fa ارزیابی شیوع ضایعات دهانی در بیماران مبتلا به کولیت اولسراتیو: یک مطالعه ی مقطعی Assessment of the Prevalence of Oral Lesions in Patients with Ulcerative Colitis: A Cross-Sectional Study دندانپزشکی Dentistry پژوهشي-کامل Research(Original) سابقه و هدف: کولیت اولسراتیو یکی از شایع‌ترین انواع بیماری‌های التهابی روده است که علاوه بر درگیری دستگاه گوارش، با تظاهرات خارج‌ روده‌ای متعددی همراه می‌باشد. حفره دهان به‌عنوان بخشی از دستگاه گوارش، به‌طور شایع درگیر شده و ضایعات دهانی می‌توانند همزمان با فعالیت بیماری ظاهر، و یا شدت یابند. از آن‌جا که سلامت دهان به‌عنوان شاخصی از سلامت عمومی و ابزاری بالقوه برای تشخیص زودهنگام بیماری‌های سیستمیک شناخته می‌شود، شناخت الگوی ضایعات دهانی در این بیماران از اهمیت بالینی و پیشگیرانه برخوردار است. مطالعه حاضر با هدف ارزیابی شیوع ضایعات دهانی در بیماران مبتلا به کولیت اولسراتیو، انجام پذیرفت. مواد و روش ‌ها: این مطالعه به‌صورت توصیفی–تحلیلی مقطعی، بر روی 82 بیمار مبتلا به کولیت اولسراتیو مراجعه‌ کننده به بیمارستان امام خمینی ساری در سال 1403 انجام شد. بیماران بر اساس شاخص فعالیت بالینی (CAI) Rachmilewitz به دو گروه بیماری فعال و غیرفعال تقسیم شدند و شدت بیماری طبق طبقه‌بندی Truelove و Witts تعیین گردید. معاینه دهان توسط دندان ‌پزشک آموزش‌ دیده انجام و ضایعات دهانی بر اساس ابزار استاندارد Thamson Murray ثبت شد. داده‌ها با آزمون کای‌دو در SPSS 22 تحلیل و سطح معنی‌داری 0/05< P در نظر گرفته شد. یافته‌ها: میانگین سنی بیماران 44/99 سال با انحراف معیار 8/18  بود. 57/3  درصد بیماران حداقل یک نوع ضایعه دهانی داشتند. خشکی دهان (58/5 درصد) شایع‌ترین یافته بود که شیوع آن ارتباط معناداری با شدت و وضعیت فعالیت بیماری نشان داد (0/05<‌P). سایر ضایعات شامل زخم‌های مخاطی و اریتم نیز مشاهده شد. استنتاج: ضایعات دهانی، به‌ویژه خشکی دهان، در بیماران مبتلا به کولیت اولسراتیو شیوع بالایی دارد. بررسی منظم تظاهرات دهانی می‌تواند به عنوان بخشی از روند پایش و مدیریت این بیماران در نظر گرفته شود و ممکن است به تشخیص زود هنگام دوره‌های فعال بیماری کمک کند.   Background and purpose: Ulcerative colitis (UC) is one of the most common types of inflammatory bowel disease, affecting the gastrointestinal tract and often presenting with extraintestinal manifestations. The oral cavity is commonly involved, and oral lesions may appear or worsen alongside the activity of the disease. Since oral health serves as an indicator of general health and a potential tool for the early detection of systemic conditions, understanding the pattern of oral manifestations in UC patients is clinically and preventively important. Materials and methods: This cross-sectional descriptive-analytical study was conducted on 82 UC patients who visited Imam Khomeini Hospital in Sari in 2024. Patients were classified into active and inactive disease groups based on the Rachmilewitz Clinical Activity Index (CAI), and disease severity was determined according to Truelove and Witts criteria. Oral examinations were performed by a trained dentist, and lesions were recorded using the standardized Thomson–Murray questionnaire. Data were analyzed using the Chi-square test in SPSS version 22, with a significance level set at p<0.05. Results: The mean age of participants was 44.99 ± 8.18 years. At least one oral lesion was observed in 57.3% of patients. Xerostomia (58.5%) was the most prevalent manifestation, showing a significant association with disease severity and activity (p<0.05). Other lesions included mucosal ulcers and erythema. Conclusion: Oral lesions, particularly dry mouth, are common in UC patients.
