fa جداسازی و بررسی خصوصیات باکتریوفاژهای لیتیک موثر جهت کنترل عفونت‌های حاصل از باکتری‌های کلبسیلا پنومونیه مقاوم به آنتی‌بیوتیک در شمال غرب ایران Isolation and Characterization of Effective Lytic Bacteriophages to Control the Antibiotic Resistant Klebsiella Pneumoniae Infections in Noth-West of Iran ميکروبيولوژي Microbiology پژوهشي-کامل Research(Original) سابقه و هدف: با ظهور باکتری‌های مقاوم به آنتی‌بیوتیک، یافتن مواد ضد‌میکروبی جایگزین موثر برای مبارزه با این باکتری‌ها اجتناب‌ناپذیر می‌باشد. باکتریوفاژ‌درمانی به‌عنوان یک رهیافت بیولوژیک دارای پتانسیل درمانی بالا اخیرا در این زمینه مورد توجه قرار گرفته است. این مطالعه با هدف جداسازی باکتریوفاژهای لیتیک علیه ‌جدایه‌های بالینی کلبسیلا پنومونیه دارای مقاومت‌های چندگانه آنتی‌بیوتیکی و ارزیابی خصوصیات زیستی و شکلی آن‌ها به‌عنوان کاندیداهایی برای فاژ درمانی، انجام پذیرفت. مواد و روش‌ها: در این مطالعه توصیفی- مقطعی، جدایه‌های بالینی به‌دست آمده از بیمارستان شهید مدنی شهر تبریز با استفاده از تست‌های استاندارد بیوشیمیایی به‌عنوان کلبسیلا پنومونیه تایید شدند و سنجش حساسیت آنتی‌بیوتیکی آن‌ها با استفاده از روش استاندارد انتشار از دیسک انجام شد. سپس جداسازی باکتریوفاژهای لیتیک علیه 16 جدایه باکتریایی مقاوم به آنتی‌بیوتیک صورت گرفت. به‌طور خلاصه نمونه‌های آب اخذ شده جهت جداسازی فاژ از فاضلاب سانتریفیوژ گردید. مایع رویی با فیلتر 45/0 میکرومتر فیلتر شد و با محیط کشت تریپتون سوی براث تازه و سولفات منیزیم مخلوط شد و در دمای اتاق انکوبه‌گردید. سپس محلول موردنظر با کلروفورم مخلوط شده و ارزیابی تولید پلاک با استفاده از روش آگار دو لایه انجام شد. ژنوم باکتریوفاژ با استفاده از آنزیم‌های HindIII و EcoRV تحت روند آنالیز مولکولی قرار گرفت. تعیین طیف میزبانی فاژهای جداسازی شده با استفاده از روش نقطه‌ای و آگار دو لایه انجام گرفت. سه باکتریوفاژ دارای طیف میزبانی وسیع‌تر برای بررسی بیش‌تر خصوصیات آن‌ها انتخاب شدند. منحنی رشد یک مرحله‌ای فاژها ترسیم گردید و زمان نهفته و سایز انفجاری آن‌ها مشخص شد سپس پایداری و زنده مانی باکتریوفاژهای لیتیک در برابر دماهای مختلف (20، 37-، 50 درجه سلسیوس)، محدوده‌های مختلف pH (3، 4، 5، 7، 9، 11و12) و هم‌چنین تشعشع UV بررسی گردید. در نهایت خصوصیات شکلی باکتریوفاژ دارای بیش‌ترین طیف میزبانی (PKpMa2/20) با استفاده از میکروسکوپ الکترونی مشخص شد. یافته‌ها: 36 جدایه بالینی باکتریایی براساس تست‌های استاندارد بیوشیمیایی به‌عنوان کلبسیلا پنومونیه تایید تشخیص شدند. در آزمایش سنجش حساسیت آنتی‌بیوتیکی باکتری‌ها به ترتیب بیش‌ترین و کم‌ترین مقاومت آنتی‌بیوتیکی نسبت به سفازولین (80 درصد) و توبرامایسین (26 درصد) مشاهده گردید. سپس جداسازی باکتریوفاژها علیه همه 16 جدایه منتخب با موفقیت انجام شد. بعد از استخراج ژنوم فاژهای جدا شده تنوع توالی بالایی مشخص گردید. در مورد تعیین طیف میزبانی، PKpMa2/20 دارای اثر کشندگی روی 11 جدایه از 16 جدایه مذکور بود. در مطالعه منحنی رشد یک مرحله‌ای سه فاژ دارای طیف میزبانی بیش‌تر، زمان نهفته سه فاژ جداشده به‌ترتیب 10، 10 و 15 دقیقه و سایز انفجاری آن‌ها به ترتیب 350، 210 و 180 برای PKpMa2/20، P8 و P10 ارزیابی گردید. هر سه فاژ پایداری خوبی در دمای بین 20- و 37 درجه سلسیوس نشان دادند. در دمای 50 درجه سلسیوس اثر لیتیک فاژ PKpMa2/20 بیش‌تر از دو فاژ دیگر بود. فاژ PKpMa2/20 در بازه pH بین 3 تا 12 فعال بود در حالی که دو فاژ دیگر در میزان pH برابر با 3 کاملا از بین رفتند. در بررسی میکروسکوپ الکترونی، باکتریوفاژ PKpMa2/20 یک دم کوتاه با طول 10 نانومتر و یک سر با قطر 60 نانومتر داشت که احتمالا متعلق به خانواده پودوویریده بود. استنتاج: با توجه به اثرات لیتیک مناسب و نیز زمان نهفته و سایز انفجاری خوب سه فاژ منتخب جدا شده در این مطالعه به خصوص (PKpMa2/20) و هم‌چنین پایداری مناسب آن‌ها در برابر دماهای مختلف و محدوده pH مختلف و نیز تشعشع UV، این فاژها می‌توانند به‌عنوان کاندیدا جهت استفاده در فاژ درمانی برای کنترل سویه‌های مقاوم کلبسیلا پنومونیه عفونی مورد توجه قرار گیرند، اما جهت استفاده درمانی نیاز به تجزیه و تحلیل مولکولی بیش‌تری می‌باشد.   