiovascular disease. It is synthesized in the human body from L-arginine by constitutive endothelial NOS (eNOS) as one out of three known isoforms of NO synthase (NOS) involved in this process. Based on recent studies, single nucleotide polymorphisms (SNPs) of some genes, such as eNOS, appear to be genetic risk factors for CAD however, the results are inconsistent across different ethnic populations. Therefore, the current study was carried out to assess the association between -786T/C polymorphism (rs2070744) and susceptibility to CAD risk in Iranian population.
Materials and methods: The study included 50 patients with angiographically confirmed CAD (≥50% stenosis in at least one coronary vessel) and 100 aged-matched individuals without cardiovascular background, hypertension, diabetes, or any other specific illness. The genomic DNA was extracted from peripheral blood cells and genotyping was performed using amplification refractory mutation system-polymerase chain reaction (ARMS-PCR).
Results: The frequency of risk allele (C) was significantly higher in case group compared to control group (P= 0.04). According to the findings, the presence of the C allele was associated with 1.67-fold increased risk of CAD compared with individulas lacking the risk allele (95% CI= 1.01-2.73, P= 0.04).
Conclusion: Several studies revealed association of -T786C polymorphism with coronary spasm, myocardial infarction, hypertension, and multivessel CAD in different populations. In current study, a possible association was found between -T786C polymorphism and risk of CAD, which is in line with previous findings. These results suggest this variation as a predictor marker for estimating the risk of CAD in Iranian population.</CONTENT>

<CONTENT>Background and purpose: Myrtus communis L. (Myrtle, Myrtaceae) is a known medicinal plant used in traditional medicine worldwide. It is a well-known plant with distinctive antioxidant activity. In this study we investigated the impact of extraction methods on total phenolic and flavonoids contents and antioxidant activities of Myrtle (Myrtus communis L.) leaf.
Materials and methods: The shade-dried leaf was extracted by three different methods including meceration, ultrasonic assisted, and soxhlet assisted extraction. Antioxidant capacity was assessed using four different methods: DPPH and nitric oxide (NO) free radicals scavenging, reducing power and iron chelating activity. The total phenolic and flavonoid contents were also identified. The data were analyzed by analysis of variance and the means separated by Newman-Keuls Multiple Comparison test (GraphPad Prism 5).
Results: The highest yield of extraction was achieved with soxhlet assisted extraction. It also showed highest amount of total phenolics and flavonoids contents. In DPPH radical scavenging activity, the soxhlet extract (IC50 = 11.3 ± 0.3 µg ml) had a higher activity which was significantly different from other extracts (P&#60; 0.05). All extracts were stronger than BHA (P&#60;0.001). This extract had higher iron chelating activity, too. It was significantly more potent than ultrasonic (P&#60;0.01) and maceration extracts (P&#60;0.001). But EDTA was so stronger than other extracts (P&#60;0.001). In reducing power assay, maceration extract showed the highest activity (P&#60;0.001). In nitric oxide radical scavenging activity, IC50 for ultrasonic extract, soxhlet assisted extraction and maceration extract were 355.1, 402.9, and 173.3 µg/ml, respectively. Here, the maceration extract showed the highest activity, too (P&#60;0.001).
Conclusion: The results clearly indicate that extraction methods used in this study significantly affected antioxidant capacities and total phenolic and flavonoids contents. Soxhlet assisted extraction and maceration methods were found to be more efficient in extraction of antioxidant components of Myrtle.</CONTENT>

Background and purpose: Methotrexate as a chemotherapy drug causes chronic liver damage, infiltration of neutrophils, oxidative stress, and direct renal tubular damage. Quercetin is a flavonoid with antioxidant and anti-inflammatory properties. Therefore, the aim of this study was to investigate the effect of quercetin on eliminating the liver and kidney toxicity of methotrexate.
Materials and methods: In this experimental study, 32 rats were divided into 4 groups. Group I (control) was given regular diet. Group II received single-dose methotrexate. Group III received methotrexate + a single dose quercetin and the last group (positive control) received methotrexate + a single dose silymarin. After five days, blood samples were taken and the serum GOT, GPT, ALP, Cr, urea and antioxidant capacity of plasma were measured. Some parts of liver and kidney were removed to measure the liver and kidney SOD, MDA, catalase activity and histopathological studies.
Results: Serum GOT, GPT, ALP, Cr, and liver and kidney MDA were significantly higher (P&#60;0.05) in group II, compared with those of the control group. These parameters significantly decreased (P&#60;0.05) in group III. Compared to the control group, antioxidant capacity of plasma, activity of the liver and kidney SOD, catalase and serum urea decreased significantly in group II (P&#60;0.05). Administration of quercetin significantly increased these parameters (P&#60;0.05) and decreased hepatic and renal lymphocyte infiltration.
Conclusion: According to the results, administration of quercetin could have a protective role in preventing liver and renal toxicity induced by methotrexate which could be due to its antioxidant property.</CONTENT>

