fa مقایسه غلظت Zinc و کورتیزول سرم بین دو گروه بیماران با اختلال افسردگی عمده تحت درمان با فلوکستین و درمان شناختی رفتاری: کارآزمایی بالینی تصادفی شده Comparing Serum Zinc and Cortisol Concentrations in Patients with Major Depressive Disorder Treated with Fluoxetine and Cognitive-Behavioral Therapy: A Randomized Clinical Trial روانپزشکی psychiatry پژوهشي-کامل Research(Original) سابقه و هدف: اختلال افسردگی عمده (MDD) یک چالش بزرگ برای سلامت روان جامعه است. Zinc و کورتیزول سرم نشانگرهای زیستی مناسبی برای پاسخ به درمان می‌باشند. هدف مطالعه حاضر مقایسه نشانگرها در بین دو گروه بیماران MDD تحت درمان با فلوکستین تراپی و درمان شناختی رفتاری انجام پذیرفت. مواد و روش‌ها: در این مطالعه کارآزمایی بالینی تصادفی شده، 40 بیمار زن MDD (معیارDSM-V) در دوگروه فلوکستین‌تراپی (روزانه 20 میلی‌گرم به‌مدت 3 ماه) (20N=) و CBT گروهی (جلسه 90 دقیقه‌ای، یکبار در هفته، به مدت 12 هفته) (20N=) جای گرفتند. شدت افسردگی، غلظت سرمی کورتیزول و Zinc در ابتدا و پایان درمان سنجش شدند. داده‌ها در سطح معنادار 0/05≥P با کمک نرم‌افزار Graphpad Prism9 آنالیز شدند. یافته‌ها: نمره BDI-II در هر دوگروه فلوکستین (از1/5 ± 23/2 به 1/1 ± 11/1) و CBT (از 0/6 ± 26/8 به 0/8 ± 9/5) کاهش معناداری داشت. میزان کورتیزول سرم در گروه فلوکستین بدون تغییر و درگروه CBT (از0/09 ± 453 به 0/07± 386/4) کاهش داست. غلظت Zinc سرم در گروه فلوکستین (از 4/8 ±91/6 به 2/8 ± 72/8) کاهش و در گروه CBT (از 2/1 ± 78/9 به 2/6 ± 86/5) افزایش داشت. از نظر مقایسه بین گروهی مقادیر بعد از درمان، میزان کاهش نمره BDI-II و غلظت کورتیزول در گروه CBT نسبت به گروه فلوکستین‌تراپی بیش‌تر بود. استنتاج: امتیاز BDI–II، کورتیزول سرم در گروه CBT کاهش بیش‌تر اما Zinc سرم افزایش بیش‌تر نسبت به گروه فلوکستین داشتـه است. بنابراین برای درمـان MDD مناسب به نظر می‌رسد. مطالعه با حجم بیش‌تر و دوره طولانی‌تری توصیه می شود. شماره ثبت کارآزمایی بالینی : 1N20190710044171IRCT Background and purpose: Major depressive disorder (MDD) is a massive challenge for community mental health. Serum zinc and cortisol are suitable biomarkers for assessing the response to a given treatment. In the present study, serum concentrations of these markers were compared between two groups of patients treated with fluoxetine and cognitive behavioral therapy. Materials and methods: In this randomized clinical trial study, 40 female MDD (DSM-V criteria) patients were divided into two groups: fluoxetine therapy (20 mg/daily/3 months, n=20) and group CBT (90-min/once a week/12 weeks, n=20). Severity of depression was studied by Beck Depression Inventory (BDI-II) and serum cortisol and zinc concentrations were measured before and after treatments. Data were analyzed in Graphpad Prism 9. Results: BDI-II scores significantly reduced in both fluoxetine therapy (from 23.2±1.5 to 11.1± 1.1) and CBT (from 26.8±0.6 to 9.5±0.8) groups after treatments. Serum cortisol level did not change in patients treated with fluoxetine, but it decreased in CBT group (from 453 ± 0.09 to 386.4±0.07). Serum zinc level decreased in the fluoxetine group (from 91.6±4.8 to 72.8±2.8) and increased in the CBT group (from 78.9±2.1 to 86.5±2.6). Post intervention inter-group comparison showed greater decrease in BDI-II score and cortisol concentration in CBT group than the fluoxetine therapy group. Conclusion: The decrease in BDI-II score and cortisol level and increase in serum zinc were higher in CBT group than fluoxetine group. Therefore, CBT seems to be suitable for treatment of MDD. Further studies are needed with larger sample size and longer study periods.