Background and purpose: Due to emerging antibiotic-resistant bacteria, finding effective alternative antimicrobials to combat the bacteria is unavoidable. Bacteriophage therapy as a biologic and highly potentiated therapeutic approach was considered recently. This study aimed to isolate lytic bacteriophage(s) against multi-drug resistant clinical isolates of Klebsiella pneumonia and evaluate the morphological and biological characteristics as candidates for phage therapy. Materials and methods: Obtained clinical isolates from Shahid Madani Hospital in Tabriz, were confirmed as K. pneumoniae, and an antibiotic sensitivity test was performed using the standard disk diffusion method. Then, the isolation of lytic bacteriophages against 16 selected antibiotic-resistant bacterial isolates was carried out. Briefly, samples for phage isolating were gained from sewage water and centrifuged. The supernatant was filtered using a 0.45 µm Filter, mixed with fresh tripton soy broth medium and magnesium sulfate and incubated at room temperature. Then, the solution was mixed with chloroform and evaluation of plaque formation was performed using the agar bilayer technique. Genomes of the bacteriophages were subjected to molecular analysis using Hind III and EcoRV enzymes. Host range determination of the isolated bacteriophages was evaluated using spot test and agar bilayer techniques. Three bacteriophages with a broader host range than others were selected for more characterization. One step growth curve of the phages was drawn and their latent period and burst size were investigated. Then, the stability and survivability of the isolated lytic bacteriophages to different temperatures (-20, 37, 50℃), different rates of pH value (3,4,5,7,9,11,12), and also UV radiation were evaluated. Finally, morphological characteristics of bacteriophage with the broadest host range (PKpMa2/20) were investigated using electron microscopy. Results: 36 clinical bacterial isolates were confirmed as K. pneumoniae based on standard biochemical tests. In the antibiotic susceptibility assay, it was revealed that the highest and lowest rates of antibiotic resistance belonged to cefazoline (80%) and tobramycin (26%) respectively. Isolation of bacteriophages against all 16 selected isolates was successfully performed. After the extraction of the genomes of the phages, a high variety of genome sequences was detected according to molecular analysis. In host range determination, PKpMa2/20 had lytic activity against 11 of 15 isolates. In a study of the one-step growth curve of the phages, with a broader host range than other isolated bacteriophages, latent periods were shown 10, 10, and 15 min and their burst size were 350, 210, and 180 pfu/ml respectively. All of the phages showed appropriate stability in temperatures of -20 and 37 ℃. In the temperature of 50℃, the survivability of PKpMa2/20 was higher than two other phages. Also, PKpMa2/20 was stable in the pH range of 3-12; in the pH value of 3, two other phages stability was ceased but PKpMa2/20 showed appropriate lytic activity.