Background and purpose: Safety control in edible oil industry regarding its pollution to heavy metals is one of the most important factors in maintaining the health of people and assuring their safety. This research aimed at evaluating the effect of production process of sunflower oil on its lead and arsenic content.
Materials and methods: All sunflower samples were collected from a local factory during various stages of production process including raw oil, neutralization, de-coloring, de-odoring, winterizing and packaging. Sample preparation was performed by closed-vessel microwave extraction and lead and arsenic were analyzed by graphite furnace and vapor hydride atomic adsorption spectrometry, respectively.
Results: Results: The mean concentration of lead and arsenic in sunflower oil in various stages of production process were 2.9±0.2 and 0.5±0.1 for raw oil, &#60;0.1 and 0.2±0.1 for neutralized oil, 2.0±0.7 and 0.8±0.1 for de-coloring oil, 0.2±0.1 and &#60;0.1 for de-odoring oil, 1.3±0.1 and 0.9±0.1 for winterized oil, and 5.1±0.3 and 1.6±0.3 µg/Kg for packaged oil, respectively. Neutralizing the oil and its de-odoring reduced the concentration of lead and arseneic, while de-coloring and winterizing the oil increased the concentration of these metals in oil.
Conclusion: It seems that heavy metal impurities in Bentonite soil and poor coat conditions of oil reservoirs were the major factors in increasing the concentration of lead and arsenic in de-coloring and packaging phases. Also, saturated glyceride and waxes which are coming out in winterizing phase has a major contribution in polluting the oil by heavy metals.</CONTENT>

Background and purpose: Nitrate is one of the chemical water contaminants which can be eliminated through physical, chemical and biological techniques. In this study we aimed for optimization of nitrate adsorption from water onto Magnetic Graphene sheet Nano Particles (G-Fe3O4 MNPs) via Taguchi experimental design.
Materials and methods: Batch reactor was used for optimizing of the five parameters (pH, contact time, temperature, adsorbent dosage, and nitrate concentration) at four levels by Minitab software, based on Taguchi experimental design. Signal to noise ratio was used with ‘the highest is better’ approach for optimization of experimental conditions and the highest nitrate removal efficiency.
Results: The results revealed that in optimized status (pH=3, contact time= 60 min, initial concentration= 50 mg/L, adsorbent dose= 2g/L, temperature= 50oC) nitrate removal efficiency and adsorption capacity reached 86.4% and 39.37 mg/g, respectively. Contribution percentage of each variable implied that pH and initial concentration of nitrate with 40.20% and 7.49% had the highest and lowest influences on nitrate removal, respectively. Isotherm and kinetic studies illustrated that, nitrate adsorption complied with Langmuir isotherm model (R2&#62;0.993) and pseudo second-order kinetics models (R2&#62;0.94).
Conclusion: G-Fe3O4 MNPs can be used as an effective sorbent for nitrate removal from water or wastewater due to several advantages including easy and rapid separation from solution and high removal efficiency.</CONTENT>

<CONTENT>Background and purpose: Citrobacter species can cause opportunistic hospital-acquired infections and their treatment has become problematic following the acquisition of extended spectrum beta- lactamases (ESBLs). The purpose of this study was to determine the frequency of ESBL genes in clinical isolates in Kermanshah Imam Reza Hospital.
Materials and methods: In this study, 70 Citrobacter isolates were collected from various clinical specimens and confirmed by standard bacteriological tests and API20E Kit. After antibiotic susceptibility testing using disk diffusion method, the presence of ESBL phenotype was determined by combined disk test. Specific primers were used to detect blaCTX-M, blaTEM and blaSHV genes among isolates using PCR. Data was analyzed in SPSS V.18 applying Chi-Square test and Fisher's exact test.
Results: From 70 isolates of Citrobacter, 5 (7.1%) were phenotypically positive for ESBL. The PCR test determined 19 isolates (31.6%) genetically positive for ESBL, and the frequency of blaCTX-M, blaTEM and blaSHV genes were 21.3%, 11.4% and 1.4%, respectively. The highest antibiotic resistance of isolates was to cefazolin (84.8%), ampicillin (71.2%) and co-trimoxazole (36.4%), and the lowest was to gentamicin (9.1%), piperacillin/tazobactam (4.5%), and imipenem (1.5%).
Conclusion: The results showed that Citrobacter freundiii was the dominant species for hospital-acquired infections in Kermanshah. A high percentage of isolates contained ESBL genes and blaCTX-M was the most common. There was a high gap between the frequencies of genotypic and phenotypic for ESBL, which may indicate ESB genes are not highly expressed in Citrobacter isolates.</CONTENT>

