fa وضعیت تولید آنتی‌بادی و ماندگاری آن به دنبال ابتلا در بیماران سرپایی مبتلا به کووید-19 The Status of Antibody Production and its Stability after Infection in Outpatients with Covid-19 ايمونولوژي Immunology پژوهشي-کامل Research(Original) سابقه و هدف: کووید-19 یک بیماری ویروسی دستگاه تنفس است. با توجه به این‌که میزان ماندگاری آنتی‌بادی بر علیه کووید-19 در گزارشات متفاوت بوده است و از آن‌جایی‌که تفاوت‌های قومی و نژادی در این بیماری تاثیر گذار بوده است این مطالعه با هدف تعیین بررسی وضعیت آنتی‌بادی‌ها و ماندگاری در بیماران مبتلا به کووید-19، انجام پذیرفت. مواد و روش‌ها: این مطالعه طولی، در سال 1399 بر روی بیماران مراجعه‌کننده به کلینیک تب در شهرستان میاندورود انجام شد. تشخیص بیماری بر اساس شرح و تست Real time PCR یا سی‌تی اسکن ریه بود 87 بیمار براساس معیارهای تشخیصی وارد مطالعه شدند و معیار خروج شامل موارد عدم مراجعه در پیگیری بود. سطح آنتی‌بادی IgM و IgG توسط روش الیزا و کیت پیشتاز طب در ابتدای تشخیص اندازه‌گیری گردید و برای بررسی ماندگاری مجددا 6 ماه بعد سطح IgG همان بیماران مجدد چک گردید که براساس دستورالعمل کیت مقادیر بالاتر از 1/1 مثبت در نظر گرفته شد. اطلاعات شامل اطلاعات زمینه‌ایی، درگیری ریه در CT اسکن و نتایج الیزا پس از ورود به کامپیوتر با نرم‌افزار آماری 21 SPSS و آزمون‌های T-test و Fisher's exact مورد تجزیه و تحلیل قرار گرفت. سطح معنی‌داری 0/05P< در نظر گرفته شد. یافته‌ها: میانگین سنی بیماران مورد بررسی 17±7/47 سال بود. از افراد مورد بررسی 55 نفر (63 درصد) زن، 20 نفر (23 درصد) افزایش فشارخون و 18 نفر (20/7 درصد) دیابت داشتند. 84 نفر از بیماران تست Real time PCR آن‌ها مثبت بود 43 نفر (49/4 درصد) درگیری CT داشتند. IgM اولیه در 20 نفر (23 درصد)، IgG اولیه در 58 نفر (66/7 درصد) مثبت بود که بعد از شش ماه IgG در 36 مورد (41/4 درصد) مثبت بود. پایداری آنتی‌بادی در 67/4 درصد مردان و 56/4 درصد زنان دیده شد(0/41=P). ابتلا به فشارخون (0/329=P) و دیابت (0/21=P) و نیز درگیری ریه در سی‌تی‌اسکن (0/586=P) ارتباط معنی‌داری با پایداری آنتی‌بادی نداشت. میانگین سنی افراد با پایداری آنتی‌بادی (16/2±50 سال) و افرادی که آنتی‌بادی در آن‌ها منفی شده بود (14/1±42 سال)، اختلاف معنی‌داری (0/077=P) نداشت. استنتاج: یافته‌ها نشان داد آنتی‌بادی علیه بیماری در یک سوم بیماران تولید نشده بود و بعد از گذشت 6 ماه میزان IgG در بخش قابل ملاحظه‌ای از بیماران کاهش یافته بود. هم‌چنین نتایج نشان داد پایداری آنتی‌بادی ارتباط معنی‌داری با سن، جنس، درگیری ریه در سی‌تی اسکن و بیماری زمینه‌ایی از قبیل فشارخون و دیابت نداشت. Background and purpose: Covid-19 is a viral disease of the respiratory system. Considering that the persistence of antibodies against COVID-19 has been different in the reports and since ethnic and racial differences have been effective in this status, the purpose of this study was to examine the status of antibodies and persistence in patients with COVID-19. Materials and methods: This longitudinal study was conducted in 2019 on patients referred to a fever clinic in Miandorod City. The diagnosis of the disease was based on the history and real-time PCR test or lung CT scan. 87 patients were included in the study based on the diagnostic criteria, and the exclusion criteria included cases of non-return in the follow-up. The level of IgM and IgG antibodies was measured by ELISA method and Pishtaz Teb kit at the beginning of the diagnosis, and to check the stability, the IgG level of the same patients was rechecked 6 months later, and according to the instructions of the kit, values higher than 1.1 were considered positive. Data including background information, lung involvement in CT scan, and ELISA results were analyzed after entering the computer with SPSS 21 statistical software and T-test and Fisher's exact tests. P<0.05 was considered a significance level. Results: The mean age of the patients was 47.7±17 years. 55 people (63%) were women, 20 people (23%) had high blood pressure and 18 people (20.7%) had diabetes. 84 patients had positive real-time PCR tests, and 43 patients (49.4%) had CT lung involvement. Initial IgM was positive in 20 people (23%), initial IgG was positive in 58 people (66.7%), and after six months IgG was positive in 36 cases (41.4%). Antibody was stable in 67.4% of men and 56.4% of women (P=0.41). High blood pressure (P=0.329) diabetes (P=0.21) and lung involvement in CT scan (P=0.586) had no significant relationship with antibody stability. The average age of people with stable antibodies (50 ± 16.2 years) and those whose antibody was negative (42±14.1 years) had no significant difference (P=0.077). Conclusion: The findings showed that the antibody against the disease was not produced in one-third of the patients and after 6 months, the amount of IgG had decreased in a significant part of the patients.
