fa بررسی ارتباط بین وضعیت مزاج کلی، مزاج رحم و نمره آپگار با جفت اکرتا در مادران باردارکاندید جراحی سزارین: یک مطالعه مورد-شاهدی Relationship between General Temperament (Mizaj), Uterine Temperament, and Apgar Score with Placenta Accreta in Pregnant Mothers Candidates for Caesarean Section: A Case-Control Study زنان و زایمان و نازایی Gynecology and Infertility پژوهشي-کامل Research(Original)  سابقه و هدف: مزاج در طب ایرانی، مفهوم مهمی است که کاربردهای فراوانی برای تشخیص و پیشگیری انواع بیماری‌ها، مراقبت و درمان هر فرد دارد. هدف این مطالعه تعیین ارتباط بین وضعیت مزاج کلی و مزاج رحم و آپگار نوزادان با جفت اکرتا در مادران باردار کاندید جراحی سزارین بود. مواد و روش‌ها: در این مطالعه مورد-شاهدی، جامعه پژوهش شامل مادران کاندید سزارین بود، که در دوگروه مبتلا به اکرتا(33 نفر) و گروه شاهد(66 نفر) قرار گرفتند. مزاج کلی فعلی و مزاج رحم با پرسشنامه استاندارد تعیین مزاج کلی مجاهدی و مزاج رحم تن‌ساز و آپگار نوزادان با استفاده از چک لیست استاندارد جهانی آپگار اندازه‌گیری شد. برای مقایسه یافته‌ها از آزمون تی‌تست و کای دو استفاده ‌شد. یافته‌ها: ویژگی دموگرافیک دو گروه مشابه بود. در گروه اکرتا، میانگین نمره سردی و گرمی کلی 1/55±14/81 و تری خشکی 1/23±3/72 و در گروه شاهد به‌ترتیب 4/38±16/81 و 0/65±0/39 بود(0/002>P). میزان شانس ابتلا به اکرتا در افراد سرد مزاج نسبت به گرم مزاجان 8/571 و شانس ابتلا به جفت اکرتا در مزاج رحم سرد نسبت به مزاج رحم گرم 15/385 برابر و مزاج رحم خشک نسبت به مزاج تر، 10/35 برابر بود، هم‌چنین میانگین نمره مزاج سردی و گرمی رحم در گروه اکرتا 2/21±33/27 و تری وخشکی 1/58±11/45 و در گروه شاهد به‌ترتیب 5/20±39/60 و 2/16±13/71 بود (0/001>P). استنتاج: شانس ابتلای مادران کاندید سزارین با مزاج کلی و مزاج رحمی سرد به اکرتا بیش‌تر از سایر وضعیت‌های مزاجی است. هم‌چنین شانس ابتلا در مادران با مزاج کلی تر و مزاج رحم خشک بیش‌تر می‌باشد. به‌کارگیری تدابیر لازم بر مبنای اصول طب سنتی جهت مادران با ویژگی‌های مزاجی مستعد، در مراحل اولیه بارداری پیشنهاد می‌شود.    Background and purpose: Mizaj (temperament) is an important concept in Persian medicine that has many applications in the diagnosis, prevention, care, and treatment of many diseases. This study aimed to determine the relationship between placenta accreta and the state of the general and uterine temperament and Apgar score of newborns in mothers candidates for cesarean section. Materials and methods: A case-control study was performed in mothers who were candidates for caesarean section. The subjects included expectant mothers with placenta accreta (n= 33) and a control group (n= 66). Current general and uterine temperament were measured using Mojahedi and Tansas questionnaire. Demographic variables and Apgar score of newborns was measured using the Apgar International Standard Checklist Score. T-test and Chi-square were used to compare the data. Results: Demographic characteristics were similar between the two groups. In pregnant mothers with placenta accreta the mean scores for total warm-cold and wet-dry temperaments were 14.81±1.55 and 3.72±1.23, respectively. In the control group, these scores were 16.81±4.38 and 3.39±0.65, respectively (P<0.002). The chance of placenta accreta in cold-tempered mothers was 8.571 compared to warm-tempered mothers. The chance of placenta accreta in pregnant women with cold-tempered uterine was 15.385 times higher compared to pregnant women with warm-tempered uterine. Those with dry uterus were 10.35 times more likely to develop placenta accreta compared to mothers with wet temperament. The mean score for warm-cold tempered uterine in the placenta accreta group was 33.27±2.21 and for wet-dry tempered uterine was 11.45±1.58 and in the control group the scores were 39.60±5.20 and 13.71±2.16, respectively (P<0.001). Conclusion: The chances of developing placenta accreta is higher in mothers candidate for Caesarean section with general and cold uterine temperament than in other temperamental conditions. Also, the chance of contracting placenta accreta is higher in mothers with wet general temperament and dry uterine temperament.