Background and purpose: Gestational diabetes mellitus (GDM) is glucose intolerance that begins or is diagnosed during pregnancy. Given the importance of early detection and timely treatment of the disease, this study aimed to determine the prevalence of GDM in different age groups in city of Razan, Hamadan province (Iran).
Materials and methods: In a cross-sectional study (according to the screening program for GDM conducted in all health centers in the country) 493 pregnant women attending Razan central laboratory for gestational diabetes screening (in 2014) were recruited. The participants did not have any acute or chronic diseases. Glucose tolerance test (GTT) was done and expectant mothers were considered to have gestational diabetes if their blood glucose concentration was higher than reference range at least once. Data was analyzed using descriptive statistics and Student t-test in SPSS V. 20.
Results: Among the participants 60 (12.2%) were diagnosed with gestational diabetes. The lowest and highest incidences of GDM were found in women aged 25-29 years old (7.1%) and 40-44 years old (33.3%), respectively. These results indicated higher incidence of gestational diabetes in older women.
Conclusion: The prevalence of GDM in studied region is similar to the reports from other regions in Iran. Therefore, it is necessary to inform pregnant women about GDM and its complications. Also, further studies concerning the screening program for GDM are required, to reduce its incidence and complications.</CONTENT>

<CONTENT>Background and purpose: Polycystic ovary syndrome (PCOS) is one of the most common causes of infertility in modern societies. The etiology of PCOS is unknown, so a good animal model can be valuable in studying its pathogenesis and could help in making a more effective diagnosis and choosing appropriate treatments. All PCOS disorders may be the result of multiple genetic and environmental factors. One of these environmental factors is reactive oxygen species (ROS). The aim of this study was to make oxidative stress by induction of polycystic ovary model.
Materials and methods: PCOS was induced in 30 NMRI mature female mice during 8 weeks which were divided into a control group and an experimental group. PCOS was induced by single injection of estradiol valerate (40 mg/kg). After PCOS induction, body weight was measured weekly for 8 weeks. Mice in each group were scarified and their ovarian tissues were collected. Histopathological study was done to confirm the model. Oxidative stress and antioxidant capacity levels were measured in ovarian tissue using Flow cytometric and Fluorometric techniques. Data was analyzed by independent sample T-test.
Results: Results: Both groups showed significant increase in weight at the end of the study (P≤0.05). Histopathological studies confirmed PCO model. Flow cytometric analysis revealed that oxidative stress significantly increased in ovarian tissue in PCO group, (P≤0.05). Also, in TAC study of ovarian tissue no significant difference was observed between the two groups.
Conclusion: According to this study, PCO can increase oxidative stress in ovarian tissue. This increase could make more cysts in mouse ovarian tissue.</CONTENT>

<CONTENT>Background and purpose: Family is among the most important foundations in a society which forms the personality of individuals. Satisfaction, happiness and quality of life provided by the family will influence the growth and life skills of its individuals. Family health promoting lifestyle can immensely affect children’s habits. The aim of this study was to evaluate the relationship between family health promoting lifestyle and sleeping habits among children (6 to 11 years of age) in Qazvin, Iran 2014.
Materials and methods: In this analytical cross-sectional study we included 278 families and their 6-11-year old children (year 1 to 6 in state and private primary schools) who were selected through random cluster sampling. Data was collected using Health-Promoting Lifestyle Profile [HPLP II] and the Children's Sleep Habits Questionnaire (CSHQ). Pearson Correlation Coefficient was applied to analyze the data.
Results: The participants had an overall HPLP-II score of 125.48. Significant relationships were found between children wake up time and family’s responsibility (sig = 0.00, r = 0.12), family’s spiritual growth (sig= 0.00, r=0.14) and the total HPLP-II score (sig=0.00, r=0.18). There was also significant associations between children´s sleeping habit and the relationships in the family (sig=0.00, r=0.19).
Conclusion: The families were found to have average standards in health promoting lifestyle which led to having children with relatively good sleeping habits. Therefore, improving the health promoting lifestyle in a family can modify children sleeping habits.</CONTENT>

Background and purpose: Hypertension is one of the most common diseases in the world that like many chronic diseases decreases the quality of life (QOL). Maintaining and promoting the quality of life in these patients requires support of their families. This research aimed at investigating the effect of family-centered empowerment model on the quality of life and blood pressure levels in patients with hypertension.
Materials and methods: This experimental study was conducted in 120 patients with hypertension, selected by stratified sampling in Gonabad, Iran, 2013. They were randomly divided into a control group and an experimental group. Family-centered empowerment model was implemented in experimental group. Demographic characteristics were recorded and SF-36 was used to measure the QOL. Data was analyzed applying chi-square test, Paired-samples t test and Independent-samples t test.
Results: There was no significant difference between the two groups in demographic characteristics. The two groups did not have significant differences in quality of life before the intervention (P=0.819). The QOL mean score in experimental group increased significantly after the intervention (P=0.001). Before the study, no significant difference was observed between the mean systolic (P=0.555) and diastolic (P=0.358) blood pressure levels. But they lowered significantly after the intervention (P=0.008 and 0.014, respectively).
Conclusion: Our results showed that family-centered empowerment model could enhance quality of life and lower blood pressure in hypertension patients.</CONTENT>

<CONTENT>Background and purpose: Fertility is an effective factor on population growth that its influential factors can be changed over time. This study aimed at surveying some effective social, cultural and economic factors on total fertility rate (TFR) in Kermanshah.
Materials and methods: The study population included women aged 15-49 years old resident in Kermanshah during 1996, 2006, and 2011. Data was obtained from Statistical Center of Iran, National Organization for Civil Registration, and Kermanshah health center. Data was then analyzed by SPSS software using multiple regression.
Results: In 1996, three factors were effective on TFR including: general activity rate (-0.064), literacy rate in society (-0.069), and rate of aging (-0.031). In 2006, six factors were found to be more influential on TFR including women literacy rate (-0.190), illiteracy rate in society (0.146), general activity rate (-0.129), women continuing education (0.50), rate of aging (-0.031), and net dependency rate (0.026). But in 2011, there were 13 effective factors on TFR including young population rate (0.370), continuing education (-0.184), family size (0.256), women's economic participation (-0.191), women unemployment (-0.048), society’s economic participation rate (0.210), general activity rate (-0.965), rate of aging (-0.179), women marriage rate (-0.018), illiteracy rate in society (0.266), literacy rate in society (-0.935), dependency rate (0.145), and women continuing education (-0.065).
Conclusion: Cultural variables on the one hand and economic pressures on households, on the other hand, as the most determinant factors, lead to decline in the total fertility rate in the study period.</CONTENT>

<CONTENT>Background and purpose: Dialectical behavior therapy (DBT) is a new approach that is used in treatment of complicated mental health disorders. This study aimed at assessing the efficacy of DBT on depression.
Materials and methods: This quasi-experimental study included an experimental group (DBT) and a control group. The study population included all mothers having children with mental disorders who were diagnosed with major depressive disorder. Using convenience sampling 16 mothers were selected. Experimental groups were studied in three stages: before the treatment, during training and follow-up. Data was collected through structured clinical interview for one axis disorder and Beck Depression Inventory. Data was analyzed using repeated measures ANOVA and t-test for paired samples.
Results: The results showed significant differences between the level of depression in experimental and control groups after DBT implementation (P&#60;0.001).
Conclusion: Group DBT sessions could be of great benefit in treatment interventions and help in reducing depression in target groups.</CONTENT>

<CONTENT>Schwannomas are benign tumors of the nerve sheath and are usually single encapsulated and slow growing in peripheral or sympathetic nervous system. In this report a 49 year-old man is presented with one year complain of abdominal pain and intermittent hematospermia. The CT scan of the abdomen showed a 60 × 65 × 60 mm mass in anterior pelvic cavity with deviation to the sacral bone, originated from nerve. Several examinations revealed neurofibroma. Due to the large size of the tumor and it's position to the pelvic nerves, to remove the mass the patient only underwent laparotomy with partial resection. Pathology tests confirmed Ancient Schwannoma with degenerative changes. Radiotherapy was done with the aim of reducing the size of the rest of tumor. In our case, schwannoma was diagnosed incidentally. The size of the tumor indicated a relatively long period from the time that tumor was generated until the time of diagnosis. Despite using paraclinical findings, a definite diagnosis of the disease was made by histopathological tests.</CONTENT>

During normal cellular activities Reactive Oxygen Species (ROS) and Reactive Nitrogen Species (RNS) are produced. In addition to beneficial functions they play a critical role in cell death and prevent apoptosis. Every cell is equipped with an extensive antioxidant defense system to combat the excessive production of active radicals. Oxidative stress occurs with destruction of cellular antioxidant defense in overproduction of these reactive species induced by exogenous (e.g. ultraviolet rays) and endogenous (at the cellular level where mitochondria are involved). ROS and RNS can be beneficial or harmful to cells and tissues. At physiological low levels, reactive oxygen species act as redox messengers in intracellular signaling and regulation, whereas excess ROS induce oxidative modification of cellular macromolecules, inhibit protein function, and promote cell death. A full understanding of the redox control of apoptosis induction and inhibition can develop therapeutic interventions associated with oxidative stress-related disorders. In this review, major sources of ROS and RNS in a cell, the role of these reactive species
in normal physiology and carcinogenesis and the role of antioxidants as regulators of oxidative stress
and apoptosis are described. Finally, the relationship between oxidative stress and cell death are demonstrated.</CONTENT>
